This randomized, open-label, parallel-group study will compare the efficacy and safety of Crisaborole 2% ointment and Tacrolimus 0.1% ointment in children with mild to moderate atopic dermatitis. A total of 66 participants will be randomized (1:1) to receive either Crisaborole or Tacrolimus, applied twice daily to affected areas for 28 days. Primary endpoint is proportion of participants achieving Investigator's Static Global Assessment (ISGA) success (clear/almost clear with ≥2-point improvement from baseline) at Day 28. Safety assessments include adverse events and local application site reactions.

Comparative Evaluation of the Safety and Effectiveness of Crisaborole Ointment (2%) Versus Tacrolimus Ointment (0.1%) for the Topical Treatment of Atopic Dermatitis

Atopic dermatitis (AD), commonly known as eczema, is a chronic skin condition that causes itchy, red, and scaly patches on the skin. It often starts in childhood but can continue into adulthood, affecting a person's quality of life. People with AD may also suffer from asthma or allergies, as these conditions are linked. The exact cause is unclear, but it involves a mix of genetics, immune system problems, and environmental factors** like pollution or dry weather.
In developed countries, about 20% of children and 1-10% of adults have AD, and cases have been increasing over the years. In Asia, including Pakistan, studies suggest 3.5-20% of children and up to 10% of adults may have it. However, in Pakistan, only 10% of children and 2-5% of adults are officially diagnosed, likely because many cases go *untreated or misdiagnosed. People often try home remedies or self-medication before seeing a doctor, which delays proper care.
Current Treatments and Their Problems
The standard treatment includes:
Moisturizers - Help repair the skin barrier. Topical Steroids (creams/ointments) - Reduce inflammation but can thin the skin if used too long.
Steroid-Free Alternatives - Used to avoid steroid side effects.
Two key steroid-free options are:
Tacrolimus (0.1% ointment) - Works well for moderate-to-severe eczema but can cause burning, itching, and has a **cancer risk warning (though rare).
Crisaborole (2% ointment) - A newer treatment for mild-to-moderate eczema, with fewer side effects but less research on its long-term use.
Why This Study is Needed
Most research on these treatments comes from Western countries, and Pakistani patients may respond differently due to:
Climate* (hot, humid weather affects skin). Pollution and dust, which worsen eczema. Limited access to specialists, leading to late diagnosis. There are no direct comparisons* between tacrolimus and crisaborole in Pakistan, making it hard for doctors to choose the best option.
What This Study Will Do
This research will *compare tacrolimus 0.1% and crisaborole 2% in Pakistani patients with eczema to see:
Which works better? (Measured by clear/almost clear skin after 6 weeks). Which is safer? (Fewer side effects like burning or infections). Which improves quality of life? (Less itching, better sleep). How the Study Will Work
Patients (adults and children over 2 years) with mild-to-moderate eczema will be divided into two groups:
One group uses tacrolimus ointment twice daily. The other uses crisaborole ointment twice daily. Doctors will check their skin at start, 2 ,4 and 8 weeks* to see improvements. Side effects and patient feedback will be recorded. Expected Results Tacrolimus might work better for severe cases but could cause more irritation. Crisaborole may be gentler, especially for children and sensitive skin. The findings will help Pakistani doctors make better treatment choices and improve eczema care in the country.
Why This Matters Eczema is a lifelong condition, and using the wrong treatment can lead to worse flare-ups, infections, or side effects. Since steroids are overused in Pakistan, safer alternatives like tacrolimus and crisaborole need proper testing in local patients. The evolving genetic landscape of ILVEN : A comprehensive review

Abstract:

Advances in genomics have redefined inflammatory linear verrucous epidermal naevus (ILVEN) as a mosaic inflammatory disorder with diverse molecular drivers, enabling more precise diagnostic and therapeutic strategies. This review aimed to summarize the published data on genotype–phenotype correlations and associated targeted therapies in ILVEN and ILVEN‐like disorders. A structured literature search was conducted using PubMed, Embase and Google Scholar, focusing on peer‐reviewed studies describing mutations, pathomechanisms and treatment responses in ILVEN or ILVEN‐mimicking dermatoses. Search terms included “ILVEN”, “mosaic inflammatory dermatoses”, “genetic mutations” and “targeted therapy”. Multiple genes have been implicated in ILVEN. Somatic mutations in CARD14 activate NF‐κB and IL‐12/23/IL‐17 signalling pathways and may be responsive to biologics such as secukinumab and ustekinumab. Mutations in NSDHL and PMVK , involved in cholesterol biosynthesis, are associated with ILVEN‐like presentations and may respond to topical statin/cholesterol therapy. KRT10 mutations, which affect keratinocyte differentiation, show favourable responses to crisaborole and acitretin. GJA1 mutations disrupt connexin 43 function, resulting in ILVEN‐Erythrokeratodermia Variabilis et Progressiva (EKVP) overlap and may respond to retinoids. HRAS mosaicism activates the RAS‐MAPK pathway, supporting the theoretical use of MEK inhibitors. Recessive mosaicism in ABCA12 can produce linear, ILVEN‐like lesions due to defective lipid barrier formation, with potential responsiveness to lipid‐replenishing therapies and IL‐17 blockade. The molecular characterization of ILVEN and related disorders supports a shift toward precision dermatology. As accessibility increases, genotype‐directed therapy may replace empiric approaches with more targeted, effective and personalized management frameworks.

Crisaborole as a Potential Treatment for Pediatric Alopecia Areata: A Case Report and Literature Review

Alopecia areata (AA) is an autoimmune disorder that causes non-scarring hair loss, often affecting pediatric patients, where it can significantly impact both psychological well-being and quality of life. While corticosteroids and other systemic therapies are commonly used to treat AA, their side effects, especially in children, highlight the need for safer alternatives. This case report evaluates the use of crisaborole, a topical phosphodiesterase 4 (PDE4) inhibitor, as a potential treatment for pediatric AA. A 10-year-old female patient, who initially presented with a small patch of hair loss and failed to respond to topical corticosteroid therapy, was switched to crisaborole after concerns about corticosteroid-related side effects. After two weeks of treatment, significant hair regrowth was observed, and a follow-up at three months confirmed full regrowth. The mechanisms through which crisaborole may benefit AA include modulation of inflammation, improvement in blood flow to hair follicles, and promotion of follicular stem cell activity. Crisaborole's safety profile, with minimal systemic absorption, makes it a promising non-corticosteroidal alternative for pediatric patients. This case suggests that crisaborole may be an effective treatment for pediatric AA, but further studies with larger sample sizes and longer follow-up periods are needed to validate these findings.
