Effect of Spacer Use on Exacerbation Risk in High-Risk Older Adults With Chronic Airway Diseases Receiving Triple Therapy: A Multicenter Randomized Controlled Trial

This study evaluates whether adding a spacer device to triple inhaled therapy (ICS/LABA/LAMA via pMDI) can reduce acute exacerbations in elderly patients (≥65 years) with stable chronic airway diseases (COPD or asthma) who are classified as high-risk based on GOLD or GINA guidelines. High-risk is defined as ≥1 hospitalization or ≥2 moderate exacerbations in the past 12 months.
Despite receiving maximum inhaled treatment, these patients often have poor inhaler technique due to age-related limitations. A spacer may improve drug delivery, adherence, and reduce local side effects.
In this multicenter, open-label, randomized controlled trial, 380 participants will be assigned to standard therapy with or without a valved face-mask spacer. The primary outcome is the 3-month incidence of moderate-to-severe exacerbations. Secondary outcomes include lung function, adherence, inhalation technique, side effects, and patient satisfaction.

Drug-Coated Balloon Versus Drug-Eluting Stent in Patient With Premature ST-Segment Elevation Myocardial Infarction

The incidence of premature coronary artery disease (PCAD) is on the rise, and there is a critical need to improve long-term outcomes. Drug-coated balloon (DCB) represent a promising treatment option for patients with PCAD. However, high-quality clinical data on the impact of DCB on the long-term prognosis of myocardial infarction patients are lacking. This study aims to compare the 12-month clinical outcomes, including all-cause mortality, non-fatal myocardial infarction, and any revascularization, of DCB versus drug-eluting stent (DES) in patients with premature ST-segment elevation myocardial infarction (STEMI). The objective is to verify the efficacy and safety of the DCB implant-free regimen in these patients.

A Clinical Study on the Combination of Mitoxantrone Liposome Injection, Bendamustine, Etoposide, and Cytarabine for Pretreatment of Autologous Hematopoietic Stem Cell Transplantation in Patients with Lymphoma

This is a single-arm, open, single-center clinical study. Patients with lymphoma who were to undergo autologous hematopoietic stem cell transplantation were pretreated with mitoxantrone hydrochloride liposome injection combined with standard doses of bendamustine, etoposide, and cytarabine before transplantation, in order to explore the safety and efficacy of this combined pretreatment regimen.
Tests were performed during the study to observe efficacy, safety, and tolerability. The treatment period was from pre-treatment to +28 days after transplantation and +28 days after stem cells were transfused. The follow-up period was followed up once a month for 6 months, every 3 months for 6 months to 2 years, and every 6 months for 2 years to 3 years after stem cell reinfusion. All subjects underwent protocol-mandated examinations during treatment to observe safety, tolerability, and efficacy.

Care-seeking and treatment pathways of multidrug-resistant tuberculosis patients: an analysis of real-world data from regional health information system in Ningbo City in Eastern China

OBJECTIVES:

The patient pathway of multidrug-resistant tuberculosis (MDR-TB) in China is an essential but not well-studied area. This study aimed to understand the alignment between patient-initiated care-seeking demands for MDR-TB and the availability of diagnostic and treatment services in Ningbo, a city in eastern China, using patient pathway analysis (PPA).

METHODS:

We collected the diagnostic and treatment data of 240 patients with MDR-TB in Ningbo from 2015 to 2019. Using patient pathway analysis, we matched the medical data of patients from different medical institutions and mapped their care pathways to illustrate their access to medical services.

RESULT:

Our study indicated that the proportion of patients with MDR-TB who chose non-TB-designated medical institutions (55%) was higher than those who chose TB-designated medical institutions (45%) at their initial visit. An estimated 69% of patients with MDR-TB patients received initial TB screening services during their first visit. In this study, 47% of patients needed to visit 4-7 medical institutions to be diagnosed with MDR-TB. Overall, 80% (n = 192) of patients were diagnosed with MDR-TB within four visits, while 13% (n = 30), 4% (n = 10), and 3% (n = 8) of patients were not diagnosed at the fourth visit and remained at level 2, 1, and 0 medical institutions, respectively.

CONCLUSION:

The care-seeking pathway of patients with MDR-TB in Ningbo is complex. This indicates room for improvement in local diagnosis and referral services. There is a need to promote the deployment of MDR-TB screening, diagnosis, and treatment services at lower-level institutions.

Separation and photostability analysis of retinoic acid isomers and their derivatives based on ion mobility mass spectrometry and theoretical calculations

Retinoic acid (RA) regulates cell proliferation, differentiation, and apoptosis and exists in three primary isomers: all-trans-RA (ATRA), 9-cis-RA (9cRA), and 13-cis-RA (13cRA). RA derivatives, such as bexarotene (BT) and arotinoid acid (TTNPB), are used to treat cutaneous T-cell lymphoma and induce cellular pluripotency. Due to their distinct biological activities, accurate identification and analysis of these isomers are crucial. This study developed a rapid and efficient method for analyzing RA isomers and their derivatives using ion mobility mass spectrometry (IM-MS) and theoretical calculations. RA isomers and derivatives exhibit overlapping mobilities and cannot be separated by direct IM-MS. By forming ternary complexes with β-cyclodextrin (β-CD) and metal ions, baseline separation was achieved with a peak resolution (R_p-p) of up to 3.42. Tandem MS and theoretical calculations revealed distinct differences in interactions, spatial structures, and binding energies, enabling mobility-based separation. Photostability tests showed rapid photoisomerization of 13cRA into 9cRA and ATRA within seconds. Analysis of 13cRA soft capsules from four brands demonstrated good stability, with measured content aligning closely with labeled values. This method offers a new approach for accurate RA isomer analysis, with potential applications in pharmaceuticals and food industries.

Oxygen plasma induced MoS2/MoOx heterojunction for high performance SERS application

Surface-enhanced Raman scattering (SERS) is a non-destructive spectroscopic analysis technique, and it is known for its high sensitivity and specificity in the detection of trace molecules. In this paper, a novel MoS₂/MoO_x heterostructure based non-metallic SERS substrate for highly sensitive molecule detection was reported. Experimentally, the MoS₂/MoO_x heterojunction was prepared by partially oxidizing the MoS₂ nanosphere into MoO_x in an oxygen plasma environment (O²⁺). It evidenced that the electronic structure and surface properties of the MoS₂/MoO_x heterojunction could be adjusted by precisely controlling the oxidation time, thereby optimizing the SERS performance. The SERS performance of the substrate was evaluated by using the Rhodamine 6G (R6G) molecules as Raman reporters. It showed that a limit of quantification as low as 10^-8 M and an enhancement factor (EF) of 8.54 × 10⁸ (R6G@10^-8 M) could be achieved on the optimal SERS substrate with the set oxidizing time to 300 s. Ultimately, the optimal SERS substrate was employed for detecting bright blue dyes. The results demonstrated that the lowest detectable concentration reaches as low as 10^-6 M, fulfilling the requirements for food safety assessment.
