The goal of this clinical trial is to learn whether chlorpromazine and promethazine（C+P）is safe in Acute Ischemic Stroke（AIS) patients and determine the maximum dosage. It will also evaluate the preliminary efficacy of C+P in AIS. The main questions it aims to answer are:
What is the optimal dosage of C+P that is safe without causing adverse effects in AIS patients? What is the optimal dosage of C+P that potentially works to treat AIS? Researchers will compare C+P with placebo (saline solution without C+P) to see if C+P is safe and effective in treating Acute Ischemic Stroke.
Participants will:
Receive C+P or placebo at the same time as endovascular thrombectomy begins. Patients will be observed for 72 hours to see if there were any adverse effects related to C+P. Infarct volumes will be evaluated using Computed Tomography. Functional outcomes will be assessed at 90 days.

Retroviral gene transfer is the preferred method for stable, long-term integration of genetic material into cellular genomes, commonly used to generate chimeric antigen receptor (CAR)-T cells designed to target tumor antigens. However, the efficiency of retroviral gene transfer is often limited by low transduction rates due to low vector titers and electrostatic repulsion between viral particles and cellular membranes. To overcome these limitations, peptide nanofibrils (PNFs) can be applied as transduction enhancers. Among these, PNFs derived from the 12-mer peptide EF-C are well-investigated and commercially available. EF-C PNFs enhance transduction by forming EF-C PNFs/virus complexes that overcome electrostatic repulsion through their polycationic surface and interaction with cellular protrusions. However, the safe application of PNFs as transduction enhancers in gene therapeutic applications requires a fundamental understanding of their transduction-enhancing mechanisms, uptake, and degradation. In this study, we demonstrate that EF-C PNFs induce plasma membrane invaginations, increasing the membrane surface for viral attachment and reducing the distance to the nuclear membrane, thereby facilitating viral entry and transport to the nucleus. Furthermore, we identified macropinocytosis as the main entry pathway for EF-C PNFs and their subsequent degradation by lysosomal peptidases. The lysosomal degradation of EF-C PNFs prevents their accumulation as amyloid deposits, mitigating potential side effects and supporting their safe use in clinical applications.

The residues of sedative and anesthetic drugs in aquatic products are an important factor affecting food safety.In order to screen the residues of sedative and anesthesia drugs in aquatic products, a rapid and sensitive method was established using solid phase extraction (SPE) and high-performance liquid chromatog.-tandem mass spectrometry (HPLC-MS/MS).The samples were extracted by 80% aqueous acetonitrile by an alumina column and separated by a C18 column.The mass spectra were determined in pos. ion mode.The standard curves were matrix matched and quantified by the external standard method.The results showed that the linear ranges of the drugs in aquatic products were from 0.5 to 100 μg kg-1.The correlation coefficient (R2) was greater than 0.990 and the detection limits were between 0.001 and 0.322 μg kg-1.The average recovery rate and relative standard deviation were between 78.64% and 112.15% and from 0.41%-8.26%.Overall, the developed method showed promising results for the simultaneous qual. and quant. determination of the drug residues in aquatic products.

Bioactive phosphorus dendrimers deliver protein/drug to tackle osteoarthritis via cooperative macrophage reprogramming

Reprogramming imbalanced synovial macrophages and shaping an immune microenvironment conducive to bone and cartilage growth is crucial for efficient tackling of osteoarthritis (OA). Herein, we present a co-delivery nanosystem based on generation 2 (G2) hydroxyl-terminated bioactive phosphorus dendrimers (G2-OH₂₄) that were loaded with both catalase (CAT) and quercetin (Que). The created G2-OH₂₄/CAT@Que complexes exhibit a uniformly distributed spherical morphology with a size of 138.8 nm, possess robust stability, and induce macrophage reprogramming toward anti-inflammatory M2 phenotype polarization and antioxidation through cooperative CAT-catalyzed oxygen generation, Que-mediated mitochondrial homeostasis restoration, and inherent immunomodulatory activity of dendrimer. Such macrophage reprogramming leads to chondrocyte apoptosis inhibition and osteogenic differentiation of bone mesenchymal stem cells. Administration of G2-OH₂₄/CAT@Que to an OA mouse model results in attenuation of pathological features such as cartilage degeneration, bone erosion, and synovitis through oxidative stress alleviation and inflammatory factor downregulation in inflamed joints. Excitingly, the G2-OH₂₄/CAT@Que also polarized macrophages in adherent effusion monocytes (AEMs) extracted from joint cavity effusions of OA patients to M2 phenotype and downregulated reactive oxygen species levels in AEMs. This study suggests a promising nanomedicine formulation of phosphorus dendrimer-based co-delivery system to effectively tackle OA through the benefits of full-active ingredients of dendrimer, drug, and protein.

The Making of BMS’ Cobenfy: From Alzheimer’s to Schizophrenia

Former Karuna and Lilly executives discuss the “accidental” and “serendipitous” discovery. What began as a therapeutic candidate for Alzheimer’s disease at Eli Lilly completed its development journey Thursday in the hands of Bristol Myers Squibb as Cobenfy, the first novel treatment for schizophrenia in 35 years . Andrew Miller, former CEO of Karuna Therapeutics who co-invented the drug, said this trajectory is not uncommon in neuropsychiatric diseases. “The only treatments we have for schizophrenia was because chlorpromazine, which was an anesthetic drug in the ’50s, was discovered to have antipsychotic effects . . . and that was kind of the parent of all of the currently available treatments for schizophrenia,” Miller told BioSpace . Similarly, he added, the first approved treatment for depression was an anti-tuberculosis medicine. “When you look at the history of psychiatric drug development, the source of innovation, in many respects, is serendipitous, human to human findings,” Miller said. Weight Loss Rivals Were Once Alzheimer’s Partners In the mid-1990s, Lilly and current obesity and diabetes archrival Novo Nordisk were collaborating on a muscarinic acetylcholine receptor stimulator agonist to treat cognitive impairment in Alzheimer’s disease. Acetylcholine is the neurotransmitter through which cholinergic neurons, which are involved in memory and learning, send their messages. Steven Paul People with Alzheimer’s lose these neurons early in the disease process, said Steven Paul, who oversaw the program at Lilly. “So, our idea in those days was if we could stimulate the receptors for acetylcholine, maybe in the absence of those neurons we could still enhance memory and learning,” Paul told BioSpace . This idea birthed xanomeline, an acetylcholine receptor agonist that the partners studied in a large Phase II trial. The results showed xanomeline had a “modest” but “significant” effect in improving memory, Paul said. However, the trial also revealed adverse events such as nausea, vomiting and excess salivation—effects associated with stimulating muscarinic receptors. Ultimately, the companies made the decision not to proceed with xanomeline “because we felt we didn’t have the requisite tolerability safety margin for patients with Alzheimer’s,” Paul said. But, as part of the trial, the companies had also studied xanomeline’s effect on “abnormal behaviors” in Alzheimer’s such as hallucinations and delusions. “The drug seemed to treat these psychotic symptoms, and it seemed to prevent the emergence of these psychotic symptoms in these patients,” Paul said. With the exception of the adverse events, “that was the most striking finding.” Paul and colleagues published their findings in a 1997 paper . “This was by accident,” Paul said. “We didn’t really think this might happen.” A few years later, researchers at Lilly wondered if these same effects might translate to patients with schizophrenia. To answer this question, the company conducted a small study at Indiana University. “And sure enough, the drug seemed to work in those patients as well,” Paul said. But the adverse effects remained, and the companies did not pursue this indication either. KarXT Is Born Enter PureTech Health, where Miller, who was vice president in 2009, had an idea: Instead of tweaking xanomeline for more selectivity and fewer side effects—which Paul said “probably wasn’t going to work anyway”—why not combine it with a muscarinic antagonist molecule that blocks the peripheral side effects while leaving efficacy unaffected? In 2009, PureTech founded Karuna Pharmaceuticals, which later changed its name to Karuna Therapeutics, to take the candidate forward, with Miller at the helm. Andrew Miller “It took a lot of effort to get from that concept to the actual, real matter of KarXT,” Miller said. Perusing the scientific literature, Karuna identified 65 acetylcholine receptor agonist molecules and 114 muscarinic antagonist molecules. “So that gives you 7,410 possible combinations.” Ultimately, Karuna selected 95 parameters, some specific to the agonist molecule, others specific to the antagonist, and still others about how combinations would function. At the end of this process, the researchers stuck with xanomeline as the agonist and selected trospium as the antagonist, and the combination became KarXT. Interestingly, “[Karuna] couldn’t raise any money. There was very little interest,” said Paul, who joined the company as CEO in 2018 after Karuna published Phase I data. “Sometimes these are the best companies, when people turn you down.” The Phase I trial , in which 70 healthy volunteers received either KarXT or xanomeline plus placebo, showed a significant difference in terms of side effects: 64% of patients in the xanomeline/placebo group saw the most common xanomeline-associated side effects vs. 34% in the KarXT cohort. These early results generated some excitement. In 2018, Karuna raised $42 million in a Series A round, followed six months later by a $68 million Series B , which was later topped up to $80 million . That same year, Paul and Miller took the company public . Then, in November 2019, KarXT met the primary efficacy endpoint in the Phase II EMERGENT-1 trial , where it was also found to be generally well-tolerated, “and our stock just went off the roof,” Paul said. According to Biopharma Dive , Karuna’s value increased from around $400 million to nearly $3 billion following this data drop. BMS Buys Karuna for $14B By December 2023, Karuna had two successful Phase III trials under its belt, and BMS came knocking. Paul said they’d actually been in discussions with BMS since the J.P. Morgan Healthcare Conference in January 2023 but it wasn’t until the company submitted the New Drug Application for KarXT in September 2023 that the pharma company was ready to commit to buying Karuna. “I presented to them the story, and we didn’t hear from them afterwards until later that year,” Paul said. “Then, once we filed the NDA with the FDA, we had them and another company kind of beating down our door.” While Paul wouldn’t reveal the name of the other suitor, he did say that Karuna had “narrowed it to a bit of a bake-off with those two companies.” Prior to agreeing to the buyout deal with BMS, Paul said Karuna had intended to launch KarXT itself. “We were building a sales force. We were building a commercial presence.” In the end, however, Karuna decided BMS’ offer was in the best interest of its stakeholders. “When I joined the company, as a private company of maybe $20- $30-, $40 million bucks . . . we went public, and maybe we were worth at most $400 [million]. And then we were offered $14 billion dollars for the company, in less than five years.” He called the decision bittersweet “because if the drug had been approved, if we had ourselves commercialized it, if it became a mega billion-dollar blockbuster drug ourselves, well, the company’s value would be much greater than $14 billion dollars.” Paul calls KarXT “one of my early children” and said he is pleased with its progress. “They’re [BMS] doing a great job with it, so far as I can tell,” he said. While BMS is starting with schizophrenia, KarXT/Cobenfy’s journey may be far from over. A recent post-hoc analysis showed that a subset of patients in the pivotal Phase III EMERGENT-1 and EMERGENT-2 studies with predominantly negative symptoms of schizophrenia—such as apathy and social withdrawal—at baseline saw a larger effect on negative symptoms than the overall trial population. And the drug may also treat the disease’s cognitive symptoms, including poor memory and low attention span, said Paul, adding that another paper on this was recently accepted for publication. In a return to its roots, BMS has also shared plans to test Cobenfy in Alzheimer’s disease psychosis, as well as bipolar disorder. “Our expectation is that KarXT is our first foray into schizophrenia and serious mental illnesses,” Carlos Dortrait, SVP and general manager of U.S. immunology and neuroscience at BMS, previously told BioSpace , “but we are looking at expanding and looking very broadly across many diseases.”

Study eases concern at antipsychotics use in pregnancy

The use of antipsychotics during pregnancy isn't linked to childhood neurodevelopmental disorders or learning difficulties, study suggests -- giving assurance to those concerned about continuing their medications during pregnancy. Antipsychotics -- a branch of medication designed to treat schizophrenia and bipolar disorder -- are important tools for mental health care management. They work by blocking the effect of dopamine, which can help reduce psychotic symptoms such as hallucinations or delusions. These versatile medications are also widely used for other mental health conditions and developmental disorders, like anxiety, depression, autism spectrum disorder, and insomnia. But many women and pregnant people using these medications may feel concerned about the potential risks they pose to their unborn baby. A new international study led by UNSW Sydney, published today in eClinicalMedicine, tracked the long-term risk of a child developing neurodevelopmental disorders and learning difficulties after being exposed to antipsychotics in the womb. The findings show there's little to no increased risk of the exposure leading to intellectual disability, poor academic performance in maths and language, or learning, speech and language disorders. "The findings are really reassuring for both women managing these psychiatric conditions during pregnancy and their providers," says Dr Claudia Bruno, a pharmacoepidemiologist based at UNSW's School of Population Health and lead author of the study. "There's no increased risk when taking the medication during pregnancy, not only for the specific neurodevelopmental disorders that we looked at, but also ADHD and autism as shown in our team's previous studies." This research is the most comprehensive study on antipsychotics and neurodevelopmental outcomes to date: it pulls together nationwide data from Denmark, Finland, Iceland, Norway, and Sweden into a large sample size of 213,302 children born to mothers with a diagnosed psychiatric condition, 5.5 per cent (11,626) of which were prenatally exposed to antipsychotics. These five Nordic countries all have similar health and education systems and keep detailed data on birth records, filled prescriptions, and diagnoses from inpatient and outpatient specialist care, as well as antenatal care. The researchers teamed these data with results from the children's first standardised national school test (similar to Australia's NAPLAN tests), which happens between the ages of 8-10. "It's reassuring that everything points to the same 'no major indication' of increased risks overall," says Scientia Associate Professor Helga Zoega, senior author of the study and pharmacoepidemiologist, also based at UNSW's School of Population Health. "The study builds on our team's previous work that looked at birth outcomes, including serious congenital malformations, where we've seen similar null results. "I think it's important to get excited about null results because this is essential information for the management of serious mental health conditions in pregnancy. It's as equally important as finding an increased risk of outcomes." A gap that big health data is trying to fix While this study is part of a growing body of research about medication safety in pregnancy, there's still a lot left in this field to discover, says A/Prof. Zoega. "This is a hugely understudied area," she says. "Unfortunately, we know way too little about medication safety during pregnancy." One of the reasons so little is known about medicines and pregnancy is that it's simply not feasible -- or in many cases, ethical -- to conduct randomised clinical trials on pregnant women. The potential risks of testing or withholding treatment to the unborn child and mother or pregnant person is often too great. That's where harnessing big data can step in -- although the research isn't as simple as looking at the raw data alone. For example, women treated with antipsychotics during pregnancy were more likely to smoke, have higher BMIs, lower education levels, to be older (35 years or more) and use other medications during pregnancy compared to women who didn't take antipsychotics during pregnancy -- all of which are risk factors that can potentially impact birth outcomes. These circumstances -- called 'confounding factors' -- are accounted for in observational research using careful study design and complex adjusted risk models to make sure the results show the impact of the medication alone. "These types of studies are methodologically tricky, and can take a long time to do," says A/Prof. Zoega. "This study has been in the making for almost 10 years now. "We already know these women are dealing with psychiatric conditions, and by genetic default, their children would be more likely to have psychiatric or neurodevelopmental outcomes. But we're focused on the risks and benefits of the medication treatment in pregnancy, so we use methods to make the comparison groups as similar as possible." The researchers also strengthened their findings by slicing up the data to take a closer look at whether individual medications, trimesters of exposure, and siblings carried higher risk levels. While one antipsychotic, chlorpromazine, showed potential increased links to language and speech delays, these findings were based on small sample sizes of 8-15 children, so more research is needed to investigate this potential link. Other than this anomaly, the results supported the finding that there was little to no increased risk of children prenatally exposed to antipsychotics developing neurodevelopmental disorders or learning difficulties. Looking ahead Dr Bruno is currently involved in two related studies on prenatal medication use and pregnancy outcomes. One explores if there is a relationship between the use of antiseizure medications during pregnancy and child school performance, and the other examines whether taking ADHD medication use and discontinuation during pregnancy on child health outcomes. But she sees many avenues for future research to build on this work, including harnessing more Australian big health data. "There's so much to learn about medication safety in pregnancy," says Dr Bruno. "These women are typically excluded from clinical trials, so there's a real lack of data or evidence. "While these results are highly generalisable to women in Australia, we now have real-world linked Australian data that can start contributing to large-scale international studies like this one which we're very excited for." A/Prof. Zoega co-leads an international research collaboration called International Pregnancy Drug Safety Study (InPreSS), which investigates the safety of medication in pregnancy. She says there's plenty to do in this space. "Antipsychotics are only one class of medications, and we already know that up to 80 per cent of women use at least one prescription medicine during pregnancy. Most often, there's little or no guidance on safety. "There are so many unanswered questions that there's enough for a lifetime of research."
