The goal of this clinical trial is to learn more about using phenazopyridine (Pyridium) for pain control during bladder onabotulinumtoxinA ("botox") injections for surgical management of overactive bladder (OAB) for patients at Montefiore Medical Center in The Bronx. It will also learn about if using the phenazopyridine pill affects how satisfied patients are with their experience, how much pain patients feel afterwards, and if doctors think using the Pyridium pill affected how they performed the procedure. The main question it aims to answer is:
- Is oral phenazopyridine at least as good as intravesical lidocaine is for managing pain with bladder botox injections for OAB in Montefiore patients?
Researchers will compare phenazopyridine to intravesical lidocaine, a liquid put into the bladder, to see if phenazopyridine is at least as good at controlling pain with bladder botox injections as intravesical lidocaine is.
Participants will be assigned, by chance, to receive either the oral phenazopyridine pill or the intravesical lidocaine as their pain control for the procedure.

Management of Lower Urinary Tract Symptoms during the Treatment for Non-Muscle Invasive Bladder Cancer

PURPOSE OF REVIEWThis narrative review aims to report upon the existing treatment evidence and strategies for managing lower urinary tract symptoms (LUTS) during treatment, including transurethral resection and intravesical therapy. This review also attempts to examine novel approaches to mitigate treatment-related lower urinary tract symptoms and improve treatment adherence.RECENT FINDINGSThere is sparse but promising evidence in improving LUTS secondary to intravesical therapy. Oral agents including phenazopyridine and hyaluronic acid, Bacillus Calmette-Guerin dose reduction, and emerging therapies including beta-3 agonists as well as Onabotulinumtoxin A injections all have demonstrated encouraging improvement in LUTS in limited research. Although recent literature explores new medications and potential strategies for managing intravesical therapy-related LUTS, further research is required to establish efficacy and new consensus on treatment strategies. Further research is also required to establish effective LUTS mitigation strategies with other emerging intravesical therapy regimens.

Hyphenated Centrifugal Microsolid Phase Extraction and Centrifugal Acceleration Thin-Layer Chromatography in a Single Device: An Innovative Approach to Perform Consecutive Extraction and Separation

An innovative approach for extracting and separating analytes consecutively from complex matrixes was introduced for the first time. The approach involves applying centrifugal force to a novel circular chip to expedite the process of microsolid phase extraction and centrifugal acceleration thin-layer chromatography, followed by red-green-blue (RGB) analysis as a simple, fast, and in situ detection method. Furthermore, the natural product of rice husk was utilized to create a sorbent with great adsorption capabilities. In this method, curcumin and phenazopyridine were employed as model drugs. The one-variable-at-a-time (OVAT) method was used to study the factors that affect the extraction and separation. Under the optimized conditions, a good relationship was observed between the |R| + |B| intensity and the concentration of analytes within the range of 1.0-10.0 μg/mL (R² = 0.9903) for curcumin and 0.1-7.5 μg/mL (R² = 0.9933) for phenazopyridine in urine samples. Intra- and interday RSDs% (n = 3) were obtained at less than 5.6 and 7.1%, respectively. In urine samples, the limit of detection values was 0.75 μg/mL for curcumin and 0.05 μg/mL for phenazopyridine, and relative recovery values were in the acceptable range of 85.0-100.2%. The suggested approach has the appropriate sensitivity, good accuracy, and acceptable applicability to determine curcumin and phenazopyridine in complicated matrices.

Medications and Acute Hemolysis in G6PD‐Deficient Patients – A Real‐World Study

Many drug labels contain precautions of use in G6PD‐deficient patients due to hemolytic concerns, but much of this is based on scarce clinical, epidemiological, or structural data. In this real‐world study, we aimed to examine if the administration of presumably risky medications for G6PD‐deficient patients was followed by hemolysis. The study is based on data from Clalit Health Services database that provides inclusive health care for more than half of the Israeli population (~ 4.7 million). Within the database, we identified all G6PD‐deficient patients by G6PD <6 U/g Hb. Within the G6PD‐deficient cohort, we identified all hospitalizations with a discharge diagnosis of hemolysis (January 1, 2010 to December 31, 2022), validated the cases, and identified the culprit event. For the rest of the G6PD‐deficient patients with no‐hemolysis, we recorded filled prescriptions of medications listed as presumably risky. We identified 31,962 G6PD‐deficient patients. Within the cohort, there were 71 cases of major hemolysis requiring hospitalization (0.2% of the cohort), of whom 51 (71.8%) had been caused by ingestion of fava beans, six (8.5%) were associated with an infection, and three (4.2%) suggested to be associated with medications (nitrofurantoin, phenazopyridine, and a “pain killer”). Within the 31,875 patients with no major hemolysis, nitrofurantoin has been prescribed safely to 1,366 G6PD‐deficient males and females; hundreds/thousands of G6PD‐deficient patients had been prescribed safely ciprofloxacin, glibenclamide, ofloxacin, phenazopyridine, sulfamethoxazole/cotrimoxazole, sulfasalazine, hydroxychloroquine, glimepiride, mesalazine, and sulfacetamide. In this real‐world study, we are showing that a list of medications, suspected previously as carrying risks for hemolysis in G6PD‐deficient patients, have been prescribed safely to G6PD‐deficient patients, providing reassurance to patients, prescribers, and regulators.
