Wild birds as reservoirs of antimicrobial-resistant enterococci: Genomic surveillance in Gauteng, South Africa, during the migratory season

Wild birds, including migratory species, are understudied reservoirs of enterococci in South Africa. This study investigated the resistome, mobilome, virulome and genetic diversity of enterococci from wild birds frequenting water bodies during the migratory season in Gauteng, South Africa. Faecal samples were collected from five sites between November 2023 and March 2024. Enterococci were isolated on selective media and identified with polymerase chain reaction and Matrix-Assisted Laser Desorption Ionization-Time of Flight Mass Spectrometry, with 48 isolates undergoing whole-genome sequencing analysis: Enterococcus faecalis (E. faecalis) [56.3% (27/48)], Enterococcus faecium (E. faecium) [18.8% (9/48)] and other enterococci [25.0% (12/48)]. Aminoglycoside resistance genes aph(3')-III [11.1% (3/27)] and ant(6')-Ia [7.4% (2/27)] were detected in E. faecalis and linked to Inc18 and RepA_N plasmids. Linezolid resistance was confirmed in an E. faecalis (MIC = 24 μg/mL) and E. faecium (MIC = 8 μg/mL) isolate carrying optrA and rplD_N130K, respectively, with optrA linked to plasmid-derived sequences. A daptomycin resistance-associated liaS_E192G mutation was identified in one E. faecium isolate (MIC = 4 μg/mL). Five potential multidrug-resistant E. faecalis and E. faecium isolates belonged to infrequently described strains. Clinically relevant virulence genes in E. faecalis included: gelE (100%), asa1 [25.9% (7/27)], hylA [59.3% (16/27)], hylB [74.1% (20/27)] and sex-pheromone genes (100%). Phylogenetic analysis revealed wild bird-adapted strains, alongside One Health-relevant E. faecalis ST16 and ST21 and E. faecium ST54 and ST800 lineages, with limited clustering with global isolates. Wild birds harbour enterococcal lineages carrying antimicrobial resistance genes, underscoring surveillance within a One Health framework in low- and middle-income countries.

Comparative outcomes of daptomycin and linezolid in vancomycin-resistant enterococcal intra-abdominal infections with bacteraemia

OBJECTIVE:

We set out to see if daptomycin or linezolid (8-12 mg/kg) is effective for vancomycin-resistant enterococci (VRE) intra-abdominal infections (IAIs) because of its efficacy for VRE bloodstream infections (BSIs).

METHODS:

We conducted a multicentre observational study. Between April 2010 and October 2024, we screened all patients with VRE-BSIs and included those with a documented VRE-IAI treated with daptomycin or linezolid. Limiting enrolment to patients with concurrent bloodstream infections reduced misclassification bias. The primary outcome was 28‑day in‑hospital mortality.

RESULTS:

Of the 184 patients included, 149 received daptomycin and 35 received linezolid. Most patients (98.3%) underwent source control procedures. A total of 96 patients (52.2%) had hepatobiliary IAI, while 88 (47.8%) had non-biliary IAI. The 28-day mortality did not differ significantly between the daptomycin and linezolid groups (29.5% vs. 37.1%; P = .42). Multivariable logistic regression identified independent predictors of mortality, including higher Pitt bacteraemia score (adjusted OR [95% CI], 1.22 [1.06-1.41]; P = .006), lower platelet count (0.97 [0.94-0.99]; P = .02), and malignancy (2.41 [1.10-5.25]; P = .03). Daptomycin was not associated with increased mortality compared with linezolid (adjusted OR [95% CI], 0.63 [0.28-1.44]; P = .27). Daptomycin ≥11 mg/kg was linked to reduced mortality (adjusted OR [95% CI], 0.29 [0.09-0.91]; P = .03), whereas daptomycin 8-11 mg/kg showed no significant difference (0.79 [0.34-1.85]; P = .59). Findings were consistent in an augmented inverse probability weighting analysis.

CONCLUSIONS:

Daptomycin is comparable with linezolid for treating VRE-IAI and BSI, with daptomycin ≥11 mg/kg associated with improved survival. Further investigations are warranted given the small sample size.

Persistent pseudomembranous cystitis associated with Vagococcus lutrae in a setter dog: First clinical report

Urinary tract infections (UTIs) are among the most common diseases in companion animals, but empirical antibiotic treatments without culture often lead to recurrence and resistance. Vagococcus species are rarely reported as pathogens in animals, and Vagococcus lutrae has not previously been described in dogs. We report the first case of V. lutrae-associated cystitis in a dog in Türkiye. An 8-year-old female English Setter developed persistent dysuria and hematuria after surgery for pelvic trauma. Despite multiple antibiotic treatments (enrofloxacin, ceftriaxone, ciprofloxacin, amoxicillin-clavulanic acid), no clinical improvement was observed. On presentation, hematology indicated leukocytosis with regenerative anemia, and ultrasonography revealed bladder wall thickening and intraluminal hyperechoic material consistent with cystitis. Urine obtained by cystocentesis appeared dark red, alkaline (pH 9), and contained struvite crystals and urothelial cells. Urine culture obtained by cystocentesis yielded a pure growth of Gram-positive cocci identified as V. lutrae using MALDI-TOF mass spectrometry. Antibiotic susceptibility testing showed resistance to commonly used antimicrobials, including amoxicillin, ceftriaxone, ciprofloxacin, doxycycline, and gentamicin, but susceptibility to daptomycin, vancomycin, and teicoplanin. Supportive therapy was initiated; however, the patient died on the third day of treatment. This case suggests the potential pathogenic relevance of V. lutrae in dogs and emphasizes the importance of culture and sensitivity testing in recurrent or treatment-resistant UTIs to guide appropriate antimicrobial therapy.

Back in action: Researchers make drug-resistant bacteria vulnerable again

New findings from Scripps Research show that disabling a single bacterial protein can restore vancomycin’s power against resistant bacteria–and point to a new strategy for overcoming antibiotic resistance. June 24, 2026 LA JOLLA, CA—Antibiotic resistance is one of the most urgent threats to global health, linked to an estimated 4.7 million deaths worldwide in 2019 alone. As more bacteria evolve to evade even last-resort drugs, the supply of effective treatments is shrinking. Now, scientists at Scripps Research have discovered a way to restore the effect of one of those critical antibiotics by disabling a key bacterial enzyme. The study, published in Nature Communications on June 16, 2026, focused on vancomycin-resistant Enterococcus faecium, or VREfm, a hospital-acquired infection that is becoming increasingly common. VREfm can resist multiple antibiotics held in reserve for the toughest infections, including its namesake, vancomycin, a potent drug often used to treat life-threatening infections. The researchers wanted to know whether disabling a specific bacterial enzyme called secreted antigen A (SagA) could make VREfm vulnerable to vancomycin again, rendering it treatable without needing a new antibiotic. SagA is found across a wide range of VRE strains, making it an attractive target for therapies designed to work broadly against resistant infections. When the researchers genetically deleted the enzyme from VREfm, the bacteria became markedly more susceptible to vancomycin. “The SagA enzyme remodels the cell wall so that bacteria divide properly,” says senior author Howard Hang, a professor at Scripps Research. “If you disrupt that, the bacteria don’t divide as well, and it makes the bacteria more sensitive to vancomycin.” There was already evidence that genetic deletion of the enzyme makes the bacteria more vulnerable to antibiotics, says Hang. “But the fact that it sensitizes the cells to vancomycin, when the bacteria are already resistant to it, was a surprise.” Deleting SagA had little impact on the bacteria's resistance to other antibiotics such as ampicillin, daptomycin and ceftriaxone. This suggested that the deletion wasn’t just making the bacteria frailer overall, but was specifically exposing the site where vancomycin binds. The strategy also worked in a mouse model of sepsis. The researchers also screened a large chemical library and identified a class of compounds called β-chloroalkenyl sulfonyl fluorides that chemically disable SagA. Their lead compound, called pghi-4, reduced the amount of vancomycin needed to kill VREfm by up to 8-fold when the two were combined. The effect held up across multiple clinical isolates and reduced the bacterial burden in infected mice. SagA belongs to a family of cell-wall-remodeling enzymes called NlpC/P60 peptidoglycan hydrolases that no drug had ever successfully targeted before. “To demonstrate you can pharmacologically target this enzyme family is a big step forward,” says Hang. The approach is part of a promising category of treatments known as antibiotic adjuvants: compounds that aren’t antibiotics themselves but help existing antibiotics work better against resistant bacteria. Proven adjuvants already exist for a narrow range of drugs, but this is the first such helper developed for vancomycin. The study data suggest that pghi-4 and related compounds may target other peptidoglycan-remodeling enzymes beyond SagA. That raises the possibility that bacteria carrying extra copies of SagA-like enzymes could be more vulnerable to this approach, which is significant because VREfm strains tend to carry more of these enzymes. “This is a promising new weapon in the ongoing arms race against antibiotic resistance,” says Hang. The broader lesson, he adds, is that targeting basic aspects of bacterial physiology can overcome antibiotic resistance. The researchers are already working on more potent second-generation derivatives that couple the existing compound with vancomycin. They believe the same strategy could eventually be extended to other combinations of resistant bacteria and antibiotics, including pathogens such as tuberculosis and drug-resistant Staphylococcus aureus. TSRL Contributes to New Publication Highlighting Promising Antibacterial Candidate for Resistant Hospital Infections
