Endotherapy and/or Extracorporeal Shockwave Lithotripsy Versus Conservative Treatment in Painless Chronic Pancreatitis

Chronic pancreatitis (CP) is caused by factors such as genetics and the environment, leading to the destruction of pancreatic tissue, infiltration of inflammatory cells, and progressive fibrosis of the pancreas. As the disease progresses, pancreatic tissue is damaged, resulting in impaired endocrine and exocrine functions, manifesting as symptoms like steatorrhea and diabetes. Pain associated with chronic pancreatitis (PACP) is the most common symptom in CP patients. Approximately 75% of patients initially present with abdominal pain, and previous studies have shown that PACP can occur in 85-97% of CP patients. However, there is a type of pancreatitis where patients do not experience abdominal pain and typically seek medical attention due to routine check-ups or symptoms of pancreatic endocrine and exocrine dysfunction; this particular type of pancreatitis is known as painless CP. A meta-analysis showed that painless CP accounts for about 12% of all CP patients.
In patients with CP, continuous pancreatic damage leads to the destruction of pancreatic tissue, subsequently causing insufficiency in both exocrine and endocrine functions. "Painless" does not equate to "harmless"; patients with painless CP often present with severe symptoms of pancreatic exocrine and endocrine dysfunction at the time of diagnosis. These findings underscore the importance of early identification and active management of patients with painless CP.
Whether active endoscopic intervention is needed for painless CP remains a matter of debate. The European Society of Gastrointestinal Endoscopy (ESGE) suggests endoscopic therapy and/or extracorporeal shockwave lithotripsy (ESWL) as the first-line therapy for painful uncomplicated chronic pancreatitis (CP) with an obstructed main pancreatic duct (MPD) in the head/ body of the pancreas; however, due to the unclear potential benefits (preservation of pancreatic function) of endoscopic and/or ESWL treatment for painless CP, it is not recommended for patients without pain, although the evidence is of low quality.
Although there is currently no consensus on whether endoscopic treatment can protect the pancreatic function of patients with painless chronic pancreatitis, preliminary evidence suggests that endoscopic treatment may have a positive impact on pancreatic atrophy and dysfunction in patients with painless CP. In a retrospective study by Ikeura et al., which included 268 patients with CP and painless pancreatic duct stones, it was found that endoscopic treatment and complete clearance of stones in patients with painless pancreatic duct stones helped maintain the volume of the pancreatic parenchyma.
Although preliminary research suggests that endoscopic treatment may protect pancreatic function in patients with painless CP, it could also trigger pain. A study by Ikeura et al. found that painless CP patients who underwent endoscopic treatment but did not have complete clearance of pancreatic duct stones were at a significantly higher risk of developing pain. This indicates that incomplete endoscopic treatment might lead patients from a painless state to one of pain. However, a study by Amodio et al. suggests that the likelihood of painless CP patients experiencing pain in the short term is low, implying that conservative treatment might be more appropriate.
Current research on the clinical characteristics of painless CP and its response to treatment is relatively limited, necessitating further clinical studies to clarify the impact of ERCP and/or ESWL on these patients. We plan to conduct a randomized controlled trial to assess whether these minimally invasive interventions can improve clinical outcomes for patients with painless CP

Design, synthesis, α-glucosidase inhibitory activity, and molecular docking of novel pyrazolyl–porphyrin hybrids as potential antidiabetic agents

A novel series of A₂B₂ and A₄ type porphyrin derivatives bearing pyrazole moieties was efficiently synthesized and structurally characterized. These pyrazolyl-porphyrins were evaluated for their α-glucosidase inhibitory activity using Saccharomyces cerevisiae α-glucosidase as the target enzyme. In vitro enzyme assays and kinetic studies revealed that compounds 6c, 7b, and 6d exhibited inhibitory effects comparable to the standard drug Acarbose. Notably, compound 6c demonstrated the highest potency, with an IC₅₀ value of (31.36 μM), closely matching that of Acarbose (31.69 μM) under identical experimental conditions. In silico molecular docking studies further supported the biological data, showing that compound 6c interacts with key residues within the enzyme's active site in a binding mode similar to Acarbose. These findings suggest that pyrazolyl-porphyrin hybrids, particularly compound 6c, hold promise as potential α-glucosidase inhibitors for the development of antidiabetic agents.

In silico and in vitro characterization of Phytolacca latbenia (Moq.): QSAR, phytochemical, and toxicological insights into antiglycation and antidiabetic potential

Diabetes and the related comorbidities have been associated with elevated levels of advanced glycation end products (AGEs). The biochemical process of advanced glycation, is believed to be playing a pivotal role in the development of complications. Since there exists a great deal of promise for natural products offering antidiabetic potential, we studied advanced glycation inhibition and anti-diabetic profile of Phytolacca latbenia (Moq). on fractions and the sorted compounds based on the QSAR and molecular docking analysis. The top two bioactive compounds; Kaempferol and Esculentoside G, were further evaluated for the MD simulation studies at 150 ns run, compared with the standard. Among the tested compounds, Kaempferol presented significant binding energies in MM-GBSA (-48.63 Kcal/mol) and MD simulation studies (73 %) with transcriptional regulator 4F5S. Molecular docking studies revealed that kaempferol formed three hydrogen bonds with Val342, Ser343 and Ser453, along with Pi-Pi stacking and Pi-cation interactions with Trp213 and Arg217 residues of the 4F5S protein. Kaempferol also displayed significant α-glucosidase inhibition (IC₅₀ 0.042 ± 2.31 μg/ml) compared to the acarbose (IC₅₀ 0.036 ± 0.31 μg/ml). Almost all of the selected compounds demonstrated adherence to the safety requirements established by ADMET investigation. Liquid-liquid partitioning of the crude methanolic extract with solvents of increasing polarity yielded five solvent fractions;the ethyl acetate fraction (ETOA) obtained by liquid-liquid partitioning of the crude extract with ethyl acetate and water proclaimed substantial results in both the non-oxidative (61 %) and oxidative (58 %) antiglycation assays for thiol group estimation. The ethylacetae fraction (ETOA) demonstrated comparatively strong antioxidant activity, with an IC₅₀ value of 13.25 ± 0.69 μg/ml as determined by the DPPH assay. In α-glucosidase assay, Aqueous fraction demonstrated a considerable inhibition with IC₅₀ value of 0.108 ± 0.32 μg /ml compared to the standard (IC₅₀ 0.083 ± 0.43 μg/ml). The safety assessment revealed a slight decline in HeLa cell viability, dropping from 82 % at a 2.5 % concentration to 69 % at a 10 % concentration over 24 h, relative to the control.Therefore, Phytolacca latbenia (Moq). and its phytocompounds tested inhibit α-glucosidase and Advanced glycation end product-the process that underlie diabetic complications and may therefore holds great promise as therapeutic agent, with no toxicity concern,against diabetes and related comorbidities.

Crosstalk-free dual-mode biosensing in a single MOF for α-glucosidase activity monitoring and inhibitor screening

This study presents a novel crosstalk-free dual-mode biosensing platform based on a single cerium-incorporated metal-organic framework (UiO-66-NH₂(Ce)) for sensitive detection of α-glucosidase activity and inhibitor screening. Unlike traditional dual-mode systems affected by signal interference, this monolithic MOF platform intrinsically produces orthogonal colorimetric and fluorescent responses. Upon α-glucosidase-mediated generation of hydroquinone, simultaneous signal transduction occurs through independent pathways: colorimetric attenuation via suppression of the oxidase-like catalytic process, and fluorescence recovery due to ligand-to-metal charge transfer modulation. Rigorously optimized for clinical application, the platform achieves ultra-low detection limits of 0.0166 U/L in colorimetric mode and 0.0094 U/L in fluorescent mode, surpassing existing methods. The platform demonstrates selectivity against potential interferents and successfully quantifies α-glucosidase in human serum with satisfactory recovery rates. Furthermore, the system facilitates precise inhibitor screening with IC₅₀ values of 8.13 μM for fluorescence-based detection and 10.06 μM for colorimetric detection of acarbose. By integrating self-validating detection and drug evaluation within a single-material architecture, this work establishes a paradigm for designing robust MOF-based biosensors equipped with built-in cross-validation capabilities.

Newly discovered coffee compounds beat diabetes drug in lab tests

Roasted coffee may do more than wake you up—it could help control blood sugar. Researchers discovered several new coffee compounds that inhibit α-glucosidase, a key enzyme linked to type 2 diabetes. Some of these molecules were even more potent than a common anti-diabetic drug. The study also introduced a faster, greener way to uncover health-boosting compounds in complex foods.Three newly identified compounds were found to strongly inhibit α-glucosidase, an enzyme that plays a central role in breaking down carbohydrates during digestion. Because this enzyme directly affects how quickly sugars enter the bloodstream, the discovery points to possible new functional food ingredients aimed at managing type 2 diabetes. Functional foods offer more than basic nutrition. Many contain naturally occurring molecules that may support health, including compounds with antioxidant, neuroprotective, or glucose-lowering effects. Finding these helpful substances is difficult because foods are chemically complex. Older discovery methods can be slow and inefficient, which has pushed researchers to adopt more advanced tools such as nuclear magnetic resonance (NMR) and liquid chromatography-mass spectrometry (LC-MS/MS). These techniques are especially valuable for studying roasted coffee, which contains a wide range of overlapping chemical components. Study Reveals Anti Diabetic Potential in Coffee Researchers led by Minghua Qiu at the Kunming Institute of Botany, Chinese Academy of Sciences, reported their findings in Beverage Plant Research. Their work highlights previously unknown anti-diabetic activity in coffee and adds new insight into its role as a functional food. The team designed a three-step, activity-focused process to uncover bioactive diterpene esters in roasted Coffea arabica beans. Their approach aimed to detect both common and extremely low-level compounds that could inhibit α-glucosidase, while also reducing solvent use and speeding up analysis. First, the crude diterpene extract was separated into 19 fractions using silica gel chromatography. Each fraction was then analyzed with ^1H NMR and tested for α-glucosidase inhibition. By applying cluster heatmap analysis to the ^1H NMR data, the researchers identified Fr.9-Fr.13 as the most biologically active fractions based on distinctive proton signal patterns. Further analysis of a representative sample, Fr.9, using ^13C-DEPT NMR revealed the presence of an aldehyde group, confirming earlier findings. After purification with semi-preparative HPLC, the scientists isolated three previously unknown diterpene esters, named caffaldehydes A, B, and C. Their chemical structures were verified through 1D and 2D NMR along with high-resolution mass spectrometry (HRESIMS). Stronger Effects Than a Common Diabetes Drug Although the three caffaldehydes differed in their fatty acid components (palmitic, stearic, and arachidic acids), all showed notable α-glucosidase inhibition. Their IC₅₀ values were 45.07, 24.40, and 17.50 μM respectively, indicating stronger activity than the comparison drug acarbose. To uncover additional trace compounds that were difficult to detect using NMR or HPLC alone, the team applied LC-MS/MS to combined fraction groups. They then built a molecular network using GNPS and Cytoscape. This analysis revealed three more previously unknown diterpene esters (compounds 4-6) that were closely related to caffaldehydes A-C. While they shared similar fragment patterns, these molecules contained different fatty acids (magaric, octadecenoic, and nonadecanoic acids). Searches of existing compound databases confirmed that these substances had not been reported before. Together, the results show that this integrated dereplication strategy is highly effective for identifying structurally diverse and biologically meaningful compounds in complex foods such as roasted coffee. What This Means for Functional Foods and Future Research The findings suggest new opportunities to develop coffee-based functional foods or nutraceuticals that support glucose control and may help manage diabetes. Beyond coffee, the same low-solvent, high-precision screening approach could be applied to other complex food sources to rapidly uncover health-related compounds. Future studies will focus on testing the biological effects of the newly discovered trace diterpenes and evaluating their safety and effectiveness in vivo.
