Delving into the Latest Updates on Scopolamine Butylbromide with Synapse

For outpatient hysteroscopy (OH), it is recommended to take a standard dose of NSAIDs or more hyoscine-n butyl bromide (HBB) an hour prior to the procedure to minimize pain during the first postoperative hour. As there is currently no clear consensus in the literature regarding the best approach to pain management associated with office hysteroscopy procedures. This Phase 4 study is being conducted to evaluate the effectiveness of oral ibuprofen alone and in combination with HBB to determine the most appropriate strategy for improving pain perception in outpatients.

Dihuang Yinzi improves scopolamine-induced learning and memory impairment by regulating plasma exosome-derived BDNF

ETHNOPHARMACOLOGICAL RELEVANCEDihuang Drink (DHD), formulated by Liu Hejian during the Yuan Dynasty, is listed as one of the first ancient classical prescriptions by the National Medical Products Administration of China. It is commonly used for the prevention and treatment of Alzheimer's disease (AD). This study further investigates the therapeutic effects and potential mechanisms of DHD in AD.AIM OF THE STUDYThis study aimed to evaluate the cognitive improvement effects of DHD on scopolamine (SCOP)-induced memory impairment in mice and to explore its anti-AD mechanisms mediated by exosomes.MATERIALS AND METHODSA cognitive impairment model was established in C57BL/6J mice via intraperitoneal injection of SCOP (1 mg/kg) for 21 consecutive days, followed by DHD intervention to assess its effects on learning, memory, hippocampal synaptic density, and the cholinergic system. SD rats were gavaged with DHD (22.00 g/kg) for 7 days, and plasma exosomes were extracted. These exosomes were injected into SCOP-treated mice (2 mg/kg, every other day for 14 days, 7 injections) to verify the role of exosomes in improving cognitive function. Behavioral performance and brain ChAT and BDNF levels were measured.RESULTSDHD improved learning and memory in SCOP model mice, attenuated neuronal loss and decreases in dendritic spines induced by scopolamine, and modulated the expression of BDNF, SYN-1, PSD95, and M1 mAChR. DHD-derived plasma exosomes further enhanced learning and memory function and significantly increased brain ChAT activity and BDNF levels.CONCLUSIONSDHD may alleviate cognitive impairment in SCOP model mice, potentially through exosome-mediated neuroprotection.

BackgroundColonic motility in constipation can be assessed non-invasively using MRI.ObjectiveTo compare MRI with high-resolution colonic manometry (HRCM) for predicting treatment response.DesignPart 1: 44 healthy volunteers (HVs), 43 patients with irritable bowel syndrome with constipation (IBS-C) and 37 with functional constipation (FC) completed stool diaries and questionnaires and underwent oral macrogol (500–1000 mL) challenge. Whole gut transit time (WGTT), segmental colonic volumes (CV), MRI-derived Motility Index and chyme movement by ‘tagging’ were assessed using MRI and time to defecation after macrogol recorded. Left colonic HRCM was recorded before and after a 700 kcal meal. Patients then proceeded to Part 2: a randomised cross-over study of 10-days bisacodyl 10 mg daily versus hyoscine 20 mg three times per day, assessing daily pain and constipation.ResultsPart 1: Total CVs median (range) were significantly greater in IBS-C (776 (595–1033)) and FC (802 (633–951)) vs HV (645 (467–780)), p<0.001. Patients also had longer WGTT and delayed evacuation after macrogol. IBS-C patients showed significantly reduced tagging index and less propagated pressure wave (PPW) activity during HRCM versus HV. Compared with FC, IBS-C patients were more anxious and reported more pain. Abnormally large colons predicted significantly delayed evacuation after macrogol challenge (p<0.02), impaired manometric meal response and reduced pain with bisacodyl (p<0.05).Part 2: Bisacodyl compared with hyoscine increased bowel movements but caused more pain in both groups (p<0.03).ConclusionAn abnormally large colon is an important feature in constipation which predicts impaired manometric response to feeding and treatment responses. HRCM shows that IBS-C patients have reduced PPW activity.Trial registration numberThe study was preregistered on ClinicalTrials.gov, Reference:NCT03226145.

High insulin levels during early life may lead to the late development of obesity and diabetes. We investigated whether attenuation of insulin levels by using an antimuscarinic agent could prevent diet-induced obesity and diabetes. Rats’ early exposure to an antimuscarinic agent reduced insulin levels during the lactation period and promoted protection against obesity and metabolic dysfunctions. Independent of the programming mechanisms, insulin levels during early life may be a defining factor of health or diseases later in life.

Sanofi's Negotiations to Sell Opella Stake Exposed, €16 Billion Deal Triggers Threat of Strikes from French Unions!

Sanofi is currently in "exclusive talks" with U.S. private equity firm Clayton, Dubilier & Rice (CD&R) to sell a 50% controlling stake in its consumer healthcare business Opella. The French government has confirmed a commitment from CD&R, based in New York, to ensure that Opella's production facilities in France will remain intact and that there will be no disruption to the supply of drugs to the country, removing a potential obstacle to the deal. Sanofi said the proposed deal valued Opella at 16 billion euros and stressed that CD&R's offer for a 50 percent stake was "binding and fully funded." In addition, French public sector investment bank Bpifrance plans to buy a 2% stake in Opella and take a seat on its board. The deal, expected to close "at the earliest" in the second quarter of next year, will see Sanofi step up its investment in innovative drugs and vaccines and mark the French drugmaker's formal joining the ranks of biopharmaceutical giants Johnson & Johnson, Pfizer and GlaxoSmithKline. "With this transaction, Sanofi will be able to focus even more on providing innovative treatments for patients suffering from debilitating or fatal diseases and viruses, such as RSV, COPD or multiple sclerosis," Sanofi CEO Paul Hudson said in a press release. Analysts at ODDO BHF supported the decision, arguing that a divestiture or partial divestiture of the consumer business would allow Sanofi to reallocate capital to research and business development, and recommended that Sanofi use some of the capital to offset the financial impact of the divestiture through share buybacks. Hudson first floated the idea of spinning off the consumer health division last October. Since then, CD&R and PAI Partners, another private equity giant, have made separate bids for Opella. Earlier this month, Reuters reported that CD&R had made a formal offer to buy 50 percent of Opella's consumer healthcare business. Last week, Sanofi said it was surprised by PAI's move to raise its 200 million euro offer after the deadline. Sanofi's commitment that Opella would continue to operate in France followed a call by French unions for workers to strike on Thursday in protest at the deal. Opella is a leader in over-the-counter medicines, vitamins, minerals and supplements. Its product line includes the well-known antihistamine Allegra, non-opioid painkiller Doliprane, stomach cramping relief Buscopan, Selsun Blue shampoo, and pain treatments Icy Hot and Gold Bond, among others. Opella employs 11,000 people in 100 countries and generated sales of 5.2 billion euros ($5.6 billion) in 2023, accounting for 11% of Sanofi's total revenue. In addition, the company operates 13 production sites and four research and development innovation centers.

EU authorities fine Boehringer, others for operating drug-ingredient 'cartel'

Five pharmaceutical companies, including the global player Boehringer Ingelheim, have landed in hot water for running a "cartel" to dominate the market for a specific drug ingredient in Europe. The European Commission (EC) is fining Alkaloids of Australia, Alkaloids Corporation, Boehringer, Linnea and Transo-Pharm a total of €13.4 million ($14.2 million) to settle claims that the companies agreed to fix prices and allocate business on the pharmaceutical component N-Butylbromide Scopolamine/ Hyoscine (SNBB), which is used to make the decades-old abdominal antispasmodic Buscopan and its generics. Boehringer received the largest fine by far, settling with the EC for €10.4 million. A spokesperson for Boehringer said the company "accepts the settlement decision and remains committed to fair competition, integrity, and compliance." "We have a zero-tolerance policy for any illegal or illicit business practices and are dedicated to always doing the right thing," the spokesperson added. Notably, C2 Pharma—which prides itself on tackling human rights, labor, the environment and corruption—has dodged all fines for blowing the whistle on the cartel to the EC. All six companies “admitted their involvement in the cartel and agreed to settle the case,” the European Commission said in a press release. The companies were accused of conspiring to fix the minimum sales price of SNBB, allocating quotas and exchanging “commercially sensitive” information, the EC said. The behavior ran from late 2005 to Sept. 17, 2019, according to authorities. Notably, Boehringer stopped its participation in the cartel at the end of 2014, with whistleblower C2 Pharma calling it quits in February of 2016. The EC said this is the first time it’s sanctioned a pharmaceutical cartel over an active pharmaceutical ingredient (API). The agency worked in tandem with Swiss and Australian competition authorities to level the charges. A seventh company, Alchem, is also being investigated but decided not to settle. Proceedings against that company will continue in tandem with Europe’s cartel procedure, the EC said.

A first for the European Commission: Five pharma companies fined $14M for API price fixing

The European Commission on Thursday announced a first for its regulation of the pharma industry: It’s fining five pharma companies — Alkaloids of Australia, Alkaloids Corporation, Boehringer Ingelheim, Linnea and Transo-Pharm — €13.4 million ($14.2 million) for colluding to fix the price of a pharmaceutical ingredient used in a drug to treat stomach cramps. The Commission said its investigation revealed that six companies — the sixth company C2 Pharma revealed the price fixing to the Commission and was not fined — coordinated and agreed to fix the minimum sales price of the ingredient, known as N-butylbromide scopolamine/hyoscine, which is commonly used in the antispasmodic drug Buscopan as well as in its generic versions. The ingredients were shipped to customers such as distributors and generic drugmakers, and the companies collaborated to allocate quotas.
