Delving into the Latest Updates on Scopolamine Butylbromide with Synapse

For outpatient hysteroscopy (OH), it is recommended to take a standard dose of NSAIDs or more hyoscine-n butyl bromide (HBB) an hour prior to the procedure to minimize pain during the first postoperative hour. As there is currently no clear consensus in the literature regarding the best approach to pain management associated with office hysteroscopy procedures. This Phase 4 study is being conducted to evaluate the effectiveness of oral ibuprofen alone and in combination with HBB to determine the most appropriate strategy for improving pain perception in outpatients.

Scopoletin alleviates high glucose-induced toxicity in human renal proximal tubular cells via inhibition of oxidative damage, epithelial-mesenchymal transition, and fibrogenesis

BACKGROUND:

Recent years of evidence suggest the crucial role of renal tubular cells in developing diabetic kidney disease. Scopoletin (SCOP) is a plant-based coumarin with numerous biological activities. This study aimed to determine the effect of SCOP on renal tubular cells in developing diabetic kidney disease and to elucidate mechanisms.

METHODS AND RESULTS:

In this study, SCOP was evaluated in vitro using renal proximal tubular (HK-2) cells under hyperglycemic conditions to understand its mechanism of action. In HK-2 cells, SCOP alleviated the high glucose-generated reactive oxygen species (ROS), restored the levels of reduced glutathione, and decreased lipid peroxidation. High glucose-induced alteration in the mitochondrial membrane potential was markedly restored in the SCOP-treated cells. Moreover, SCOP significantly reduced the high glucose-induced apoptotic cell population in the Annexin V-FITC flow cytometry study. Furthermore, high glucose markedly elevated the mRNA expression of fibrotic and extracellular matrix (ECM) components, namely, transforming growth factor (TGF)-β, alfa-smooth muscle actin (α-SMA), collagen I, and collagen III, in HK-2 cells compared to the untreated cells. SCOP treatment reduced these mRNA expressions compared to the high glucose-treated cells. Collagen I and TGF-β protein levels were also significantly reduced in the SCOP-treated cells. Further findings in HK-2 cells revealed that SCOP interfered with the epithelial-mesenchymal transition (EMT) in the high glucose-treated HK-2 cells by normalizing E-cadherin and downregulating the vimentin and α-SMA proteins.

CONCLUSIONS:

In conclusion, SCOP modulates the high glucose-generated renal tubular cell oxidative damage and accumulation of ECM components and may be a promising molecule against diabetic nephropathy.

A refined formula derived from Jiawei-Xiaoyao pill exerts rapid antidepressant-like effects in LPS-induced depression by reducing neuroinflammation and restoring neuroplasticity signaling

ETHNOPHARMACOLOGICAL RELEVANCE:

Jiawei-Xiaoyao Pill (JWX), a classic formula in traditional Chinese medicine, is derived from Xiaoyao Pill by adding significant amounts of Gardeniae Fructus (GF) and Moutan Cortex (MC). It is frequently used for the treatment of depression. JWX has been demonstrated to uniquely elicit rapid antidepressant-like effects within the prescribed dosage range. To date, GF has been shown to have rapid antidepressant-like effects, but a much higher dose is required than its proportion in JWX. It is assumed that the synergism of GF with a minimum number of other herbs in JWX serves as a refined formula that exerts these rapid antidepressant-like effects. Identification of a refined formula is important for prioritizing the herbs and ingredients to optimize the quality control of JWX. However, such a refined formula for JWX has not been identified yet.

AIM OF THE STUDY:

Here we aimed to identify a refined formula derived from JWX for optimized rapid antidepressant-like effects. Since the neuroinflammation mechanism involving in depression treatment has not been previously investigated for JWX, we tested the mechanism for both JWX and the refined formula.

MATERIALS AND METHODS:

Individual herbs (MC; ASR, Angelica Sinensis Radix; Bupleuri Radix; Paeonia Radix Alba) that show antidepressant-like responses were mixed with GF at the proportional dosage in JWX to identify the refined formula. Rapid antidepressant-like effects were assessed by using NSF (Novelty Suppressed Feeding Test) and other behavioral tests following a single administration. The identified formula was further tested in a lipopolysaccharide (LPS)-induced depressive model, and the molecular signaling mechanisms were investigated using Western blot analysis, immunofluorescence, and pharmacological inhibition of mTOR signaling. Scopolamine (Scop) was used as a positive control for induction of rapid antidepressant effects.

RESULTS:

A combination of GF, MC and ASR (GMA) at their dosages proportional to JWX induced behavioral signs of rapid antidepressant-like responses in both normal and LPS-treated mice, with the antidepressant-like effects sustained for 5 d. Similar to JWX or Scop, GMA rapidly reduced the neuroinflammation signaling of Iba-1-NF-кB, enhanced neuroplasticity signaling of CaMKII-mTOR-BDNF, and attenuated the upregulated expressions of the NMDAR sub-units GluN1 and GluN2B in the hippocampus of LPS-treated mice. GMA, JWX and Scop rapidly restored the number of BDNF-positive cells reduced by LPS treatment in the CA3 region of the hippocampus. Furthermore, rapamycin, a selective inhibitor of mTOR, blunted the rapid antidepressant-like effects and hippocampal BDNF signaling upregulation by GMA.

CONCLUSION:

GMA may serve as a refined formula from JWX, capable of inducing rapid antidepressant-like effects. In the LPS-induced depression model, the effects of GMA were mediated via rapidly alleviating neuroinflammation and enhancing neuroplasticity.

Scoparone alleviates nonalcoholic fatty liver disease by modulating the PPARα signaling pathway

Scoparone (Scop), a natural compound derived from Artemisia capillaris Thunb, has demonstrated efficacy in improving nonalcoholic fatty liver disease (NAFLD). This study aims to explore the underlying mechanism. NAFLD was induced by a high-fat diet in C57BL/6J mice, followed by an 8-week treatment with Scop. The effect of Scop on mice NAFLD was assessed. mRNA sequencing of liver tissues was performed to identify potential targets, which were validated through in vitro experiments using palmitic acid-induced AML12 hepatocytes. The results demonstrated that Scop promoted lipid metabolism, insulin sensitivity, and liver function, and alleviated inflammation in NAFLD mice. mRNA sequencing identified the peroxisome proliferator-activated receptor α (PPARα) signaling pathway as a target of Scop, which was further confirmed by in vivo and in vitro experiments. Molecular docking studies showed that Scop could bind stably to human PPARα. In summary, Scop was proven to alleviate lipid metabolism dysfunction and inflammation by targeting the PPARα signaling pathway, which provides a basis for its potential application in NAFLD treatment.

Sanofi's Negotiations to Sell Opella Stake Exposed, €16 Billion Deal Triggers Threat of Strikes from French Unions!

Sanofi is currently in "exclusive talks" with U.S. private equity firm Clayton, Dubilier & Rice (CD&R) to sell a 50% controlling stake in its consumer healthcare business Opella. The French government has confirmed a commitment from CD&R, based in New York, to ensure that Opella's production facilities in France will remain intact and that there will be no disruption to the supply of drugs to the country, removing a potential obstacle to the deal. Sanofi said the proposed deal valued Opella at 16 billion euros and stressed that CD&R's offer for a 50 percent stake was "binding and fully funded." In addition, French public sector investment bank Bpifrance plans to buy a 2% stake in Opella and take a seat on its board. The deal, expected to close "at the earliest" in the second quarter of next year, will see Sanofi step up its investment in innovative drugs and vaccines and mark the French drugmaker's formal joining the ranks of biopharmaceutical giants Johnson & Johnson, Pfizer and GlaxoSmithKline. "With this transaction, Sanofi will be able to focus even more on providing innovative treatments for patients suffering from debilitating or fatal diseases and viruses, such as RSV, COPD or multiple sclerosis," Sanofi CEO Paul Hudson said in a press release. Analysts at ODDO BHF supported the decision, arguing that a divestiture or partial divestiture of the consumer business would allow Sanofi to reallocate capital to research and business development, and recommended that Sanofi use some of the capital to offset the financial impact of the divestiture through share buybacks. Hudson first floated the idea of spinning off the consumer health division last October. Since then, CD&R and PAI Partners, another private equity giant, have made separate bids for Opella. Earlier this month, Reuters reported that CD&R had made a formal offer to buy 50 percent of Opella's consumer healthcare business. Last week, Sanofi said it was surprised by PAI's move to raise its 200 million euro offer after the deadline. Sanofi's commitment that Opella would continue to operate in France followed a call by French unions for workers to strike on Thursday in protest at the deal. Opella is a leader in over-the-counter medicines, vitamins, minerals and supplements. Its product line includes the well-known antihistamine Allegra, non-opioid painkiller Doliprane, stomach cramping relief Buscopan, Selsun Blue shampoo, and pain treatments Icy Hot and Gold Bond, among others. Opella employs 11,000 people in 100 countries and generated sales of 5.2 billion euros ($5.6 billion) in 2023, accounting for 11% of Sanofi's total revenue. In addition, the company operates 13 production sites and four research and development innovation centers.

EU authorities fine Boehringer, others for operating drug-ingredient 'cartel'

Five pharmaceutical companies, including the global player Boehringer Ingelheim, have landed in hot water for running a "cartel" to dominate the market for a specific drug ingredient in Europe. The European Commission (EC) is fining Alkaloids of Australia, Alkaloids Corporation, Boehringer, Linnea and Transo-Pharm a total of €13.4 million ($14.2 million) to settle claims that the companies agreed to fix prices and allocate business on the pharmaceutical component N-Butylbromide Scopolamine/ Hyoscine (SNBB), which is used to make the decades-old abdominal antispasmodic Buscopan and its generics. Boehringer received the largest fine by far, settling with the EC for €10.4 million. A spokesperson for Boehringer said the company "accepts the settlement decision and remains committed to fair competition, integrity, and compliance." "We have a zero-tolerance policy for any illegal or illicit business practices and are dedicated to always doing the right thing," the spokesperson added. Notably, C2 Pharma—which prides itself on tackling human rights, labor, the environment and corruption—has dodged all fines for blowing the whistle on the cartel to the EC. The companies were accused of conspiring to fix the minimum sales price of SNBB, allocating quotas and exchanging “commercially sensitive” information, the EC said. Notably, Boehringer stopped its participation in the cartel at the end of 2014, with whistleblower C2 Pharma calling it quits in February of 2016. The EC said this is the first time it’s sanctioned a pharmaceutical cartel over an active pharmaceutical ingredient (API). The agency worked in tandem with Swiss and Australian competition authorities to level the charges. A seventh company, Alchem, is also being investigated but decided not to settle. Proceedings against that company will continue in tandem with Europe’s cartel procedure, the EC said.

A first for the European Commission: Five pharma companies fined $14M for API price fixing

The European Commission on Thursday announced a first for its regulation of the pharma industry: It’s fining five pharma companies — Alkaloids of Australia, Alkaloids Corporation, Boehringer Ingelheim, Linnea and Transo-Pharm — €13.4 million ($14.2 million) for colluding to fix the price of a pharmaceutical ingredient used in a drug to treat stomach cramps. The Commission said its investigation revealed that six companies — the sixth company C2 Pharma revealed the price fixing to the Commission and was not fined — coordinated and agreed to fix the minimum sales price of the ingredient, known as N-butylbromide scopolamine/hyoscine, which is commonly used in the antispasmodic drug Buscopan as well as in its generic versions. The ingredients were shipped to customers such as distributors and generic drugmakers, and the companies collaborated to allocate quotas.
