SummaryValacyclovir Hydrochloride, sold under the brand name Valtrex, is an antiviral drug that was first approved in the USA in 1995. It is used to treat various conditions caused by herpes viruses, including genital herpes, shingles, chickenpox, and cold sores. Valacyclovir Hydrochloride works by inhibiting the activity of DNA polymerase, a viral enzyme essential for the replication of herpes viruses. This medication is the hydrochloride salt of the L-valyl ester of the antiviral drug acyclovir, and its chemical name is L-valine, 2-[(2-amino-1,6-dihydro-6-oxo9H-purin-9-yl)methoxy]ethyl ester, monohydrochloride. Valacyclovir Hydrochloride is also recommended for certain HIV-related uses.

Congenital Cytomegalovirus Infection: Update on Screening, Diagnosis and Treatment

Cytomegalovirus (CMV) is the most common cause of viral infection in newborn babies, and affects 1 in 200 of all live born infants in high‐income countries; and 1 in 71 in low‐ and middle‐income countries. It is a major cause of hearing loss and brain damage.Women may get CMV infection for the first time during pregnancy (primary infection) or may experience ‘non‐primary’ infection, either by reactivation of previous CMV infection or by a new infection with a different strain of the virus. The most common source of infection to pregnant women is the saliva and urine of young children. Therefore, all pregnant women, especially those in regular contact with young children, should be informed about hygiene‐based measures to reduce the risks, e.g. handwashing.The UK National Screening Committee recommends against universal antenatal or newborn screening for CMV. Testing for CMV is usually offered only to women who develop symptoms of influenza, glandular fever or hepatitis (liver inflammation) during pregnancy, or for those whom a routine ultrasound scan detects fetal anomalies that suggests possible CMV infection.The risk of harm to the fetus is greatest following primary CMV infection of the woman in early pregnancy, and appears to be very low following infection after 12 weeks of pregnancy. Babies with CMV infection at birth may have jaundice, a rash, enlarged liver or spleen, a small brain, or be small for their gestational age. Around 1 in 8 babies born with CMV infection will have clinically detectable signs at birth. The rest will not have any features detectable by clinical examination alone. Therefore, all infants with CMV infection at birth should be followed up at a minimum of up to 2 years of age or later, depending upon the disease status, to check hearing and brain development.Following primary CMV infection in the first 12 weeks of pregnancy, if the woman starts taking the antiviral medicine valaciclovir (valacyclovir) it reduces the risk of the baby becoming infected.Where CMV infection of the fetus in the womb has been confirmed (by amniocentesis, for example), regular ultrasound scans should be offered every 2–3 weeks until birth. Detailed assessment of the fetal brain is an essential part of these scans. Where maternal CMV infection occurs, but fetal infection is not confirmed, repeated ultrasound scans of the fetus should be offered every 2–3 weeks until birth.In infected fetuses, as well as ultrasound scans, an MRI scan of the brain should be offered at 28–32 weeks of gestation (and sometimes repeated 3–4 weeks later) to assess for any signs of harm to the fetal brain.All babies born to women with confirmed or suspected CMV infection should be tested for CMV with a urine or saliva sample within the first 21 days of life.In newborns with symptomatic CMV infection at birth, treatment with antiviral medicine (valganciclovir or ganciclovir) can reduce hearing loss in 5 out of 6 babies, and improve long‐term brain development outcomes in some. There is no licensed vaccine for CMV.

Comprehensive insights into COVID-19 vaccine-associated multiple evanescent white dot syndrome (MEWDS): A systematic analysis of reported cases

Considering the widespread use of COVID-19 vaccines as a preventive measure against the spread of the virus, it's necessary to direct attention to the adverse effects associated with vaccines in a limited group of populations. Multiple evanescent white dot syndrome (MEWDS) following COVID-19 vaccination is a rare adverse reaction associated with COVID-19 vaccines. In this systematic review, we collected 19 articles with 27 patients up to November 1, 2023, summarizing the basic information, clinical manifestations, examinations, treatments, and recoveries of the 27 patients. The 27 enrolled patients (6 males, 21 females) had a median age of 34.1 years (15-71 years old) and were mainly from 5 regions: Asia (8), the Mediterranean region (8), North America (7), Oceania (3) and Brazil (1). Symptoms occurred post-first dose in 9 patients, post-second dose in 14 (1 with symptoms after both), post-third dose in 1, and both post-second and booster doses in 1, while details on 2 cases were not disclosed. Treatments included tapered oral steroids (6), topical steroids (3), tapered prednisone with antiviral drugs and vitamins (1), and valacyclovir and acetazolamide (1), while 16 received no treatment. All patients experienced symptom improvement, and nearly all patients ultimately recovered. Moreover, we summarized possible hypotheses concerning the mechanism of COVID-19 vaccine-associated MEWDS. The findings provide insights into the clinical aspects of COVID-19 vaccine-associated MEWDS. More attention should be given to patients with vaccine-associated MEWDS, and necessary treatment should be provided to patients experiencing a substantial decline in visual acuity to improve their quality of life.

Prenatal and postnatal antiviral therapies for the prevention and treatment of congenital cytomegalovirus infections

Purpose of review:

Congenital cytomegalovirus infection (cCMV) is the leading infectious cause of sensorineural hearing loss and lifelong neurodevelopmental disabilities. Studies suggest antiviral therapy can prevent fetal infection after maternal primary infection, as well as halt the progression of hearing loss and neurodevelopmental disabilities in newborns with symptomatic cCMV. With growing worldwide momentum on early detection and diagnosis of cCMV, this review describes the exciting recent advances in antiviral therapies in CMV infected pregnant mothers and babies, as well as emerging evidence on anti-CMV vaccines.

Recent findings:

New opportunities for prenatal and neonatal interventions have driven a rising interest in screening and identification of asymptomatic CMV infection. Routine screening of pregnant women to identify primary infection in first trimester is now advocated in Western Europe but has yet to be examined from a public health perspective in other regions. Evidence is emerging for maternal valaciclovir therapy to prevent fetal infection after a maternal primary CMV infection in the first trimester of pregnancy. For those infants who are born with symptomatic cCMV, a 6-month course of valganciclovir, started within the first 4 weeks of life, and possibly up to 13 weeks of life, is the current recommended therapy. However, there is unclear evidence for the benefit of treatment for asymptomatic cCMV and cCMV with isolated hearing loss. Research to identify more effective antivirals and an effective CMV vaccine continues.

Summary:

More research is needed to determine the region-specific applicability of the new European recommendations for routine CMV screening in pregnancy. Areas of uncertainty in postnatal management include timing of initiation, duration of treatment and identifying pediatric subgroups that benefit from modification of the standard treatment recommendations.

Candel Therapeutics Announces CAN-2409 Achieved Primary Endpoint in Phase 3 Prostate Cancer Trial, Showing Significantly Improved Disease-Free Survival

NEEDHAM, MA, USA I December 11, 2024 I Candel Therapeutics, Inc. (Candel or the Company) (Nasdaq: CADL ), a clinical-stage biopharmaceutical company focused on developing multimodal biological immunotherapies to help patients fight cancer, today announced results from a multicenter phase 3 clinical trial evaluating CAN-2409 viral immunotherapy in localized prostate cancer patients. In the United States alone, over 100,000 men are diagnosed with localized prostate cancer every year, and over 50,000 men currently receive radiotherapy. Prostate cancer continues to be the second leading cause of cancer death among men in the United States and there has not been any new treatment or significant change in the standard of care of localized, non-metastatic prostate cancer for over 20 years. The localized prostate cancer addressable market for CAN-2409 is potentially worth over $10 billion in the U.S. alone. The phase 3 clinical trial of CAN-2409 in intermediate-to-high-risk, localized prostate cancer met its primary endpoint, by demonstrating statistically significant improvement in disease-free survival in patients who received CAN-2409 plus prodrug (valacyclovir) combined with standard of care compared to standard of care alone. The 2:1 randomized, double-blind, placebo-controlled, multicenter clinical trial enrolled 745 patients (intent to treat population, ITT) to evaluate the effectiveness and safety of CAN-2409 plus prodrug (valacyclovir) viral immunotherapy in combination with standard of care external beam radiation therapy to improve disease-free survival (DFS) in patients with intermediate-to-high risk, localized prostate cancer. Patients were randomized and stratified for the use of short-term (< 6 months) androgen deprivation therapy (ADT). CAN-2409 is an investigational, off-the-shelf, replication-defective adenovirus that delivers the herpes simplex virus thymidine kinase (HSV-tk) gene to tumor cells. CAN-2409, when administered with valacyclovir, is designed to induce immunogenic cell death of tumor cells with exposure of tumor antigens in the context of an activated tumor microenvironment. Together, this regimen is designed to induce an individualized and specific CD8+ T cell-mediated response against the tumor, based on in situ vaccination against a variety of tumor antigens. Preclinical and clinical evidence suggests that CAN-2409 can be synergistic with local radiotherapy, providing further support for the design of the current phase 3 clinical trial. “The improvement observed in disease-free survival in this phase 3 clinical trial is clinically meaningful. We have not seen significant advances in this indication in decades. CAN-2409 has demonstrated the potential to significantly improve long-term outcomes without adding substantial toxicity to standard of care radiation,” said Glen Gejerman, M.D., MBA, Co-Director of Urologic Oncology at Hackensack Meridian Health, and one of the principal investigators of the study. “If approved, this approach has the potential to transform the treatment paradigm in prostate cancer, offering patients with localized disease an effective treatment option that may reduce the risk of disease recurrence.” Phase 3 Trial Results in Intermediate-High Risk Disease The study met its primary endpoint, demonstrating a statistically significant improvement in disease-free survival compared to the control arm. Key topline results include: The median follow-up time for the recruited population was 50.3 months. The primary outcome measure included the evaluation of post-treatment biopsies, performed at two years from the end of radiation, for the presence of tumor recurrence. Local or systemic recurrence and death from any cause were also part of the DFS endpoint. The safety profile of CAN-2409 was generally consistent with previous studies, with no new safety signals identified. The most common CAN-2409-related adverse events were flu-like symptoms, fever and chills, which were generally mild to moderate in severity and self-limited. The company also reported today that the phase 2 clinical trial of monotherapy CAN-2409 in 190 patients with low-to-intermediate risk localized prostate cancer undergoing active surveillance showed numerical improvement in time to radical treatment and the percentage of patients achieving negative (prostate cancer-free) biopsies at 1-year post-treatment. However, these differences did not reach statistical significance. The safety profile of CAN-2409 was generally consistent with that reported in the phase 3 clinical trial. “We are thrilled to report the phase 3 results for CAN-2409 in intermediate-to-high risk, localized prostate cancer,” said Paul Peter Tak, M.D., Ph.D., FMedSci, President and Chief Executive Officer of Candel. “This study validates previous observations of CAN-2409 activity seen in difficult-to-treat solid tumors, often resistant to immunotherapy, and confirms our previous observation of synergies with radiation therapy in models of prostate cancer. Better prevention and treatment options needed for genital herpes, WHO says

A World Health Organization (WHO) director has called for “better prevention and treatment options” after the agency released new estimates of genital herpes prevalence. Dr Meg Doherty, director of global HIV, hepatitis and sexually transmitted infections programmes, has said that more needs to be done, not only to reduce the transmission of genital herpes but said this will also reduce the transmission of HIV. “While most people with a genital herpes infection experience few symptoms, with so many infections genital herpes still causes pain and distress for millions globally and strains already overburdened health systems,” said Dr Doherty. Updated figures show that around 846 million aged between 15 and 49 years are living with the infection. This total translates to more than one in five in this age group globally living with the disease, as per a WHO 11 December press release. Genital herpes is caused by two types of herpes simplex virus (HSV) – HSV-1 and HSV-2. WHO stated that HSV-2 is more serious from a public health perspective since it is more likely to cause recurrent outbreaks, along with a higher chance of contracting human immunodeficiency virus (HIV). Currently, there is no cure for genital herpes – although there are approved antivirals that relieve symptoms. In the US, this includes Novartis ’ Famvir (famciclovir), GSK ’s Valtrex (valacyclovir), and acyclovir. Although the antivirals do not remove the virus from the body, they slow the growth of herpes, making it easier for the immune system to control the infection and reduce the frequency of outbreaks. Valtrex and Famvir were approved by the US Food and Drug Administration (FDA) for genital herpes in 2003 and 2006 respectively. Since then, there has been little change in available treatments. “Not enough has been done to address this common infection,” said Dr Sami Gottlieb, a medical officer within WHO’s Department of Sexual and Reproductive Health and Research, who also served as author of the report detailing the updated infection figures. “Expanded research and investment in developing new herpes vaccines and therapies, and their equitable use, could play a critical role in improving quality of life for people around the world,” Dr Gottlieb added. An HSV vaccine could come courtesy of Moderna or BioNTech , who have candidates in clinical trials. Moderna’s therapeutic candidate, an mRNA vaccine called mRNA-1608 is currently in Phase II development while BioNTech’s prophylactic candidate, also mRNA-based BNT-163, is in Phase I development. Developing a vaccine that is effective against HSV has proved difficult, however. A setback occurred in September this year when GSK announced it was discontinuing its vaccine candidate following a Phase I/II failure. Although deemed safe, the vaccine lacked efficacy in reducing lesions in patients with recurrent genital herpes.

Everything You Need to Know About Shingles & the Shingles Vaccine

SUNDAY, Dec. 8, 2024 -- Shingles can strike anyone who had chickenpox when they were young, and the intense pain that can accompany this body rash has sidelined many a senior. Here, one expert explains how and why shingles can surface, and what you can do to treat it, or better yet, avoid it. Shingles can happen at any age, but it most typically affects people over 50 who have stress and compromised immunity. “Shingles is caused by the varicella-zoster virus. It’s the same virus that causes chickenpox,” said Dr. Eugene Fellin , a family medicine physician at Penn State Health Medical Group – Fleetwood. “For most of us who grew up before the 1990s, when children began being immunized against chickenpox, we’ve been exposed to the virus and are at risk for shingles.” How can shingles surface? After lying dormant in the nervous system for years, the virus can reemerge as shingles, which causes painful rashes that typically surface on the face or around the side of the torso, Fellin explained. “It’s like a poison ivy rash that won’t go away,” he added in a Penn State news release. “It can occur in patches, but along that same nerve root. A lot of times, people feel some tingling or a burning sensation prior to the rash actually breaking out,” Fellin noted. “When we’re looking for the rash, it will be in a string on the torso because the nerves wrap around the torso. You get a line around you, from the back to the front.” “The other issue we worry about is if it breaks out on the face and involves the eye because this can lead to blindness,” Fellin said. “Shingles around the eye is considered dangerous, and an instant referral to an ophthalmologist is always recommended.” What can you take to treat shingles? Antivirals such as Valacyclovir can be prescribed, but they’re time-sensitive and need to be taken within 36 hours of the start of the rash because they work by slowing the spread of the virus, Fellin said. While symptoms subside after three to five weeks, pain can sometimes return in the form of postherpetic neuralgia , he said. This long-term nerve pain occurs where the shingles rash appeared and can last for months or even years. Older adults are more likely to develop postherpetic neuralgia and have longer lasting and more severe pain, Fellin said. Luckily, there is something you can do to avoid shingles altogether: get vaccinated. The U.S. Centers for Disease Control and Prevention recommends the Shingrix vaccine, given in two doses, with the second dose given two to six months after the first. People who get shingles can still receive the vaccine, which can lower the chances of another outbreak, Fellin noted. Most family doctors and pharmacies stock the vaccine, which is covered by Medicare, he added. “Most insurance programs are covering it because it has been out long enough and shows a real benefit,” Fellin said in a Penn State news release. “There’s a lot of misinformation about vaccines circulating out there.
