The goal of this clinical trial is to learn if a common antibiotic called trimethoprim-sulfamethoxazole (TMP-SMX) can help prevent urinary tract infections (UTIs) in children and young adults who recently had a kidney transplant. Most people take TMP-SMX for about 6 months after getting a kidney transplant. In this study, researchers want to see what happens if people keep taking it for 6 more months.
The main questions this study is asking are:
* Does TMP-SMX lower the number of UTIs in the first year after transplant?
* What side effects or problems do participants have while taking TMP-SMX?
Researchers will compare TMP-SMX to a placebo (a look-alike pill that does not contain any medication) to see if TMP-SMX works to prevent UTIs.
Participants will:
* Take either TMP-SMX or a placebo pill by mouth every day for 6 months
* Have three visits to touch base with the study team about any issues
* Complete short monthly online surveys about any symptoms or side effects
* Share blood and urine test results from their regular transplant clinic visits

Trimethoprim-Sulfamethoxazole (TMP/SMX) Prophylaxis After Acute Kidney Injury to Prevent Post-discharge Infections: A Randomized, Double-Blind, Placebo-Controlled Trial
Brief Summary
Acute kidney injury (AKI) is commonly followed by infections after hospital discharge. This randomized, double-blind, placebo-controlled trial will test whether prophylactic TMP/SMX reduces post-discharge infections in adults recently hospitalized with AKI. Participants will be randomized 1:1 to TMP/SMX or matching placebo and followed for 6 months. The primary outcome is the proportion of participants who develop any infection within 90 days after discharge. Secondary outcomes include time to first infection, infection-related hospitalization, mortality, safety/adverse events, and healthcare utilization through 180 days.
Detailed Description
Adults discharged after an index hospitalization complicated by AKI are at elevated infection risk. This trial evaluates whether short-term TMP/SMX prophylaxis reduces 90-day infections. After consent and eligibility confirmation near discharge, participants are randomized (1:1) to receive TMP/SMX or matching placebo with double-blind masking (participant and outcome assessor). Dosing is standardized per protocol. We will ascertain infections via structured follow-up, medical record review, and adjudication by blinded assessors. Safety monitoring will capture adverse events (e.g., rash, cytopenias, hyperkalemia). Analyses follow intention-to-treat.

Eligibility Criteria
Inclusion Criteria
* Age ≥18 years.
* Index hospitalization complicated by AKI (KDIGO criteria) prior to discharge.
* Planned discharge to community/rehabilitation with capacity for follow-up.
* Ability to provide informed consent.
Exclusion Criteria
* Known allergy to sulfonamides or TMP/SMX.
* Pregnancy or breastfeeding.
* Severe hepatic disease (e.g., Child-Pugh C).
* Severe cytopenia (e.g., ANC <1.0×10⁹/L or platelets <50×10⁹/L).
* Baseline hyperkalemia (>5.5 mmol/L) not correctable prior to randomization.
* Concomitant medications with high-risk interactions not amenable to dose/monitoring (per protocol).
* Current systemic antimicrobial therapy planned for >14 days after discharge (prophylaxis not indicated).
* Inability to adhere to study procedures or follow-up.

Enterotoxin and biofilm producing Staphylococcus aureus and MRSA isolates from animal source foods sold in Assam

Animal-derived foods (ADF) are carriers of toxin-producing multidrug-resistant (MDR) S. aureus strains, posing a global food safety risk. We evaluated the toxin and biofilm production capabilities of S. aureus isolates from ADF marketed in Assam, India. Of the 138 staphylococcal isolates, 29 (21.01%) were identified as S. aureus, mostly isolated from raw milk. Resistance to chloramphenicol (13.79%), erythromycin (68.96%), penicillin (93.10%), enrofloxacin (20.68%), tetracycline (55.17%), trimethoprim-sulfamethoxazole (20.68%), linezolid (6.89%), cefoxitin (55.17%), and oxacillin (48.27%) was noted. Approximately 82.75% isolates were MDR, whereas 27.58% carried mecA gene, indicating methicillin-resistant S. aureus. PCR toxinotyping revealed pvl (20.69%), sea (37.93%), and sed (17%) genes. Additionally, 6.89% isolates harbored seb, sei, and tst genes; all lacked eta, etb, see, seg genes. A significant difference (P < 0.05) was observed in sea and sei genes across food sources. Biofilm-forming genes were detected as icaA (65.51%), icaB (34%), icaC (48.27%), icaD (27.58%), clfA (51.72%), clfB and fnbA (44.82%). At least one biofilm-associated gene was found in 72.41% of isolates, while two or more genes were present in 80.95%. Approximately 87.5% of mecA-positive isolates contained at least one biofilm-associated gene. A significant difference (P < 0.05) was noted in icaA, icaD, clfB, and fnbA across the different ADF sources.

Burkholderia cepacia complex infections beyond hospital outbreaks: A 22-Year analysis of clinical characteristics, antimicrobial resistance, and mortality predictors

BACKGROUND:

Burkholderia cepacia complex bacteria are intrinsically multidrug-resistant opportunistic pathogens that pose a growing threat in hospital settings, particularly among immunocompromised patients. Despite its clinical significance, data on sporadic Burkholderia cepacia complex infections outside cystic fibrosis populations and hospital outbreaks remain limited.

METHODS:

A 22-year retrospective observational cohort study (May 2001-April 2023) was carried out at a tertiary care academic center in the Eastern Province of Saudi Arabia. Adults hospitalized with confirmed Burkholderia cepacia infection and ≥ 72 h of antibiotic therapy were included. Demographics, comorbidities, infection type, microbiological resistance profiles, treatment outcomes, and mortality predictors were analyzed using logistic regression and Kaplan-Meier survival estimates.

RESULTS:

Of 189 identified cases, 151 patients met the inclusion criteria (mean age 53 ± 21 years; 57.6 % male). The majority of infections were hospital-acquired (84.8 %), with bloodstream infections (33.1 %) and ventilator-associated pneumonia (26.5 %) being the most prevalent. Resistance was high to piperacillin/tazobactam (73.6 %), ceftazidime (58.6 %), levofloxacin (57.4 %), carbapenems (42.0 %), and trimethoprim-sulfamethoxazole (30.3 %). Clinical resolution was achieved in 82.1 %, but bacteriological eradication occurred in only 56.9 %, with a recurrence rate of 17.2 %. Mortality reached 9.9 % at 14 days, 17.2 % at 30 days, and 35.8 % at one year. Independent predictors of 30-day mortality included advanced age, critical care admission, central venous catheter insertion, mechanical ventilation, and resistance to fluoroquinolones or carbapenems.

CONCLUSIONS:

Our findings indicate that Burkholderia cepacia complex infections represent a significant clinical challenge due to antibiotic resistance and elevated mortality rates. The data highlight the importance of improving diagnostic and treatment approaches, as well as implementing effective infection control policies, especially for vulnerable hospitalized populations.

Prevalence and Molecular Epidemiology of Extended-Spectrum β-Lactamase–Producing Escherichia coli and Effects of Antimicrobial Use in Multiple Pig Industry Sectors in Korea

Background:

The spread of antimicrobial-resistant bacteria has close relationships among humans, animals, and the environment, necessitating the implementation of the "One Health" approach. In previous study, the prevalence and molecular epidemiology of extended-spectrum β-lactamase-producing Escherichia coli (ESBL-EC) were investigated across multiple pig industry sectors in Korea. In this study, animal developmental stages and antimicrobial usage policies on farms were also considered.

Methods:

A total of 6,288 non-duplicated samples were collected from pig farms, slaughterhouses, and their personnel from three provinces in Korea between 2020 and 2022. Antimicrobial susceptibility and ESBL gene characterization were performed on the samples.

Results:

Overall, 1,084 ESBL-EC isolates were collected, with positivity detection rates of 24.7%, 23.3%, 12.7%, 3.9%, 17.2% in pigs, workers, the environment, meat, and total samples, respectively. Common ESBL types were bla_CTX-M-55 (N=628), bla_CTX-M-15 (N=204), and bla_CTX-M-14 (N=127). Pig-derived E. coli isolates had antimicrobial resistance (AMR) rates of 80.4%, 47.2%, 32.6%, 36.7%, 43.4%, 62.4%, and 72.1% to ampicillin, piperacillin, cefazoline, ciprofloxacin, cotrimoxazole, chloramphenicol, and tetracycline, respectively. AMR rates were lower in the finishing stage pigs than in the lactating stage pigs and tended to decrease during animal development for most antimicrobials. AMR rates were higher in pig farms with a customary antimicrobial use policy than in those with a low-level antimicrobial use policy.

Conclusions:

The findings highlight the significance of elucidating AMR networks among humans, animals, and the environment. Prudent antimicrobial usage and continuous AMR monitoring in the pig industry are crucial to safeguard food safety.

NITI Aayog Meet: Small Drugmakers Push Back on NPPA Price Caps, Seek Financial Assistance

New Delhi: Amid rising concerns over the growing incidence of substandard drugs produced by MSME units in India, small drugmakers have argued that unsustainable price caps and insufficient financial support are undermining their ability to maintain quality and address other compliance issues. At a high-level meeting with NITI Aayog—attended by Dr. V.K. Paul, the Drug Controller General of India (DCGI), and other key officials— CIPI Chairman RK Jain concerned that the pricing regulations imposed by the National Pharmaceutical Pricing Authority (NPPA) have severely affected the margins of small manufacturers and has pushed many quality drugs out of the domestic market. “Unsustainable pricing has pushed many quality drugs out of the domestic market,” he said. The representative also urged the government to provide financial assistance to help MSME pharmaceutical firms build infrastructure for stability studies and reduce the incidence of Not of Standard Quality (NSQ) drugs. Citing the example of co-trimoxazole, he highlighted that the ceiling price set by the National Pharmaceutical Pricing Authority (NPPA) has compelled small manufacturers to discontinue its production. Co-trimoxazole, an antibiotic indicated to treat bacterial infections was added to the national list of essential medicines (NLEM) in 2022, that resulted in the average reduction of around 16 per cent. Currently, the ceiling price for co-trimoxazole oral liquid (5 ml suspension) is capped at ₹0.35 per ml, while the tablet form is priced at ₹0.9 and ₹2.3 for higher dosage strength. Under Schedule I of the Drug Price Control Order (DPCO), 2013, the NPPA regulates both bulk drugs and formulations listed in the NLEM by fixing ceiling and retail prices to ensure affordability. Marketers are allowed to raise prices in line with inflation, based on the Price Differential Index prepared by the Office of the Economic Adviser (OEA), Ministry of Commerce and Industry. Meanwhile, over the dealy in compliance with the revised schedule M norms, representatives from other lobby groups such as Laghu Udyog Bharati, FOPE, IDMA and several others presented that, “gap analysis and detailed data requirements create hindrances in filing for entitlement for many companies, as many changes occur during development.” “MSME below ₹50 crores needed hand-holding and further extension in Revised Schedule M, as well as simplification of DOP-derived RPTUAS as per Revised Schedule M or WHO Good Manufacturing Practices (GMP),” the representatives noted. The PTUAS (Pharmaceutical Technology Upgradation Assistance Scheme) is a credit-linked initiative offering up to ₹1 crore in financial aid for MSME pharma units aiming to upgrade their facilities to WHO-GMP or Schedule M standards. Besides this, the industry representatives has also called for incentivising the compliance process by offering rewarding with ₹50 lakhs to ₹2 crores to companies that successfully obtain license through joint inspections of CDSCO and state regulatory bodies. Small drugmakers are those entities who have an annual turnover of or less than ₹250 crore and of the country’s total poll of 10,500 manufacturers around 8,500 fall under the MSME category. However, according to the Ministry of Health, only 2,000 of these units are WHO-GMP certified—highlighting the urgent need for targeted policy support, infrastructure upgradation, and regulatory collaboration to ensure quality manufacturing at scale. Paracetamol among 135 drugs fails CDSCO quality test

New Delhi: During its monthly review undertaken to monitor and ensure the quality of drugs manufactured in the country, the apex drug regulator CDSCO has identified 135 drug batches including Paracetamol, Vitamin D3 supplements, anti-diabetic pills, high blood pressure etc. as ‘Not of Standard Quality’ (NSQ). Paracetamol Tab IP 500 mg which failed to meet the quality standards was picked by the Drugs Testing Laboratory Thiruvananthapuram and is manufactured by Kerala Medical Services Corporation Ltd. As per the regulators' alert the identified sample, “fails the test ‘Dissolution’ as per IP ( Indian Pharmacopoeia ).” Another Paracetamol Tablet with a dosage strength of 650 mg was manufactured by VivekPharmachem and failed to meet the quality standards when tested by the Karnataka state laboratory. It is flagged as NSQ. Paracetamol is indicated for headache, period pain and cold or flu symptoms and is easily available as over-the-counter medication. In the December drug alert, the Central Drugs Standards Control Organisation (CDSCO) declared 135 drug samples as NSQ. Among these 51’ samples were picked by the regulator’s central drug laboratories while the state laboratories identified the remaining ‘84’ NSQ samples. In the central list, the drugs and formulations which have been identified as NSQ include Paracetamol Paediatric Oral Suspension manufactured by ION Healthcare; Doxycycline capsules by Hindustan Antibiotics; Glimepiride Tablets by Life Max Cancer Laboratories; Co - trimoxazole by Omega Pharma among 47 others. The State NSQ list comprises Metformin Hydrochloride tablets manufactured by Cipla Ltd; Divalproex Extended Release tablets of Biodeal Pharmaceuticals; and Betahistine Dihydrochloride Tablets by CMG BIOTECH among 81 others. The majority of the drug batches tested by the regulator were manufactured in 2023, 2024 and were set to expire in the next two years depending upon the date of their manufacturing. As per the Health Ministry, “previous month, this month’s NSQ reporting from states has further increased showing increased participation of States in reporting NSQs to the central database.” “Increased reporting of NSQs/Spurious identifications from states to central database will further help in improving availability of quality medicines in the country and beyond,” the ministry added. Nitrofurantoin, Cranberry Products May Cut UTI Episodes in Children

MONDAY, Nov. 4, 2024 -- For children with a history of recurrent urinary tract infection (RUTI), nitrofurantoin and cranberry products may reduce the incidence of symptomatic UTI episodes, according to a review published online Nov. 4 in Pediatrics . Nikolaos Gkiourtzis, M.D., from the Aristotle University of Thessaloniki in Greece, and colleagues conducted a systematic literature review of randomized controlled trials comparing prophylaxis options for the prevention of UTI and kidney scarring in children with a history of RUTI. A total of 3,335 participants from 23 studies were included in the meta-analysis. The researchers found that cranberry products (compared with the control group) and nitrofurantoin (compared with the control, trimethoprim-sulfamethoxazole, or trimethoprim groups) led to lower odds of symptomatic UTI episodes during prophylaxis. Compared with all available documented interventions, nitrofurantoin appeared to be the best option for reduction in the incidence of UTI. None of the prophylaxis options showed a reduction in kidney scarring. "Future studies with optimal methodology, studying nonantibiotic prophylaxis options, focusing on children with RUTI, and the risk for kidney scarring are needed to draw further conclusions," the authors write.
