Comparison of vaginal moisturisers with ultra-low-dose oestriol and lactobacillus vaginal tablets in women with POP who used a pessary: a randomised controlled trial

BACKGROUND:
This study aim to compare the proportion of pessary users reporting satisfaction with ultra-low dose oestriol and Lactobacillus vaginal tablets and vaginal moisturiser.
METHODS:
A randomised-controlled trial enrolled patients who were able to take care, insert, and remove a pessary themselves and did not use any local oestrogen or vaginal moisturiser for 3 months at a tertiary centre. Participants were randomised to receive either vaginal moisturisers or ultra-low-dose oestriol and Lactobacillus vaginal tablets for 2 months. A single-blinded evaluator assessed outcomes. The oestriol group received one vaginal tablet daily for 6 days followed by two tablets weekly for 8 weeks. The moisturiser group received one application every 3 days for 8 weeks. The primary outcome was the proportion of patients reporting high satisfaction, defined as 'very much improved' or 'much improved', using the Thai version of the Patient Global Impression of Improvement (PGI-I) scale. Secondary outcomes were subjective and objective outcomes of vaginal health. Sixty patients were randomised, of whom one did not complete treatment.
RESULTS:
The majority of enrolled participants were postmenopausal women with a mean age of 65.54 years, mean BMI of 25.13 kg/m2, and advanced-stage POP. The average duration of pessary use was 12-15 months. Patient satisfaction was not significantly different between the oestriol and moisturiser groups at 4 or 8 weeks (93.10% vs. 96.67%, P = 0.61 and 89.66% vs. 93.33%, P = 0.67, respectively). There were no serious adverse events.
CONCLUSIONS:
Vaginal moisturisers and ultra-low-dose oestriol and Lactobacillus acidophilus vaginal tablets provided high satisfaction in pessary users. Both preparations were comparable in both subjective and objective outcomes.

Regulation of TREG/TH17 balance by NK cells pretreated with ESTRIOL and bacterial cells in multiple sclerosis

We studied the influence of the pregnancy hormone estriol (E3) and bacterial strains on the ability of NK cells to modulate levels of T regulatory (Treg) and IL-17-producing (Th17) lymphocytes from patients with multiple sclerosis (MS) and healthy donors. NK cell phenotype and cytokine production in co-cultures with CD4+ lymphocytes was also investigated after incubation with the hormone and bacterial cells. Treatment of NK cells with these factors stimulated Treg formation and inhibited Th17 development. Within 24 h, NK cell cytotoxicity and production of IL-2 and IFN-γ were inhibited, while IL-10 secretion increased. This was accompanied by a rise in IL-6 and TNF-α. After three days, NK cells showed increased IL-10 and decreased IL-17 expression. Notably, cells from MS patients were more sensitive to this regulatory influence than those from healthy donors.

Identification of Drugs Associated With Oral Ulcers Based on Pharmacovigilance

BACKGROUND:
Oral ulcers are common, painful lesions that significantly affect quality of life. Certain medications, especially antimetabolites and mTOR inhibitors, have been implicated in their development. However, drug-induced oral ulcers have not been systematically evaluated using large-scale real-world data.
METHODS:
The study employed a dual-validation strategy combining pharmacovigilance signal mining and multivariate logistic regression analysis. By querying specific 'preferred terms' (PTs), adverse event reports in the FAERS database of the Food and Drugs Adminstration of USA, related to oral ulcers were identified. Signal detection was conducted using four disproportionality analysis methods. For the initially screened candidate drugs, a multivariate logistic regression model was constructed, adjusting for potential confounders such as age, gender, weight, reporter type, reporting country and year, to further validate their independent association with oral ulcer reports (PFDR < .05).
RESULTS:
Reports of oral ulcerations have shown a significant and sustained increase since 2010, peaking in 2020. The multi-method validation framework identified 289 statistically significant drug-oral ulceration association signals. Drugs with the highest signal strength included cyclizine, mycophenolate sodium and aredia. Other drugs with significant signals included lactulose, acetaminophen codeine phosphate, leflunomide, omalizumab, everolimus and atezolizumab. Integrating signal detection results, logistic regression analysis further confirmed that multiple drugs were significantly associated with the risk of oral ulcer reports. Drugs with extremely high odds ratios (ORs) included estriol and docetaxel sagent. Strongly associated drugs also included immunosuppressants, antihistamines, combination analgesics, bisphosphonates, sartans and antibiotics.
CONCLUSION:
This study systematically identified approximately 289 drugs significantly associated with oral ulcer risk, including immunosuppressants, cardiovascular drugs and bronchodilators. Based on pharmacovigilance signal detection and regression analysis, patients on long-term treatment with these drugs should receive enhanced oral monitoring. As this is an observational study, further prospective and experimental research is needed to confirm causality.
