Entacapone Combined With Bevacizumab in Patients With Glioblastoma at First Recurrence Based on the MYOF/p-STAT3 Signaling Axis: A Multicenter, Randomized, Double-Blind, Placebo-Controlled Study

This investigator-initiated, multicenter clinical trial will evaluate the safety and preliminary effectiveness of adding entacapone to bevacizumab in adults with isocitrate dehydrogenase (IDH)-wildtype glioblastoma at first recurrence after standard first-line treatment. Participants must not have previously received bevacizumab. A multidisciplinary neuro-oncology team must determine that repeat tumor resection is not appropriate, or the patient must decline repeat surgery after being informed of its potential benefits and risks.
The study has two stages. In the first stage, 6 participants who can be evaluated for dose-limiting toxicity will receive open-label entacapone 400 mg by mouth three times daily plus bevacizumab. Enrollment will occur in two cohorts of 3, with safety review by an independent data and safety monitoring board. Up to 3 participants who cannot complete the 28-day toxicity evaluation for reasons unrelated to toxicity may be replaced.
If the combination has acceptable safety and the required approvals are obtained, 90 new participants will enter the second stage and be randomly assigned in a 1:1 ratio to entacapone or matching placebo. Both groups will receive bevacizumab. The primary endpoint of the randomized stage is progression-free survival assessed by blinded independent central review. The study plans to obtain data from 96 evaluable participants, with no more than 99 participants receiving study treatment.

Reduction-Alkylation Proteome Profiling (RAPP): A novel strategy for proteome-wide target identification

The discovery of target proteins for drugs is crucial, and mass spectrometry (MS)-based proteomics is one of the most important tools for identifying drug targets. Different target identification methodologies are suited to different applications, each offering unique strengths but also facing specific challenges. Herein, we propose Reduction-Alkylation Proteome Profiling (RAPP), a novel method for proteome-wide ligand target identification based on differential reduction-alkylation reaction. The RAPP method was successfully validated by verifying the interaction between BSA and heparin, recapitulating the known targets of staurosporine, oxamate, and entacapone, and identifying ATP-binding proteins. Beyond the expected target LDHA, RAPP also identified FAHD1 as a previously unrecognized binding target of oxamate, suggesting its potential to detect weak binding events. Furthermore, RAPP revealed proteome-wide differences in protein stability under reducing conditions. Compared with the classic TPP method, our approach enables the identification and denaturation curve fitting of 295 additional proteins that could not produce discernible melting curves in TPP, features a more simplified workflow without complicated operations or specialized equipment, and can serve as an important complementary strategy to classical modification-free ligand target identification methods. In conclusion, RAPP serves as a valuable modification-free approach for target identification and holds practical application potential in chemical biology and drug discovery research.

Targeting the SETD2–FTO–KIT pathway as a pharmacological strategy for TKI-refractory GIST

Tyrosine kinase inhibitors (TKIs) have significantly improved outcomes in gastrointestinal stromal tumors (GIST), but acquired resistance remains a major clinical obstacle. The underlying mechanisms that sustain oncogenic KIT signaling in TKI-refractory GIST are not fully understood. Here, we identify a previously unrecognized epigenetic-epitranscriptomic regulatory axis involving SETD2, FTO, and KIT that promotes TKI resistance by stabilizing KIT mRNA. FTO, a key N ⁶-methyladenosine (m⁶A) RNA demethylase, was found to be markedly overexpressed in high-risk and recurrent GIST samples and correlated with poor prognosis. Mechanistically, FTO enhances KIT mRNA stability by erasing m⁶A modification in its 3' untranslated region (UTR), thereby preventing YTHDF2-mediated degradation and sustaining oncogenic KIT expression. Furthermore, the histone methyltransferase SETD2 activates FTO transcription via H3K36 trimethylation at the FTO promoter, establishing a self-reinforcing loop that maintains FTO overexpression. Functional experiments using FTO knockout and pharmacological inhibition (CS1 and entacapone) demonstrated that disrupting FTO activity suppresses tumor proliferation, reduces KIT expression, and restores sensitivity to imatinib both in vitro and in vivo. In KIT-Asp818Tyr/+ genetically engineered GIST mouse models, the combination of entacapone and imatinib led to significant tumor regression. Most importantly, preliminary clinical results from a Phase I trial (ClinicalTrials.gov: NCT04006769) showed that treatment with entacapone plus imatinib resulted in partial remission in two out of three evaluable TKI-refractory GIST patients, supporting the translational relevance of this strategy. Collectively, our findings establish the SETD2-FTO-KIT axis as a critical mechanism driving drug resistance in GIST and reveal m⁶A demethylation as a key regulator of KIT mRNA stability. Targeting FTO represents a promising and actionable therapeutic strategy to overcome TKI resistance and improve outcomes for patients with advanced GIST.

Safety of COMT-inhibitors in parkinson’s disease: a phase-IV comparative study on adverse events of Tolcapone, Entacapone and Opicapone

Catechol-O-methyltransferase (COMT) inhibitors-Tolcapone, Entacapone, and Opicapone-are adjunct therapies for advanced Parkinson's disease (PD), improving motor control. Their distinct safety profiles may influence clinical decisions and patient adherence. This study provides a comprehensive descriptive analysis of adverse events (AEs) reported for these drugs and a comparative safety assessment based on post-marketing AE reports and drug sales data from 2018 to 2023. A retrospective analysis was conducted using data from the FDA Adverse Event Reporting System (FAERS). AEs were categorized by severity and compared across COMT-inhibitors. Drug utilization trends were evaluated through retail and hospital sales data, expressed as standard units and market share. AE incidence was normalized for drug exposure using the median number of pills per day. Statistical comparisons were performed using Fisher's exact test to assess differences in AE rates. Entacapone was sold 100 times more than Opicapone and 1000 times more than Tolcapone. When normalized for drug exposure, total AEs were significantly lower for Entacapone than Opicapone (OR = 0.0435, p < 0.001) and Tolcapone (OR = 0.1397, p < 0.001). Tolcapone also exhibited a significantly lower AE risk than Opicapone (OR = 0.3119, p < 0.001). Similarly, Entacapone had significantly fewer serious AEs than Opicapone (OR = 0.0732, p < 0.001) and Tolcapone (OR = 0.0872, p < 0.001), indicating a more favorable safety profile. This is the first study directly comparing the safety profiles of COMT-inhibitors head-to-head. Entacapone appears to be the safest long-term COMT-inhibitor, with the lowest risk of both serious and non-serious AEs. A long-term, prospective clinical trial is warranted to overcome retrospective limitations and confirm these findings.

FDA mandates label updates to common Parkinson's meds based on seizure risk finding

In its safety review, the FDA found 14 causes of seizures linked to vitamin B6 deficiency in patients taking the common Parkinson’s disease drugs. A common drug combo for Parkinson’s disease will require a new FDA warning after the agency flagged 14 cases of seizures in patients using the medications. The agency is mandating (PDF) that drugmakers marketing products incorporating levodopa and carbidopa update their prescribing information to note that the drugs can cause vitamin B6 deficiency and vitamin B6 deficiency-associated seizures. Healthcare providers should evaluate patients’ baseline vitamin B6 levels prior to and during treatment and supplement patients with the vitamin as necessary, the warning says.The label update affects Amneal’s Crexont and Rytary; Avion Pharmaceuticals’ Dhivy; Merck’s Sinemet and controlled-release Sinemet CR; Novartis’ Stalevo; and AbbVie’s Duopa. Also affected is AbbVie’s newer Vyalev, a subcutaneous treatment that delivers prodrugs of carbidopa and levodopa via an infusion pump system. Vyalev’s ingredients of foscarbidopa and foslevodopa are drug derivatives of carbidopa and levodopa that become active after entering the body. Parkinson’s disease has been linked to dopamine deficiency, making levodopa, a metabolic precursor to dopamine, a favored treatment approach dating back to the 1960s. However, the drug must be taken with another medication to prevent it from breaking down in the bloodstream before crossing the blood-brain barrier, making the combo of carbidopa and levodopa the reigning standard-of-care in Parkinson’s disease treatment.But as levodopa converts to dopamine, crucial vitamin B6 levels can be impacted, the FDA pointed out in its warning. Carbidopa also binds to the active form of the vitamin, which can further drive down vitamin B6 levels. In its safety review, the FDA identified 14 causes of seizures linked to vitamin B6 deficiency in patients taking the common Parkinson’s disease drugs, including 13 instances that were submitted to the FDA’s adverse events reporting system and 1 reported in medical literature. There are “likely additional cases about which we were unaware,” the agency notes. Each of the reviewed cases involved levodopa doses greater than 1,000 mg daily and the seizures were split across oral formulations of the drugs and an enteral suspension. AbbVie makes the enteral suspension version with its Duopa, a gel medication that’s delivered directly into the small intestine through a tube. Some of the cases progressed to status epilepticus, a longer-lasting seizure that can cause brain damage or death, the FDA added. Several of the seizure reports showed “clinical evidence supportive of vitamin B6 deficiency,” and nine seizures were resolved when treated with supplemental vitamin B6. Two deaths occurred in patients with “documented low vitamin B6 levels and poorly controlled seizures,” according to the agency. “Based on the available data, FDA concluded there is reasonable evidence of a causal association between drug products containing carbidopa/levodopa and vitamin B6 deficiency-associated seizures,” the FDA said.Although no seizures were confirmed to result from products containing carbidopa/levodopa and entacapone (Novartis’ Stalevo) and injectable versions of carbidopa/levodopa (AbbVie’s Vyalev), the FDA is slapping the same warning on those meds, considering a likely risk based on “biological plausibility” and clinical trial data from the meds. Vyalev was the most recently approved carbidopa/levodopa med, scoring its FDA nod in 2024. The drug is a follow-up to AbbVie’s landmark 2015 Duopa, which sparked a trend of delivery system innovation in the Parkinson’s space as manufacturers looked to avoid the short half-life associated with oral versions of the meds. The FDA safety sweep of carbidopa and levodopa meds could lend credence to Supernus Pharmaceuticals’ novel Parkinson’s disease approach with Onapgo, a wearable pump that delivers a continuous subcutaneous infusion of Supernus’ dopamine agonist apomorphine (Apokyn). After winning approval in 2025 following three FDA rejections, the drug made so much of a splash in the disease space that the company had to pause delivery to new patients in November based on “stronger than expected demand,” Supernus said.Onapgo’s shipments has since resumed and the drug collected $17.3 million in 2025 sales, while AbbVie’s Vyalev took home $482 million and its older Duopa garnered sales of $381 million.

FDA Warns of Seizure Risk With Some Parkinson’s Drugs

Both AbbVie and Novartis market levodopa-based products. Products containing levodopa and carbidopa, a common drug combination for treating Parkinson’s disease, could trigger seizures, the FDA has found. The agency conducted a safety review of drugs carrying these compounds and found 14 cases of seizures linked to vitamin B6 deficiency, according to a safety alert on Friday. In all of these cases, levodopa doses exceeded 1,000 mg per day. Seizures were typically focal-onset, meaning they started in a limited region of the brain. They then spread to include both hemispheres, leading to convulsions and causing emergencies in some cases. Two patients died, both of whom had had low vitamin B6 levels and poorly controlled seizures, the FDA said. Nine patients were given vitamin B6 supplementation, which in all cases led to the resolution of their seizures. All 14 cases of carbidopa/levodopa-linked seizures were detected through post-marketing surveillance or found in the medical literature, “so there are likely additional cases about which we are unaware,” the FDA said. While the safety signals arose in those taking oral formulations and enteral suspensions of carbidopa/levodopa, the regulator did not dismiss the possibility that other products—such as injectables or those that use additional compounds to enhance the original drug combo—could carry the same risk. “Biological plausibility suggests there may be a similar risk across all drug products containing carbidopa/levodopa,” the FDA wrote. The agency has already notified all manufacturers of drugs carrying carbidopa/levodopa, asking them to update the labels of their respective products to carry warnings for vitamin B6 deficiency-associated seizures. In the meantime, healthcare professionals should assess their patients’ vitamin B6 levels at baseline and throughout treatment, and consider supplementation as needed, the FDA recommended. Levodopa is a synthetic drug that, once it reaches the brain, is converted into dopamine, a neurotransmitter that is deficient in patients with Parkinson’s disease. Levodopa’s therapeutic benefits for Parkinson’s were first established in the 1960s, and the first drug that combined it with the decarboxylase inhibitor carbidopa—which improved its efficacy and safety—entered the market in 1975 . The combo remains in use today. In October 2024, the FDA approved AbbVie’s Vyalev , a levodopa-based therapy for advanced Parkinson’s disease. The product contains foscarbidopa and foslevodopa, prodrugs that can be given continuously over 24 hours as a subcutaneous infusion. Novartis also owns a carbidopa/levodopa product in Stalevo , which additionally uses entacapone to sustain heightened levels of the drug in the bloodstream. The industry is now opening up new therapeutic avenues for Parkinson’s disease, however. Eli Lilly in January scooped up Ventyx Biosciences for $1.2 billion, gaining access to a pipeline of drugs that target the NLRP3 inflammatory cascade, an underlying pathway driving the condition.
