A new gas-diffusion electrode (GDE) has been developed and optimized to apply the heterogeneous electro-Fenton (EF) process to the degradation of the antihistamine diphenhydramine.The complete removal of this hazardous drug pollutant was achieved within only 120 min in actual urban wastewater at pH 5.9.Nitrogen-doped carbon electrocatalysts were first synthesized by direct pyrolysis of blends of com. Vulcan carbon and melamine as nitrogen dopant.A radar chart was proposed to correlate the tested physicochem. and electrochem. properties.A well-balanced surface area, mesopores volume and defective carbon content were critical to ensure the highest activity (E_onset = 0.38 V vs. RHE) and selectivity (n ∼ 2, y_H2O2 ∼ 95.3 %) for two-electron oxygen reduction reaction (ORR), which allowed the efficient in-situ production of H₂O₂ as ^·OH precursor.N-doping (10.5 wt%), along with the presence of pyrrolic N (3.5 %), are also key factors to explain the enhancement.GDEs fabricated with the optimal electrocatalyst (71.4 wt% melamine content) were capable of accumulating up to ∼35 mM H₂O₂ within 5 h, outperforming the com. GDEs.This work evidences the interplay of physicochem. and electrocatalytic parameters in H₂O₂-based advanced water treatment, demonstrating that it is possible to envisage a more efficient and greener tertiary wastewater treatment.

Treatment of headache reduces blood pressure among most patients with migraine and elevated blood pressure

BACKGROUNDIt is unclear whether patients with elevated BP and acute headache require treatment with anti-hypertensive medication, anti-headache medication, or both. We determined the impact of migraine medication on blood pressure and pain scores among emergency department patients with moderate or severe migraine and elevated blood pressure.METHODSThis study uses data collected from four emergency department-based migraine clinical trials. Patients were included if they had moderate or severe migraine. Patients received one of the following medications or medication combinations: Metoclopramide + dexamethasone, metoclopramide + methylprednisolone acetate, metoclopramide + diphenhydramine, hydromorphone, or prochlorperazine+ diphenhydramine. Blood pressure and 0-10 pain scores were assessed before medication administration and 1 h later.RESULTSWe collected data from 729 patients. 13.3 % (97/729) had moderately elevated BP or worse. Among these patients, we identified an association between change in mean arterial pressure and change in pain (B coefficient=0.04,p=0.01) Among 53 study participants with elevated blood pressure but without a diagnosed history of hypertension, mean arterial pressure change was associated with pain score change (B. Among 44 patients with elevated blood pressure and a history of diagnosed hypertension, there was no association with mean arterial pressure (B coefficient=0.03,p=0.25). Among the 97 patients with moderately elevated blood pressure or worse, 73.2 % (95 %CI, 64.2-82.2 %) experienced an improvement in diastolic blood pressure and 78.4 % (95 %CI, 70.0-86.7) improved systolic blood pressure.CONCLUSIONIn this analysis of data aggregated from four ED-based migraine studies, improvement in pain and blood pressure was associated among patients with acutely elevated blood pressure but without diagnosed hypertension. Most patients with elevated blood pressure who receive headache medication will experience improvement in their blood pressure over the subsequent hour.PLAIN LANGUAGE SUMMARYMany patients present to emergency departments with headache and high blood pressure. A majority of these patients experienced blood pressure improvements upon treatment of their headache, and those with acutely elevated blood pressure but not diagnosed hypertension demonstrated an association between pain and blood pressure improvements. Instead of administering unnecessary anti-hypertensive medication, this research suggests that in a majority of patients, elevated blood pressure can be managed by treating the headache.

Comprehensive analysis of pharmaceutical and illicit drugs contamination in thermal swimming pools: Occurrence, distribution, and potential impact

The contamination of aquatic environments by micropollutants, including pharmaceuticals and illicit drugs, is a growing global issue with significant implications for environmental and public health. This study addresses a critical knowledge gap by examining pharmaceutical and drug contamination in thermal swimming pools, which are often legally discharged into water bodies without treatment. A total of 49 water samples from 19 distinct thermal pools were analysed, targeting 101 compounds, including pharmaceuticals, illicit drugs, and their metabolites. High-frequency contaminants were identified including caffeine, theophylline, cardiovascular medications (bisoprolol, metoprolol, telmisartan), antidepressants (venlafaxine, citalopram), antihistamines (cetirizine, diphenhydramine), the analgesic tramadol, the antiepileptic carbamazepine, and the illicit drug methamphetamine. The study revealed that these substances were present in more than 30% of the samples, with some occurring at relatively high maximum concentrations, e.g. caffeine (69 μg/L), and methamphetamine (1.8 μg/L). Notably, certain categories of targeted micropollutants, such as cardiovascular medications and illicit drugs, were found to be more prevalent in specific pools. A higher occurrence of cardiovascular drugs has been observed in a pool predominantly designed for relaxation and thus probably preferred by older people. In this pool also notably high levels of illicit drugs were found. The presence of illegal drugs was also confirmed in the children's pool. This study contributes valuable insights into the patterns of micropollutant distribution in thermal pools and emphasises the necessity for enhanced monitoring and management strategies to mitigate environmental and public health risks posed by micropollutants, as well as the possible further impact on aquatic ecosystems.
