Introduction: This study focuses on the treatment of painful crises in Sickle Cell Disease (SCD) patients using Paracoxib, a non-opioid, compared to Morphine. It addresses the need for alternative medications that reduce opioid dependency while providing effective analgesia.
Objectives:
Primary: Evaluate the analgesic effect of Paracoxib versus Morphine in SCD vaso-occlusive crises.
Secondary: Reduce opioid use/dependence, decrease the length of hospital stays, and monitor side effects related to Paracoxib.
Methodology: A double-blinded randomized controlled trial, conducted in a tertiary care emergency department. The study includes adult SCD patients with moderate to severe crises, excluding non-VOC pain, certain medications, and specific medical conditions. The sample size is 226 patients, split equally into two groups.
Intervention: Patients receive either Morphine or Paracoxib, with periodic assessment of vital signs and pain. Additional Morphine is administered if required. Data collection and analysis are meticulously planned.
Expected Outcomes: Improvement in SCD pain management, reduction in opioid usage, and potential benefits in terms of hospital stays and patient satisfaction.

Exploring the Effect of Preventive Analgesia with Oxycodone and Parecoxib Sodium on the Chronification of Acute Pain in Adult Patients Underwent Laparoscopic-assisted Abdominal Major Surgery

The goal of this clinical trial is to learn if the administration of oxycodone for preventive analgesia works to reduce the chronification of acute postoperative pain in adult participants undergoing major abdominal surgery. The main question it aims to answer is:
• Does administration of oxycodone before surgical incision reduce the occurrence of chronic pain or relieve the chronic pain symptoms 3 months after the operation? Researchers will compare oxycodone + parecoxib sodium to placebo + parecoxib sodium combinations to see if oxycodone works to reduce the chronification of acute postoperative pain.
Participants will:
* Take oxycodone + parecoxib sodium or placebo + parecoxib sodium combinations before the surgical incision
* Cooperate with the follow-up staffs and complete some checkups or tests within 3 months after the operation.

Synergistic therapeutic effect of parecoxib and ilomastat combination in osteoarthritis via inhibition of IL-17/PI3K/AKT/NF-κB activity

BACKGROUNDOsteoarthritis is a degenerative disease, and current drug treatment is to give nonsteroidal anti-inflammatory drugs to relieve symptoms. The anti-inflammatory ability of parecoxib and ilomastat has been confirmed, but the synergistic effect of combined administration in osteoarthritis has not been clear.METHODSMouse primary chondrocytes stimulated with IL-1β were cultured. The expression levels of inflammatory cytokines and matrix metalloproteinases were investigated by western blotting, quantitative real-time polymerase chain reaction and ELISA. The effects of parecoxib and ilomastat on chondrocyte apoptosis were evaluated by flow cytometry. In addition, the rat model of osteoarthritis was established by meniscal instability, and the morphological changes of cartilage and the expression levels of related molecules were monitored using Safranin O-Fast green and immunohistochemical staining after intra-articular injection of parecoxib, ilomastat, and the combination of the two.RESULTSIn vitro experiments showed that the combined administration of parecoxib and ilomastat more effectively inhibited the expression of proinflammatory factors and matrix metalloproteinases compared with single drug administration. The combined drug treatment could more effectively inhibit IL-1β-induced chondrocyte apoptosis. The combined drug treatment alleviated the progression of osteoarthritis by inhibiting the IL-17/PI3K/AKT/NF-κB pathway. In addition, in vivo experiments showed that the combined administration could improve the further deterioration of the osteoarthritis rat model.CONCLUSIONSThe combined administration of parecoxib and ilomastat to inhibit IL-17/PI3K/AKT/NF-κB transduction is beneficial to reduce the infiltration of inflammatory factors and matrix metalloproteinases in osteoarthritis.

Non-Opioid Analgesics for Postoperative Pain Management Following Total Joint Arthroplasty: A Systematic Review and Meta-Analysis

BACKGROUNDHip and knee arthroplasty are a fundamental part of modern orthopaedic practice. These procedures often require extensive postoperative analgesia, including opiate painkillers that are frequently associated with adverse side effects and risk of abuse. This review, therefore, investigated how postoperative administration of nonopioid coanalgesics (e.g., parecoxib, pregabalin) can affect postoperative pain scores and opioid use.METHODSA systematic search of OVID, Embase, Medline, and PubMed was conducted, and studies were filtered to trials of patients undergoing arthroplasty who received nonopioid analgesia. Several meta-analyses were conducted to investigate the effects of various medications and classes at multiple postoperative time points on pain scores and opioid use. Standardized mean differences were conducted for analyses involving more than one measure of pain. There were 28 analyses included in the final review.RESULTSOn average, nonsteroidal anti-inflammatory drugs (NSAIDs) and gabapentinoids reduced between 9.30 and 10.89 mg, respectively of intravenous morphine in a 24-hour period. Reductions were also observed at various time points for NSAIDs, gabapentinoids, parecoxib, and pregabalin. Nonopioid coanalgesia improved pain at rest for various time points, including NSAIDs at postoperative day (POD) 1 and PODs 3 and 5. Parecoxib and corticosteroids were protective at POD 3. Pain during movement was significantly reduced by NSAIDs at 6 hours, POD 1, 2, and 3, with parecoxib demonstrating similar benefits at POD 1 and POD 2 to 3.CONCLUSIONSThis review found that postoperative administration of nonopioid coanalgesia may alleviate the need for opioids and have modest protective effects on postoperative pain. The effects of these analgesics, however, may be clinically nonmeaningful for reducing pain. These results may further implicate a need to implement nonopioid coanalgesia in postoperative patient care. Future research may include an updated investigation of this research question as more medication granular research becomes available.

Effects of Parecoxib Sodium on Early Cognitive Impairment and Inflammation Levels in Burned Rats

AbstractTo study the effect of parecoxib sodium in alleviating inflammation in burned rats and restoring cognitive function in burned rats. A total of 30 specific pathogen free grade Sprague–Dawley rats were randomly divided into 6 groups: (1) blank control group (group C), (2) Sham surgery group (group Sham), (3) second-degree burn model (group B), (4) low-dose (1 mg/kg/d) parecoxib sodium (group L + B), (5) medium-dose (10 mg/kg/d) parecoxib sodium (group M + B), and (6) high-dose (20 mg/kg/d) parecoxib sodium (group H + B). ELISA measures inflammatory factors interleukin (IL)-2, IL-6, tumor necrosis factor-alpha (TNF-α), and interferon-gamma (IFN-γ), and cognitive function factors neuron-specific enolase (NSE), cortisol, and S-100β. Combined with water maze and dark-avoidance experiments to further verify the recovery of cognitive function in rats. The contents of IL-2, TNF-α, and IL-6 in group M + B were significantly lower than those in group Sham (P < .05), and the content of IFN-γ was significantly lower than that in group Sham (P < .05). The cognitive markers NSE, S-100β, and cortisol levels in group M + B were significantly higher than those in group Sham at 2 h, 1 d, 5 d, and 10 d after operation (P < .05). In the group M + B dark-avoidance experiment, the number of probes and errors was not significantly different than those in group Sham and group C (P > .05), and the number of times group M + B found a platform in the water maze experiment and crossed the platform was second only to group B and group C. Parecoxib sodium can effectively reduce inflammation in burn rats and promote cognitive recovery in burn rats, and the optimal dose of parecoxib sodium for burn rats is 10 mg/kg.
