This phase III trial studies using risk factors in determining treatment for children with favorable tissue (histology) Wilms tumors (FHWT). Wilms Tumor is the most common type of kidney cancer in children, and FHWT is the most common subtype. Previous large clinical trials have established treatment plans that are likely to cure most children with FHWT, however some children still have their cancer come back (called relapse) and not all survive. Previous research has identified features of FHWT that are associated with higher or lower risks of relapse. The term "risk" refers to the chance of the cancer coming back after treatment. Using results of tumor histology tests, biology tests, and response to therapy may be able to improve treatment for children with FHWT.

Prospective Treatment of Types I, II and III Pleuropulmonary Blastoma (PPB)

This phase III trial tests how well surgery plus chemotherapy compared to surgery alone works in treating patients with type I pleuropulmonary blastoma (PPB), and tests how well surgery plus standard chemotherapy with the addition of topotecan works compared to surgery plus standard chemotherapy alone in treating patients with type II and III PPB.
Historically, most children with type I PPB had surgery and approximately 40% of children with type I PPB received chemotherapy following their surgery, usually for 22-42 weeks. There has not been a consistent standard for which children with type I PPB receive chemotherapy after surgery. For patients whose tumor has been removed completely with surgery, observation without chemotherapy may work as well as giving chemotherapy after surgery in preventing a return of the PPB tumor.
The standard chemotherapy for patients with types II or III PPB in the United States is four cycles of IVADo (ifosfamide, vincristine, dactinomycin, and doxorubicin) followed by 8 cycles of IVA (ifosfamide, vincristine and dactinomycin). Ifosfamide is in a class of medications called alkylating agents. It works by slowing or stopping the growth of tumor cells in the body. Vincristine is in a class of medications called vinca alkaloids. It works by stopping tumor cells from growing and dividing and may kill them. Dactinomycin is a type of antibiotic that is only used in cancer chemotherapy (antineoplastic antibiotic). It works by damaging the cell's deoxyribonucleic acid (DNA) and may kill tumor cells. Doxorubicin is in a class of medications called anthracyclines. Doxorubicin damages the cell's DNA and may kill tumor cells. It also blocks a certain enzyme needed for cell division and DNA repair. Topotecan is in a class of medications called topoisomerase I inhibitors. It works by interfering with tumor cell DNA which kills them. Giving topotecan in addition to standard IVADo and IVA chemotherapy regimens may shrink the cancer as well as or better than the standard therapy or could decrease the chance the tumor spreads while causing fewer side effects.

Efficacy and Safety of Toripalimab Plus Actinomycin-D As Fist-Line Treatment in Patients with Gestational Trophoblastic Neoplasia with FIGO Score 7: a Single-Arm, Multicenter, Phase II Trial

The goal of this clinical trial is to evaluate the efficacy and safety of toripalimab plus actinomycin-D as fist-line treatment in patients with gestational trophoblastic neoplasia with FIGO score 7. The main questions it aims to answer are:
* Whether toripalimab plus actinomycin-D as fist-line treatment can achieve a high complete response rate.
* Whether an equally high cure rate can be achieved by multi-drug chemotherapy as second-line treatment in patients who have failed fist-line treatment with toripalimab plus actinomycin-D.
Participants will receive toripalimab plus actinomycin-D. Treatment will be continued until disease progression, unacceptable toxicity, or withdrawal of consent. Treatment will be completed after 4 consolidation cycles.

Epigenetic modification mediated by PHF20/METTL14/HOXA13 signaling axis modulates osteogenic differentiation of mesenchymal stem cells

This study investigates the mechanism of PHF20 in osteogenic differentiation of bone marrow mesenchymal stem cells (BMSCs). BMSCs from Balb/c mouse were cultured and identified through osteogenesis, adipogenesis, and flow cytometry. After osteogenic induction, the levels of OPN and OCN in BMSCs were detected by RT-qPCR. Alizarin red staining and alkaline phosphatase (ALP) staining were used to evaluate the osteogenic differentiation ability of BMSCs. PHF20, METTL14, and HOXA13 expressions were detected by RT-qPCR and Western blot. After quantitative analysis of m6A level, RNA immunoprecipitation (RIP) was performed to measure the enrichment of IGF2BP3 or m6A on HOXA13 mRNA. HOXA13 mRNA stability was assessed after actinomycin D treatment. PHF20, METT14, and HOXA13 expressions gradually increased during osteogenic differentiation of BMSCs. Suppression of PHF20 expression repressed the osteogenic differentiation of BMSCs, mainly resulted in a decrease in OPN and OCN levels, reduced mineralization, and weakened ALP activity. Mechanistically, PHF20 elevated METTL14 expression by enhancing the enrichment of H3K4me3 on its promoter, and METTL14 strengthened HOXA13 m6A methylation to maintain HOXA13 mRNA stability through IGF2BP3. In conclusion, PHF20 elevates METTL14 expression by enhancing H3K4me3 enrichment on its promoter and enhances HOXA13 mRNA stability via IGF2BP3-mediated m6A modification, thus facilitating HOXA13 expression and eventually inducing osteogenic differentiation of BMSCs.

What Every Pediatric Surgeon Should Know About Common Systemic Therapies for the Treatment of Renal Tumors in Children and Adolescents

INTRODUCTION:

Renal tumors are among the most common solid pediatric malignancies. The mainstay of treatment is surgical intervention, with chemotherapy and radiation given according to risk stratification schemes. Given the potential for surgery and chemotherapy to occur in proximity, it is important for surgeons to understand the impact of these systemic therapies. The purpose of this review is to provide a comprehensive overview for pediatric surgeons of the most common chemotherapeutic agents and targeted therapies utilized in the treatment of pediatric renal malignancies.

METHODS:

A literature search was conducted for the mechanism of action, indications, and surgical considerations for the following chemotherapeutic agents and targeted therapies, which comprise the drugs used in the treatment of children and adolescents with renal tumors: vincristine, actinomycin D, doxorubicin, cyclophosphamide, carboplatin, etoposide, irinotecan, topotecan, nivolumab, axitinib, tazemetostat, everolimus, and bevacizumab.

FINDINGS:

As expected, the most common side effects that warrant careful consideration in surgical planning include myelosuppression, impacting wound healing and bleeding risks, and solid organ toxicities including cardiac, renal, hepatic and pulmonary toxicities. The incidence and severity of these complications varies across therapies, and timing of expected recovery varies as well.

CONCLUSIONS:

This review provides a comprehensive overview of 11 systemic therapies utilized in the treatment of pediatric renal malignancies. It is intended to serve as a tool for surgeons to provide a holistic approach to patient care, and better plan surgical procedures to ensure the greatest efficacy and safety profile.

DFT_ANPD: A dual-feature two-sided attention network for anticancer natural products detection

The exploration of anticancer drugs has aimed at more effective, adaptable, and less harmful treatments, with natural products pivotal in cancer research. In addition to experimental techniques for identifying anticancer drug candidates, computational methods have been developed to virtually screen for potential anticancer compounds. This study introduces DFT_ANPD, a deep-learning framework for predicting anticancer properties in natural compounds by integrating molecular structural information with embeddings generated by large language models (LLMs). Its architecture combines structural and semantic insights to form enriched molecular representations. SMILES strings are processed by a 1D-CNN to uncover intricate chemical patterns, while a fine-tuned SMILES-BERT extracts semantic information. A two-sided attention mechanism fuses these features, selectively prioritizing chemically relevant features for a holistic molecular representation. Based on this comprehensive embedding, a sigmoid activation function predicts anticancer potential. Benchmarked against iANP-EC with the NPACT and CancerHSP datasets, DFT_ANPD demonstrated superior performance in AUC-ROC, AUC-PR, balanced accuracy, and MCC. Its generalization ability was further validated using the independent NPASS dataset, identifying and ranking the top 10 compounds with the highest anticancer potential, including the FDA-approved dactinomycin. Docking analysis of NPC479359, the sixth-ranked compound, revealed significant binding affinities with top five cancer-related proteins, such as the CB2 receptor and DNA lyase. These results underscore DFT_ANPD's utility in accelerating anticancer drug discovery across experimental-computational pipelines.

Concord Biotech files DRHP for IPO

Ahmedabad-based Concord Biotech, one of the leading global developers and manufacturers of select fermentation-based APIs across immunosuppressants and oncology in terms of market share, based on volume in 2021 has filed its Draft Red Herring Prospectus (DRHP) with the markets regulator, Securities and Exchange Board of India (SEBI), to raise funds through an initial public offering (IPO). The issue with a face value of Re 1 per equity share is a complete offer for sale (OFS) aggregating to 20,925,652 equity shares by Helix Investment Holdings. The offer also includes a reservation for a subscription by eligible employees. The offer is being made through the Book Building Process, wherein not more than 50 per cent of the offer shall be available for allocation to Qualified Institutional Buyers, not less than 15 per cent of the offer shall be available for allocation to non-institutional bidders and not less than 35 per cent of the offer shall be available for allocation to retail individual bidders. ("The Company") is furthering progress in discussions with investors and developing the financing model towards the next step of the development process with its Nanoemulsion drug delivery system. The Company is confident that it will progress towards this next step. The early feasibility study was intended to serve as a demonstration that the development team possessed the capability of formulating research-scale nanoemulsion formulations of dactinomycin. Initial characterization of these formulations would be carried out to demonstrate that the nanoemulsion formulation possessed several desirable characteristics that made them suitable candidates for further nonclinical development. As stated the Company is confident these steps were met and looks forward to moving ahead. Essentially, all of the specific aims under the scope of this project were met. The capability to formulate nanoemulsions that incorporate the active pharmaceutical ingredient, dactinomycin, was demonstrated. Specifically, the study identified two formulations feasible for further development. The capability to be able to innovate and make progress in this early stage demonstrates the team's ability to identify developmental goals and overcome formulation challenges to produce a drug formulation with suitable characteristics for further, formal nonclinical development. These characteristics include the ability for the nanoemulsion to incorporate the majority of available drug into the lipid nanoparticles, a demonstrated release-over-time profile, and stability during storage. Additionally, test methods were established at independent contract laboratories to support basic formulation characterization and accurately detecting the drug concentration in control and test formulations in support of future nonclinical studies.
