Tropicamide Versus Cyclopentolate for Cycloplegic Refraction in Pediatric Patients With Esotropia: A Randomized Clinical Trial

The primary outcome of this study is to compare cyclopentolate 1% and tropicamide 1% for cycloplegic refractions in pediatric populations with esotropia. This will be a prospective double-blinded randomized clinical trial (RCT), multi-center, with randomized sequencing of cycloplegic agent; each patient received one agent at one visit, and the other agent in the next visit (2 different visits ≥ 1 week apart) within 3 months.

Influence of lens thickness on the accommodative range in healthy eyes

BACKGROUNDThe maximum accommodative range is a useful indication of visual function. It decreases with age, but the exact cause of this decrease is not fully understood. It is associated with the increasing rigidity of the lens and changes to the lens shape, as well as the geometry of the zonular attachments. This work aims to determine the relationship between the accommodative range and lens shape in a group of healthy young adults.METHODSThis study includes 40 emmetropic participants between 20 and 30 years of age. They filled in a questionnaire and underwent accommodation measurements using the RAF Convergence Rule and the Negative Lens Test. After a drop of Tropicamide 0.5 % and Cyclopentolate 1 %, participants underwent measurements with autorefractor, Scheimpflug tomography (Pentacam HR, Oculus), anterior segment Optical Coherence Tomography and optical biometry.RESULTSThe accommodative range did not correlate significantly (p>0.05) with any of the lenticular or ocular parameters considered, such as lens thickness, lens radii of curvature, axial length, or corneal power.CONCLUSIONThe crystalline lens shape does not affect the accommodative range of the eye. This may be due to the smaller deformation required to bring a thicker lens to the same level of accommodation as a thinner lens. The amount of force on the zonular fibres may therefore be similar, in all lenses, regardless of the lens shape.

Impact of proxymetacaine on the dynamics of cyclopentolate in White 6‐ to 7‐year‐olds

AbstractPurposeThis study compared the efficacy of cyclopentolate hydrochloride at 10‐, 20‐ and 30‐min post‐instillation in White 6‐ to 7‐year‐olds, with and without prior instillation of proxymetacaine hydrochloride. The primary aim was to determine if accurate autorefraction values can be obtained sooner than the current standard of 30‐min post‐cycloplegia. The secondary aim was to investigate whether proxymetacaine hydrochloride enhances the efficiency of cyclopentolate.MethodsParticipants were 112 White 6‐ to 7‐year‐olds from the Child Eye Health Study. The right eye received 0.5% proxymetacaine hydrochloride and 1.0% cyclopentolate hydrochloride, and the left eye received only 1.0% cyclopentolate hydrochloride. Non‐cycloplegic and cycloplegic refractive error (at 0, 10, 20 and 30 min) was measured using a binocular, open‐field autorefractometer. Data were analysed through paired t‐tests, concordance analysis, linear regression, equivalence testing and Bland–Altman analysis, using the 95% limits of agreement.ResultsMean spherical equivalent refraction (SER) (SD) in the right eye at 0‐, 10‐, 20‐ and 30‐min post‐instillation was 0.62 (1.45) D, 1.52 (1.80) D, 1.64 (1.81) D and 1.72 (1.80) D, respectively. Mean left eye SER (SD) were 0.68 (1.24) D, 1.42 (1.66) D, 1.56 (1.66) D and 1.68 (1.72) D, respectively. Bland–Altman analysis showed a high level of agreement, and equivalence testing confirmed that there was no clinically significant difference in SER at 20 and 30 min in both eyes (within ±0.50 D), with mean differences of 0.08 (0.23) D in the right eye and 0.13 (0.30) D in the left eye (p = 0.21). However, SER at 10 and 30 min were equivalent in the right eye only.ConclusionsAccurate autorefraction values can be obtained 20‐min post‐instillation of 1.0% cyclopentolate in white children aged 6–7 years, potentially reducing clinical testing times. Proxymetacaine pre‐instillation allows for reliable measurements as early as 10‐min post‐instillation of cyclopentolate. Further research is needed to validate these findings in non‐White populations and to determine the safe discharge time post‐proxymetacaine instillation.

In-Office Suprachoroidal Viscopexy for Acute Rhegmatogenous Retinal Detachment

ImportanceIn-office suprachoroidal viscopexy (SCVEXY) is a relatively new procedure for rhegmatogenous retinal detachment (RRD), but minimal information is available regarding outcomes and safety.ObjectiveTo report outcomes with in-office SCVEXY for primary acute RRD.Design, Setting, and ParticipantsThis retrospective case series was conducted at St Michael’s Hospital in Toronto, Ontario, Canada from June 2023 to February 2024 among consecutive patients with primary acute RRDs who presented with retinal tears that were reachable with the current in-office SCVEXY technique in the temporal or nasal retina.ExposureSuprachoroidal injection of 0.6 mL of sodium hyaluronate, 2.3%, at the break location using a 30-gauge needle with a custom-made guard leaving 1 mm of the needle exposed. Laser retinopexy was applied around the break once reattachment was achieved.Main Outcome and MeasuresThe primary outcome was primary anatomic reattachment rate with SCVEXY and recovery of function and anatomic integrity.ResultsAmong 6 patients, 2 patients (33.3%) were female, and mean (SD) patient age was 52.5 (19.7) years. The final follow-up duration for each patient was 510, 420, 360, 360, 330, and 320 days, respectively. Baseline VAs were hand motions, counting fingers, 20/100, 20/100, 20/25, and hand motions for patients 1 through 6, respectively. A dome-shaped suprachoroidal buckle was present in all cases after the procedure. Five of 6 patients (83.3%) achieved anatomic reattachment with no retinal displacement or outer retinal folds, and these patients could resume normal activity immediately after the procedure with no restrictions. The mean (SD) logMAR VA at 3 months was +0.46 (0.34) (Snellen 20/57). The viscoelastic persisted in the suprachoroidal space as confirmed by multimodal imaging for 10, 14, 21, 13, 24, and 14 days, respectively, and the retina remained attached during the entire postoperative follow-up period. One patient’s SCVEXY failed due to inadequate viscoelastic at the posterior edge of the retinal break, and this patient underwent a successful pneumatic retinopexy. One patient developed a localized choroidal effusion unrelated to the SCVEXY site following laser retinopexy that resolved in 1 week associated with topical prednisolone, 1%, 4 times daily and cyclopentolate, 1%, twice daily.Conclusions and RelevanceIn this case series, the outcomes of these 6 cases suggest that SCVEXY may offer durable retinal reattachment in RRD. However, randomized clinical trial data, larger sample sizes, and longer-term follow-up are necessary to assess the risk-benefit profile of SCVEXY compared with the standard of care.
