Diabetic Charcot neuroarthropathy: A threat to both limb and life

AbstractDiabetic Charcot neuroarthropathy (DCN), first described in 1936, occurs in less than 1% of diabetic patients, but in those diabetic subjects with distal symmetrical polyneuropathy, the overall incidence increases to 30% and the risk is even greater in those with type 1 diabetes. Factors that precipitate DCN are trauma, ischaemia due to arterio‐venous shunting, increased osteoclastic activity and inflammation. DCN usually presents with a painless swollen foot and/or ankle which is ‘hot to the touch’. These clinical findings are soon followed by characteristic magnetic resonance imaging (MRI) abnormalities and later X‐ray changes. The joints that are most typically involved in chronological order are the tarsometatarsals followed by the naviculocuniform, sub‐tarsal, talonavicular and metatarsal and tarsophalangeal. The cornerstone of therapy is prolonged (3–12 months) offloading with immobilization. Bisphosphonates may possibly accelerate recovery, whereas other unproven possible therapies include rhPTH, 1–34, calcitonin and methylprednisolone, which are not only ineffective but in some cases may also prolong the time to healing. Denosumab is potentially an efficacious, if unproven, therapy to accelerate healing. The risk of amputation is high and increases in the presence of a foot ulcer. DCN is associated with manifestations of autonomic neuropathy, including cardiac denervation, so that the risks of a cardiac event and heart failure are increased with DCN. Mortality is also increased with DCN, especially in the presence of a foot ulcer. To avoid the recurrence of DCN and especially to lower the risk of the recurrence of a foot ulcer recurrence reconstructive, surgery may be needed.

ABSTRACTMedullary thyroid carcinoma (MTC) is an uncommon neuroendocrine tumour that is usually asymptomatic at its onset and therefore may not present clinically until the patient has developed advanced or metastatic disease. Common metastatic sites include cervical lymph nodes, liver, bone and lung. This is the case of a patient who presented with an incidental posterior mediastinal mass. Because the posterior mediastinum is an unusual location for MTC, MTC was not a consideration and preliminary histopathological testing did not include calcitonin, which would have been diagnostic. This case highlights the importance of testing for calcitonin more regularly when encountering a mass of unknown origin with neuroendocrine morphology, which may lead to earlier detection of MTC and thus improved prognosis.

Impact of comb type on production performance, egg production quality, reproductive performance, and blood biochemical indexes in Tianfu chickens

The comb, as a secondary sexual characteristic in chickens, serves as an important indicator of sexual maturity and potential reproductive. This study explored the relationship between comb types and production performance in Tianfu G01 chickens. Single-comb hens exhibited significantly lower laying rates compared to rose-comb hens during specific weeks (24-26, 27-28, 30, 32-34, and 38-43) but had higher average egg weights at 22, 23, 33, and 36-43 weeks. There were no significant differences in initial egg weight or 300-day body weight between the comb types. However, rose-comb hens showed higher 300-day egg production and green-shell egg rates, while single-comb hens had better egg weight and survival rates. The uterine morphology of single-comb hens was superior, with longer and wider uterine villi. Blood analysis revealed higher calcitonin and parathyroid hormone levels in single-comb hens, while rose-comb hens had higher blood calcium and phosphorus levels. In roosters, single-comb had higher germ cell counts, thicker seminiferous epithelium, and better semen quality, leading to improved fertilization rates and hatchability. These findings suggest that comb type influences both reproductive and production traits in Tianfu G01 chickens, with rose-comb favoring egg production and single-comb favoring egg weight and survival.
