Inadequate bowel preparation compromises colonoscopy quality and diagnostic accuracy, and cirrhosis is a recognized independent predictor of poor bowel cleansing. However, no bowel preparation regimen has been prospectively validated or specifically tailored for cirrhotic patients.
This multicenter, prospective, randomized, single-blind controlled clinical trial will evaluate whether the addition of adjunctive measures as an intensified bowel preparation protocol improves bowel cleansing quality in adult patients with cirrhosis undergoing elective outpatient colonoscopy.
Participants will be randomized 1:1 to receive either a standard bowel preparation protocol, consisting of a 2-litre split-dose polyethylene glycol (PEG) regimen combined with a one-day low-residue diet and clear liquids the afternoon before the procedure (control), or the same split-dose regimen with the assigned adjunctive measures: 15 mg bisacodyl, a 3-day low-residue diet, and clear liquids the day before colonoscopy (intervention).
The primary outcome is the proportion of patients achieving adequate bowel preparation, defined as a Boston Bowel Preparation Scale (BBPS) total score ≥6 with no individual segment score <2. Secondary outcomes include polyp, adenoma, advanced adenoma and colorectal cancer detection rates, caecal intubation rate, patient compliance, tolerability, and adverse events. Pre-specified subgroup analyses will evaluate the influence of etiology and severity of cirrhosis and portal hypertension complications.
By addressing a critical and unmet clinical need, this trial aims to generate high-quality evidence to optimize bowel preparation strategies in patients with cirrhosis, improve colonoscopy quality, and ultimately enhance colorectal cancer screening outcomes in this vulnerable population.

Multi-Centre Pragmatic Randomized Controlled Trial of Bowel Preparation Before Peritoneal Dialysis Catheter Insertion

Doctors need to insert catheter for patients who are going to have peritoneal dialysis. These peritoneal catheters can be obstructed or blocked afterward. The chance of not having smooth flow can be up to 12 and 31%. So far, constipation is one of the proposed reasons. Therefore, the study is aimed to investigate whether the preventive use of laxative before the surgery can help better improve the function of the catheter and hence better flow.

Precision Network Pharmacology Revealed by High-throughput Blind Docking: Deciphering the Multitarget Synergy of Taohe Chengqi Decoction against Constipation

Introduction::

Constipation, prevalent globally (16–35%), is particularly common in the elderly, reducing quality of life and causing complications like intestinal obstruction. Current treatments (e.g., methylcellulose, polyethylene glycol, bisacodyl, prucalopride) provide temporary relief but have side effects, such as electrolyte disturbances and drug dependence. It is worth noting that the traditional Chinese herbal drug Taohe Chengqi Decoction (TCD) has demonstrated good laxative efficacy and safety advantages in long-term clinical practice. However, its systematic mechanism of action involving multiple components, multiple targets, and multiple pathways remains unclear.

Methods::

This study innovatively integrates network pharmacology prediction, high-throughput blind molecular docking assisted by machine learning (DiffDock), and molecular dynamics simulation to construct a multidimensional mechanism model for TCD in treating constipation.

Results::

A total of 115 active molecules were identified from TCD through high-throughput blind docking and network pharmacology, targeting 153 constipation-related proteins. After an 80 ns molecular dynamics simulation, six core components demonstrated stable binding with key targets like AKT1 and MAPK3.

Discussions::

TCD exerts its therapeutic effects by intervening in key signaling pathways such as PI3K-Akt, IL-17 inflammation, T cell receptor, MAPK, and cAMP. It regulates the dynamic balance of intestinal epithelial cell proliferation/apoptosis, inhibits inflammatory cascades, and enhances intestinal motility and secretory functions to relieve constipation.

Conclusion::

The innovative integration of machine learning-driven high-throughput blind docking technology with network pharmacology establishes a novel paradigm for traditional Chinese medicine research, significantly enhancing target prediction reliability through advanced structural validation.

Investigating Immune Microenvironment of Inflammatory Bowel Disease and the Mitochondrial Metabolism-Related Molecular Mechanisms: Combining a Multiomics Approach and Experimental Validation

Inflammatory bowel disease (IBD) is a chronic and recurrent gastrointestinal disease, the pathogenesis of which has not been fully elucidated. Increasing evidence suggests that the disorder of mitochondrial metabolism is closely related to the pathogenesis of IBD, but its specific regulatory network and key genes remain to be further investigated. IBD-related transcriptome datasets (GSE3365 and GSE75214) and single-cell sequencing dataset (GSE134809) were obtained from the Gene Expression Omnibus database. Differentially expressed genes and hub genes were identified through differential expression analysis and weighted gene co-expression network analysis, and candidate genes were obtained by intersecting these with mitochondrial metabolism-related genes, followed by functional enrichment analysis. Machine learning algorithms were used to screen key genes and construct risk prediction models. Additionally, analysis of GSE134809 single-cell data identified characteristic cell types and expression distribution of key genes in IBD and explored communication between different cell types. Furthermore, immune cell infiltration, competitive endogenous RNA (ceRNA) network, and transcription factor prediction were performed. Finally, the diagnostic performance of key genes was validated in GSE75214 and reverse transcription–quantitative polymerase chain reaction. Two key genes, mitochondrial ribosomal protein L35 ( MRPL35 ) and MRPL39 , were identified, which were downregulated in IBD, and had good diagnostic potential. Single-cell analysis revealed that key genes were predominantly highly expressed in mononuclear phagocyte (MNP) cells. MNP cells communicated with other cells through receptor ligands including MIF-(CD74 + CXCR4), MDK-SDC1, and ITGB2-ICAM2, which are complexly related to mitochondrial metabolism. With the progression of IBD, infiltration levels of resting natural killer cells, naive B cells, M2 macrophages, and naive CD4 T cells decreased, and correlations between different cells continuously changed. A ceRNA network centered on XIST, hsa-miR-103a-3p, and MRPL35 was constructed. Additionally, therapeutic drugs targeting key genes were predicted, including cimetidine, eugenol, chlortetracycline, vincristine, irinotecan, bisacodyl, and sulpiride, with molecular docking validating high affinity between these drugs and key targets. This study constructed a multiomics integrated analysis strategy and identified MRPL35 and MRPL39 as potential markers and therapeutic targets, providing new insights for the diagnosis and treatment of IBD.

Potential drugs for reducing the occurrence of immune checkpoint inhibitor-induced interstitial lung disease: an exploratory study using the JADER and FAERS databases

BACKGROUND:

Immune checkpoint inhibitors (ICIs) play a central role in cancer immunotherapy. However, the occurrence of immune-related adverse events, especially ICI-induced interstitial lung disease (ICI-ILD), is life-threatening and affects the effectiveness of ICI treatment. This study aimed to explore potential drugs to mitigate ICI-ILD occurrence using data from the Japanese Adverse Drug Event Report (JADER) and the US Food and Drug Administration (FDA) Adverse Event Reporting System (FAERS [JAPIC AERS]).

RESEARCH DESIGN AND METHODS:

We investigated concomitant drugs that reduce ILD associated with four ICIs - nivolumab, pembrolizumab, atezolizumab, and durvalumab - across the JADER and FAERS databases. Subsequently, the identified common concomitant drugs that reduce the occurrence of ICI-ILD were detected and analyzed.

RESULTS:

We found omega-3 fatty acids, loperamide, and amlodipine as common concomitant drugs that reduced ICI-ILD occurrence in both the JADER and FAERS databases. Omega-3 fatty acids reportedly have many effects in animal models of drug-induced ILD, including their association with ILD in humans and anti-inflammatory effects against ICI-ILD. However, loperamide and amlodipine reportedly have minimal effects against ILD, thereby necessitating further evaluation.

CONCLUSION:

Omega-3 fatty acids have emerged as potential agents for reducing ICI-ILD occurrence, as evidenced by findings from two different pharmacovigilance databases.

While nine out of ten men surveyed (92%1) use their local pharmacy, almost three-quarters (74%2) of them leave without ever discussing their health with a pharmacist, a new report has revealed. This disconnect highlights a significant opportunity to catch illnesses earlier and alleviate pressure on the NHS. The report, "An Unfilled Prescription," published by the Men’s Health Forum and supported by global consumer health leader Opella, highlights a critical opportunity to convert men's regular presence in pharmacies into greater engagement with preventative health services. With UK men’s life expectancy four years shorter than women's, and over a third dying before their 75th birthday, the findings underline the valuable role the pharmacy sector can play as part of a wider effort to improve men’s health. The research identified the main barriers preventing men from living healthier lives, which, according to the men surveyed, include: Discomfort discussing health issues in a public retail space (22%3). A perception that seeking help is a sign of weakness (33%). A common tendency to delay seeking medical advice (36%). This silence, however, can have serious consequences. Over half of the men surveyed (57%4) reported that failing to seek advice led to negative impacts on their lives, including missed work (15%) and strained personal relationships (12%).
