Menopur And Rekovelle Combination Study Version 2.0

The goal of this clinical trial is to evaluate the safety and effectiveness of a personalized ovarian stimulation regimen in women aged 18 to 40 undergoing in vitro fertilization (IVF). The main question it aims to answer is:
- Does personalizing the starting doses of follitropin delta (REKOVELLE) and HP-hMG (MENOPUR) based on both age and body lead to similar results as anti-Müllerian hormone (AMH) and weight based dosing ?
Researchers will compare the new dosing regimen to the MARCS study results to see if this new approach leads to similar outcomes
Participants will:
* Receive ovarian stimulation using a personalized dose of REKOVELLE aND MENOPUR based on age and weight
* Be monitored through ultrasound imaging and blood tests to measure estradiol (E2) levels
* Undergo standard IVF procedures including egg retrieval and embryo assessment

Integrating multi-omics approaches to elucidate the mechanism of XiaoPi Decoction in treating hyperplasia of mammary glands via AMPK signaling pathway activation

BACKGROUND:

Hyperplasia of Mammary Glands (HMG) is a non-inflammatory, non-neoplastic condition characterized by structural abnormalities in breast tissue, resulting from varying degrees of mammary parenchyma and stroma hyperplasia and incomplete regeneration. XiaoPi Decoction (XPD), an empirical formula for HMG in our hospital, has shown clinical efficacy by reducing breast nodule volume, alleviating mammary pain, and regulating sex hormone levels and T lymphocyte subset activity. It also exhibits high safety, but its precise therapeutic mechanisms remain unclear. The primary aim of this study is to systematically elucidate the therapeutic mechanisms of XPD in treating HMG.

METHODS:

First, UPLC-Q-TOF-MS was used to analyze XPD's chemical components. Subsequently, an estrogen-progesterone induced HMG rat model was established to verify XPD's efficacy and explore its molecular mechanisms. Network pharmacology (with multi-database cross-validation) and transcriptomic analysis (on mammary tissue of control, model, and medium-dose XPD groups) were applied to identify potential molecular targets and pathways. Additionally, non-targeted metabolomics (on the same three groups as transcriptomics) was used to assess metabolic changes in HMG rats after XPD intervention, with a three-tier quality control protocol to mitigate batch effects.

RESULTS:

Fifty-two chemical components were identified in XPD, mainly including thioglucosides, phenolic acids, and alkaloids. Animal studies showed XPD normalized mammary tissue structure and hormone levels: it decreased estradiol (E2), prolactin (PRL), luteinizing hormone (LH), and follicle-stimulating hormone (FSH) concentrations, increased progesterone (P) levels, and downregulated mRNA expression of estrogen receptor alpha (ERα), progesterone receptor (PR), and prolactin receptor (PRLR) in mammary tissue. Network pharmacology and transcriptomics indicated XPD targeted the AMPK/mTOR/HIF1/VEGF signaling pathways. Metabolomics revealed XPD affected fatty acid biosynthesis. Western blot confirmed XPD reduced protein expression of AMPK, mTOR, ACC, p62, and VEGF, while upregulating ULK1 and TSC2 expression and their phosphorylation.

CONCLUSION:

This study suggests XPD exerts therapeutic effects on HMG by modulating hormone levels. Through AMPK activation, XPD inhibits fatty acid synthesis and energy metabolism, which reduces intracellular lipid accumulation and suppresses excessive cell proliferation. Additionally, it improves cellular energy status, thereby alleviating abnormal cell proliferation caused by energy metabolic disorders. XPD also promotes autophagy by inhibiting the downstream mTOR pathway, further suppressing cell proliferation. Moreover, it inhibits VEGF expression to exert anti-angiogenic effects, reducing blood supply to hyperplastic breast tissue and restricting its growth-thus contributing to HMG treatment.

Endometrial Expression of Angiogenesis-related Factors Following Ovarian Stimulation and Epigallocatechin Gallate Administration

Abstract:

Background::

AngiomiRs are a specialized subclass of microRNAs that target genes related to angiogenesis, such as Vascular Endothelial Growth Factor (VEGF). Angiogenesis is crucial in all stages of pregnancy and is essential for creating a receptive endometrium for embryo implantation. Both ovarian stimulation and Epigallocatechin Gallate (EGCG), a major component of green tea, can influence angiogenesis. This study aims to examine endometrial quality, with a focus on angiogenesis, as well as serum levels of estradiol and progesterone, following EGCG administration in ovarian-stimulated mice, as a valuable model for studying human reproductive health and diseases.

Methods and Materials::

Forty adult female mice were assigned to four distinct groups: 1) control, 2) ovarian stimulation (7.5 IU HMG followed by 7.5 IU HCG 48 hours later, administered intraperitoneally), 3) EGCG (5 mg/kg EGCG for 4 days, IP), and 4) ovarian stimulation + EGCG groups. Gene expression analysis of miR-16-5p was performed using real-time polymerase chain reaction. VEGF protein and CD31-positive cell density were assessed through immunohistochemistry, and serum estradiol and progesterone levels were measured using ELISA.

Results::

Endothelial cell density, VEGF protein, and miR-16-5p expression and estradiol concentration significantly increased in the ovarian stimulation group compared to the control group ( p < 0.05). The smallest reduction in these parameters was observed in the group that received EGCG. EGCG also significantly reduced the progesterone level ( p < 0.05).

Conclusion::

EGCG significantly reduced endometrial angiogenesis, and angiomiR-16-5p may mediate the effects of EGCG on endometrial quality; however, further studies are needed.

Clinical Pregnancy Rate in Intrauterine Insemination and Associated Prognostic Factors

Abstract:

Background::

Intrauterine insemination (IUI) is a popular method for the treatment of infertility, however, its success rate ranges from 7 to 13% in a cycle.

Aim::

The present study was carried out to assess the IUI success rate and its predictors.

Method::

A prospective observational study was conducted, and 167 women (aged 21-49 years) with primary or secondary infertility were enrolled to undergo intrauterine insemination. The induction of ovulation for IUI was performed according to the standard protocols. Stimulation was done with letrozole/clomiphene/ FSH/HMG or a combination of these drugs. Human Chorionic Gonadotropin (hCG) was administered at achievement of follicular size 17-18 mm. IUI was done 36–40 hours after the hCG trigger. The luteal phase was supplemented wherever needed. A positive urine pregnancy test (UPT) done 2 weeks after IUI was considered as the primary outcome. Predictors of UPT success were evaluated using Chi-square and Independent Samples t-tests.

Results::

The mean age of women was 30.58±(SD)4.43 years. The mean duration of infertility was 4±2.65 years. Majority (77.6%) had primary infertility. UPT positivity rate was 18.2% in <30 years and 21.3% in 30 to 45 years and no positive pregnancy in > 45 years. No significant association of age, BMI, level of physical activity, fertility type, ovulation induction frequency, AMH, sperm count, and HSG was seen with outcome (p>0.05). A significant association of outcome with endometrial thickness was observed( p-value 0.001).

Conclusion::

The IUI success rate was 18.8%. The endometrial thickness was significant predictors of IUI.

The company achieved total revenues of over €2.5 billion in 2025, an increase of 7% over the previous year at actual exchange rates (AER) and 10% at constant exchange rates (CER). These were mainly driven by our flagship product Menopur ® (menotropins for injection) in reproductive medicine, and the ramp-up in the US of our breakthrough gene-based therapy for non-muscle invasive bladder cancer, Adstiladrin ® (nadofaragene firadenovec-vncg). Operating expenses were contained to an increase of €61 million year-on-year (i.e. +5% at AER and +7% at CER), and this includes significant non-recurring items (notably impairment charges and restructuring provisions). Ferring launches online assessment tool, educational platform around male fertility

The overall goal of the initiative is to make the entire fertility journey more equitable for both women and men, which, according to Ferring, may ultimately help “improve drop-out rates, decrease stress, address stigma, reduce unnecessary medical costs and accelerate the path to pregnancy.” Ferring Pharmaceuticals is setting the record straight on fertility issues. Because infertility is often branded largely as a women’s issue, many men may be unaware of their own possible role in the matter and may not undergo testing in a timely manner—even though research has shown that men “substantially” contribute to about half of all cases of infertility. The issue is further compounded by stigma that may make men feel emasculated or embarrassed to be struggling with their fertility. A new initiative from Ferring, however, is aiming to boost awareness and education around male fertility and break down that stigma. According to a company announcement Thursday, the program was launched in partnership with Posterity Health, a virtual care service focused on male reproductive health and is hosted on Ferring’s existing Fertility Out Loud platform, which launched in 2021 to provide support for aspiring parents. The platform’s new addition will specifically target men, to “activate them to learn about their fertility health and how it may impact the couple’s path to parenthood,” according to Ali Castro, senior director of marketing and strategic engagement for Ferring’s reproductive medicine division. To do so, it’ll offer both educational resources and a confidential digital assessment that male partners can use to gain a better idea of their personal reproductive health. The overall goal of the initiative is to make the entire fertility journey more equitable for both women and men, which, according to Ferring, may ultimately help “improve drop-out rates, decrease stress, address stigma, reduce unnecessary medical costs and accelerate the path to pregnancy.” “When a couple is having trouble conceiving, the male partner typically waits 22 months before seeking an evaluation—during this period the female is often receiving unnecessary, and in some cases, invasive care,” Pam Pure, co-founder and CEO of Posterity, said in the announcement. “Working together with Ferring, we have made it easier to complete a male fertility assessment from home. Identifying and addressing male factor issues early in the fertility journey will empower couples to take control of their reproductive health.” Both the new initiative and larger Fertility Out Loud platform are unbranded, hosted on a standalone site with the only mention of Ferring at the bottom of each page. That said, Ferring does offer several fertility-related products, including Menopur and Novarel, both of which are used to induce ovulation in women struggling with infertility, and Endometrin, which provides extra progesterone to women who are undergoing fertility treatments.
