Comparison of the Efficacy of Microneedling Alone Versus Microneedling Coupled With Either Bleomycin or 5flourouracil in the Treatment of Plantar Warts

To compare between the efficacy and safety of microneedling alone, microneedling with topical bleomycin and microneedling with 5-flurouracil in treatment of planter warts.
Microneedling will be performed on each wart using dermapen supplied with 12 needles arranged in rows. Topical anesthetic cream will be applied for 60 min before the procedures. Microneedling will be performed on each wart using dermapen. The penetration depth will be adjusted at 2-mm, endpoint is pinpoint bleeding and the dermapen will be held by the hand in a vertical direction on the warts.
Bleomycin is typically supplied in 15 U vials. The preparation will be diluted first with 7 mL normal saline ,double the amount taken from the vial by adding the same amount lidocaine (2%), so that the concentrations become 1 U/mL . The maximum total amount of bleomycin taken to each patient in one session will be 1 U/1 mL . Microneedling will be performed on the lesion using a microneedling pen type device with a 1-cm tip diameter at a 2-mm depth setting for 2-3 minutes until pinpoint bleebing occurs 1 mL of the prepared solution of bleomycin (1 U/1 mL) will be dropped on to the wart tissue using insulin syringe and occlusion will be applied for 3 hours.
5-FU is available in 10 mL vial: 500 mg/10 mL. Application of 5-FU solution will be done by dropping using an insulin syringe (1 mL)to help dropping of the solution on the warts surface. Then, the lesion will be occluded with a plaster for 3 h.
Sessions: Sessions will be performed every 2 weeks until complete cure or for maximum 6 sessions (total 3 month)

The Efficacy of Intralesional Bleomycin as Compared to 5-Flourouracil (5-FU) and Triamcinolone Acetonide (TAC) for the Treatment of Keloids: A Randomized Control Trial.

The goal of this clinical trial is to learn if drug bleomycin intralesionally is beneficial than intralesional 5FU and Triamcinolone Acetonide in treating Keloids. The main questions it aims to answer are:
Does drug Bleomycin injected intrasleisonally lower the time duration to effectively treat the keloids in participants as compared to intralesional Triamcinolone Acetonide and 5-Flourouracil? Researchers will compare drug Bleomycin and 5FU plus Triamcinolone Acetonide and determine if bleomycin is better.
Participants will:
Treatment will be given in two groups by a researcher blinded of the treatment being given in each group. POSAS patient and observer scar scale will be used to assess the keloids at 0, 2, 4, 6, 8, 10, and 12 weeks and during follow ups afterwards, assessments will be carried out by another researcher who will assess the improvement on the scale of 0-no improvement, to excellent-75-100% improvement in terms of POSAS variables. Effectiveness will be indicated by 50% reduction in POSAS score from the baseline.

A Self-controlled Study of Bleomycin and Pingyangmycin Injection in the Treatment of Plantar Warts

All patients were treated as self-control. Half of the warts received bleomycin injection and the other half received pingyangmycin injection for 24 weeks. During the treatment period, before each injection, photos of the skin lesions must be taken, the diameter of the skin lesions must be measured, the DLQI(Dermatology Life Quality Index) and NRS scores(NRS scores) must be filled in, and adverse events must be recorded. Before the first treatment, HPV(Human Papilloma virus) virus typing is performed on the warts.

Electrochemotherapy, as a novel therapeutic approach in the management of lentigo maligna, lentigo maligna melanoma, and acral lentiginous melanoma

PURPOSE:

Lentigo maligna (LM), lentigo maligna melanoma (LMM), and acral lentiginous melanoma (ALM) are characterized by irregular borders and manifest on highly visible and/or surgically challenging areas. The challenge in treating these melanomas lies in preserving function and achieving satisfactory esthetic outcomes while ensuring complete surgical excision with an appropriate safety margin. We report three cases of elderly patients with LM, LMM, and ALM treated with electrochemotherapy (ECT).

MATERIALS AND METHODS:

All patients were treated according to the European Standard Operating Procedures of Electrochemotherapy protocol. Bleomycin was administered intravenously, followed by electroporation to allow better drug uptake into the tumor cells. The safety margin was ensured by electrode repositioning, and follow-up was scheduled regularly.

RESULTS:

Our patients experienced favorable outcomes: two achieved a complete response, with one requiring adjuvant topical imiquimod for suspected residual disease, while the third patient achieved a partial response. No serious adverse events were observed, and cosmetic results were superior compared to extensive surgery.

CONCLUSION:

ECT appears to be a safe and effective treatment alternative for LM, LMM, and ALM, especially in elderly patients where surgery may lead to significant morbidity. ECT can be used alone or in combination with other therapies, providing a wide safety margin and cosmetically favorable results.

Hesperidin alleviates pulmonary fibrosis by regulating EI24-mediated autophagy

BACKGROUND:

Etoposide-induced protein 2.4 (EI24), an essential component of autophagy, is lowly expressed in pulmonary fibrosis. Hesperidin (Hes), a flavonoid, can regulate autophagy in various diseases. However, whether Hes can inhibit pulmonary fibrosis by mechanically regulating EI24-mediated autophagy has not been uncovered.

METHODS:

RLE-6TN cells were treated with transforming growth factor β1 (TGF-β1) and rats were injected with bleomycin (BLM) to construct the pulmonary fibrosis model. The effect of Hes on pulmonary fibrosis was evaluated by cell counting kit-8, immunofluorescence, hematoxylin and eosin, masson trichome staining and western blotting.

RESULTS:

Hes reduced cell viability of TGF-β1-induced RLE-6TN cells. Administration of Hes restored the decrease in autophagy marker levels in TGF-β1-induced RLE-6TN cells. Hes inhibited the transcriptional and translational levels of α-SMA, collagen I and fibronectin that were increased by TGF-β1 in RLE-6TN cells. Mechanically, Hes restored EI24 expression, and EI24 knockdown reversed the effect of Hes on the expressions of autophagy and fibrosis-related proteins. Additionally, Hes enhanced autophagy and fibrosis markers, which were worsened by EI24 knockdown in BLM-induced rats.

CONCLUSION:

Hes activated autophagy by upregulating EI24, which improved pulmonary fibrosis both in vitro and in vivo.

Human Wharton’s Jelly Mesenchymal Stem Cells and their Extracellular Vesicles in the Management of Bleomycin-induced Lung Injury in Model Animals: A Comparative Preclinical Study Focused on Histomorphometric Analysis

Introduction::

Pulmonary fibrosis, a condition characterized by excessive lung tissue scarring, remains a significant therapeutic challenge. Given the potential of human Wharton’s jelly- derived mesenchymal stem cells (hWJ-MSCs) and their extracellular vesicles (hWJ-MSC-EVs) as minimally invasive and scalable therapeutic options for pulmonary fibrosis in clinical settings, this study investigates the potential of hWJ-MSCs and hWJ-MSC-EVs in mitigating bleomycin-induced lung injury in C57BL/6J mice.

Methods::

hWJ-MSCs were cultured and characterized for their ability to differentiate into osteogenic, adipogenic, and chondrogenic lineages. EVs were successfully induced via serum starvation, purified using ultracentrifugation, and characterized for their protein and nucleic acid content, size distribution, and EV markers. A bleomycin-induced pulmonary fibrosis model was established in C57BL/6J mice. Mice were monitored for weight loss, mortality, and lung fibrosis severity following treatment with hWJ-MSCs and hWJ-MSC-EVs. Histological analysis and Ashcroft scoring were used to assess lung fibrosis.

Results::

Bleomycin administration in mice resulted in significant weight loss, increased mortality, and severe lung fibrosis, as demonstrated by histological analysis and Ashcroft scoring. Treatment with hWJ-MSCs and hWJ-MSC-EVs significantly alleviated these symptoms. Mice receiving these treatments exhibited improved body weight, enhanced survival rates, and reduced lung fibrosis, with notable improvements in alveolar structure and decreased fibrotic tissue deposition.

Conclusions::

These findings highlight the potential of hWJ-MSCs and hWJ-MSC-EVs as therapeutic agents in treating pulmonary fibrosis by reducing inflammation and promoting lung tissue repair, offering a potential new avenue for regenerative therapy in severe lung diseases. Future research directions involve elucidating the molecular pathways involved in tissue repair, optimizing therapeutic delivery, and conducting comprehensive clinical evaluations.

Enrolled patients with newly diagnosed, Stage IIb with large mediastinal mass and/or extranodal lesions, Stage III or IV Hodgkin lymphoma were randomized to receive two cycles of either escalated BEACOPP or BrECADD, respectively, followed by interim PET staging. A decision is then made if patients received a further two or four cycles of escalated BEACOPP or BrECADD. The HD21 trial aims to evaluate a new treatment regimen to minimize side effects, while maintaining similar responses to treatment. The study has co-primary endpoints: safety is assessed by treatment-related morbidity (TRMB) (superiority), a novel endpoint focused on clinically relevant, acute toxicities of primary chemotherapy, and efficacy is assessed by PFS (non-inferiority). Secondary endpoints are tumor response (complete response [CR] rate), overall survival (OS), infertility rate at one year, second malignancies, frequency of adverse events, therapy adherence and quality of life. About ADCETRIS® (brentuximab vedotin) ADCETRIS is an antibody-drug conjugate (ADC) comprising an anti-CD30 monoclonal antibody attached by a protease-cleavable linker to a microtubule disrupting agent, monomethyl auristatin E (MMAE), utilizing Pfizer’s proprietary technology. The ADC employs a linker system that is designed to be stable in the bloodstream but to release MMAE upon internalization into CD30-positive tumor cells. ADCETRIS injection for intravenous infusion has received FDA approval for eight indications: Health Canada granted ADCETRIS approval with conditions for relapsed or refractory Hodgkin lymphoma and sALCL in 2013, and non-conditional approval for post-autologous stem cell transplantation (ASCT) consolidation treatment of Hodgkin lymphoma patients at increased risk of relapse or progression in 2017, adults with pcALCL or CD30-expressing MF who have had prior systemic therapy in 2018, for previously untreated Stage IV Hodgkin lymphoma in combination with doxorubicin, vinblastine, and dacarbazine in 2019, and for previously untreated adult patients with sALCL, peripheral T-cell lymphoma-not otherwise specified (PTCL-NOS) or angioimmunoblastic T-cell lymphoma (AITL), whose tumors express CD30, in combination with cyclophosphamide, doxorubicin, prednisone in 2019. Takeda receives EC approval for Adcetris combination in Hodgkin lymphoma

Takeda has received approval from the European Commission (EC) for its Pfizer-partnered blood cancer drug Adcetris (brentuximab vedotin). The drug has been approved in combination with the chemotherapy regimen etoposide, cyclophosphamide, doxorubicin, dacarbazine and dexamethasone (ECADD) to treat newly diagnosed stage 2b Hodgkin lymphoma (HL) in adults with risk factors/3/4. The authorisation follows a recent recommendation from the European Medicines Agency’s human medicines committee and was based on positive results from the phase 3 HD21 study. The trial achieved its co-primary endpoints, with the Adcetris regimen demonstrating significantly improved safety, as assessed by treatment-related morbidity, and non-inferior progression-free survival compared to escalated doses of bleomycin, etoposide, doxorubicin, cyclophosphamide, vincristine, procarbazine and prednisone (eBEACOPP), a standard of care in Europe. HD21 trial chairman, Peter Borchmann, University Hospital of Cologne, said: “With [the Adcetris regimen], patients now have a treatment option that not only offers greater curative potential but also significantly reduces treatment-related morbidity compared to eBEACOPP. “This new Adcetris-based combination therapy may offer a new standard of care for front-line treatment of adults with advanced stage HL, contributing to improved long-term outcomes for patients.” HL originates from abnormal lymphocytes and is distinguished from other types of lymphoma by the presence of Reed-Sternberg cells, which typically carry the CD30 protein on their surface. Takeda receives CHMP recommendation for Adcetris in Hodgkin lymphoma

Takeda has received a new recommendation from the European Medicines Agency’s human medicines committee for its Pfizer-partnered blood cancer drug Adcetris (brentuximab vedotin). The Committee for Medicinal Products for Human Use (CHMP) has recommended that the CD30-directed antibody-drug conjugate (ADC) be approved alongside etoposide, cyclophosphamide, doxorubicin, dacarbazine and dexamethasone (ECADD) to treat newly diagnosed stage IIb Hodgkin lymphoma (HL) in adults with risk factors/III/IV. The CHMP’s positive opinion was supported by results from the late-stage HD21 trial evaluating Adcetris plus ECADD against escalated doses of bleomycin, etoposide, doxorubicin, cyclophosphamide, vincristine, procarbazine and prednisone, a standard of care in Europe. The study met its co-primary endpoints, with the Adcetris regimen demonstrating significantly improved safety as assessed by treatment-related morbidity and non-inferior progression-free survival. HL is a relatively aggressive cancer that originates in the lymphatic system. It is distinguished from other types of lymphoma by the presence of Reed-Sternberg cells, which typically carry the CD30 protein on their surface. Adcetris, which is already approved in Europe for six indications, belongs to a class of cancer drugs that combine the selectivity of antibodies with the cell-killing properties of chemotherapy or other anti-cancer agents. Unlike chemotherapy, which works by attacking lots of different cells as well as the cancer, ADCs are designed to target and kill tumour cells while sparing healthy ones. The European Commission will now review the CHMP’s recommendation as it makes a decision on Adcetris in this indication. Takeda is responsible for Adcetris’ commercialisation everywhere except the US and Canada, where Pfizer holds commercialisation rights. The latest announcement for Adcetris comes less than a month after the National Institute for Health and Care Excellence (NICE) recommended in final draft guidance that Adcetris be used on the NHS alongside doxorubicin, vinblastine and dacarbazine to treat patients with untreated stage 3 or 4 CD30-positive HL. The decision made Adcetris the first drug to be recommended specifically for previously untreated late-stage classical HL and followed an “improved confidential discount” on the treatment’s list price offered by Takeda, NICE said.
