An Open-Label, Pilot Clinical Trial To Test The Safety And Feasibility Of Intestinal Microbiota Transplantation In Patients With Chronic Heart Failure

The purpose of this study is to determine whether people with CHF, who often have different gut bacteria from healthy, would benefit from replacing their gut bacteria with healthy donor bacteria (also known as Intestinal Microbiota Transplantation - IMT). IMT aims to restore healthy gut bacteria in patients with CHF, with previous studies showing its effectiveness, but further research is needed. IMT is an approved treatment for patients with infectious diarrhea. More than 10,000 IMTs are performed every year in the US. However IMT is not approved for patients with CHF, and thus considered investigational.

Evaluation of Microbiota Transplant Therapy in Patients With Alopecia Areata

Alopecia Areata (AA) is among the most highly prevalent human autoimmune diseases, leading to disfiguring hair loss due to the collapse of immune privilege of the hair follicle and subsequent autoimmune attack. AA affects about 5.3 million people in the United States alone, including males and females across all ethnic groups, with a lifetime risk of 2.1%. Autoimmunity develops against the hair follicle, resulting in non-scarring hair loss that may begin as patches that can coalesce and progress to cover the entire scalp (alopecia totalis) or eventually the entire body (alopecia universalis). In AA, there is no permanent destruction of the hair follicle, and regrowth remains possible. Treatment options for AA include intralesional steroids, topical anthralin, allergic contact dermatitis with diphencyprone (DPCP), dinitrochlorobenzene (DNCB), or squaric acid dibutyl ester (SADBE), and recently janus kinase ( JAK) inhibitors. Despite the recent approval of JAKs for the treatment of extensive alopecia areata, some patients are treatment resistant, suffer relapses, or cannot take an oral immunosuppressive medication.
This study will attempt to elucidate the pre-treatment and post treatment skin and gut microbiome composition to determine whether specific bacterial species may correlate with disease or treatment response. To determine the effects of MTT on immune cell composition and activation systemically and locally in the skin, we will analyze major immune cell populations in peripheral blood samples and collect skin biopsies for histopathology and next generation sequencing analyses. Further, to determine if changes in immune cell populations affect the inflammatory response, we will profile inflammatory cytokines. To identify if changes in the gut microbiota influence the metabolic signature in AA, we will also perform untargeted metabolomics in stool gut microbiome samples and in plasma. Altogether, this comprehensive approach aims to identify the pathogenic immunological mechanisms associated with microbiome composition correlated to pre-treatment disease, post-treatment response, and any non-responders to treatment.

From biowastes to risks? Impact of biosolids treatment and dose on antibiotic resistance in agricultural soils – A mesocosm study

Biosolids application to farmlands enhances soil fertility and promotes sustainable agriculture; yet it is vital to co-evaluate the interconnected risk of spreading antibiotic resistance throughout the agri-food sector. The impacts of sewage sludge treatments (dewatered, air-dried, limed, and thermally dried) and application rates (1-3%) on the abundance of antibiotic-resistant bacteria (ARB) and antibiotic resistance indices (ARI) in biosolids and biosolid-amended agricultural soils were tested. Thermal drying markedly reduced total heterotrophic and antibiotic-resistant bacterial abundance by nearly 5 log₁₀ units, resulting in the lowest ARI across multiple antibiotic classes, including ciprofloxacin and neomycin. Liming also effectively lowered some resistance indices through pH elevation and ammonia volatilization. Soil mesocosm experiments with 1-3% (w/w) biosolid amendment doses revealed that increasing biosolid application rates resulted in a dose-dependent increase in bacterial counts and ARI, especially for air-dried biosolids. ARB and ARI were generally lower in soils amended with limed or thermally dried biosolids compared to untreated or air-dried sludge, underscoring the importance of treatment intensity. Notably, antibiotic resistance was detected even in unamended control soils, highlighting a baseline resistance background. While biosolid amendments enhance soil fertility, insufficiently treated sludge or high application rates (greater than 1%) can promote the proliferation and spread of clinically relevant antibiotic resistance in soils. These findings highlight the crucial role of advanced biosolid processing and prudent application management in mitigating antimicrobial resistance risks across the agri-food ecosystem, aligning with the One Health framework for safeguarding human, animal, and environmental health.

The effects of drinking water with essential oils-organic acids blend replacing preventive antibiotics during withdrawal period on growth performance and gut health of broilers in a commercial farm

The maintenance of intestinal health is crucial for broilers during the withdrawal period. Substantial evidences have indicated that essential oil-organic acid complexes (EOAs) are effective in preserving poultry gut health. However, few studies were conducted to assess the effects of EOAs on production performance and gut health of broiler chickens in large-scale commercial broilers farms. The current study evaluated 0.15% EOA in drinking water as an alternative to preventive antibiotics in broilers before and during the mandatory withdrawal period (d 31-42) in a large-scale commercial broilers farm (16 rooms were randomly assigned to two equal groups with 8 rooms each group and 30,000 birds per room per replicate. The ANT group received neomycin sulfate in drinking water only pre-withdrawal (d 28-30), whereas the EOA group received 0.15% EOA from d 28 to 42, covering the entire withdrawal period. The results indicated that adding the essential oil-organic acid complex to drinking water during the withdrawal period significantly improved intestinal morphology by increasing the ratio of villus height to crypt depth (VH/CD) of jejunum and ileum (P < 0.05), increased Lactobacilli but decreased Escherichia coli and Salmonella population, lifted the activity of duodenal α-amylase and trypsin (P < 0.05), significantly downregulated tumor necrosis factor-α (TNF-α) mRNA levels and LPS content in the ileum and optimized intestinal microbial communities through enriching beneficial bacteria such as g_Ligilactobacillus, s Lactobacillus aviarius, s Ligilactobacillus salivarius, s Lactobacillus johnsonii, g_Pediococcus, and s Pedidcoccus pentosaceus in the cecum which were positively related with gut health biomarkers (P < 0.05) compared with the ANT group. While during the withdrawal phase ANT treatment notably increased Enterococcus cecorum, Burkholderia cepacia, Desulfovibrio relative abundance (P < 0.05) which were positively correlated with gut injury index. Thus, our findings indicated that EOA could be used as an effective alternative to preventive antibiotics to improve growth performance and maintain gut health during the drug-free period.

Neomycin-sensitive gut bacteria-derived brassicasterol mediates the anti-obesity effects of Cordyceps militaris polysaccharide

Diet-based modulation of the gut microbiota has emerged as a promising strategy to alleviate obesity and its related complications. Our previous study demonstrated that polysaccharide derived from Cordyceps militaris (CMP) exerts anti-obesity effects, yet the specific mechanism linking gut microbiota to its metabolic impact remains unclear. Herein, we utilized murine models with distinct gut microbial profiles created via antibiotic cocktails to investigate these mechanisms. The protective effects of CMP against high-fat diet (HFD)-induced obesity and associated metabolic disturbances were substantially impaired in mice depleted of neomycin-sensitive gut bacteria. Metagenomic analyses further established that CMP required these bacteria to restore gut microbial homeostasis. Notably, we observed that CMP elevated hepatic levels of brassicasterol in a manner dependent on neomycin-sensitive gut bacteria. Brassicasterol treatment alone replicated the anti-obesity effects of CMP, as indicated by reduced body weight gain, improved lipid and glucose metabolism, and decreased inflammation. Through transcriptomic and functional analyses, we identified hepatic Apoa4 as a key downstream effector of brassicasterol. Our results indicated that brassicasterol upregulated Apoa4, facilitating lipid transport and suppressing inflammation both in vitro and in vivo. Collectively, our findings indicate that CMP exerts its anti-obesity effects through a neomycin-sensitive gut bacteria-brassicasterol-Apoa4 pathway. This work expands the mechanistic understanding of CMP and highlights a novel microbiota-metabolite-host regulatory axis for dietary intervention in metabolic disorders.
