Efficacy of a portable BLT device on sleep and depressive symptoms in major depressive disorder: A double-blind randomized study

BACKGROUND:

Major depressive disorder (MDD) is a leading cause of disability worldwide and is often associated with anxiety and sleep disturbances. About one-third of patients do not achieve adequate response to pharmacological treatments, underscoring the need for adjunctive non-pharmacological therapies. Bright light therapy (BLT) has shown efficacy in MDD, but conventional devices require patients to remain stationary, limiting adherence. Portable BLT devices may improve convenience and daily integration. This study assessed the efficacy of a portable BLT device on mood, anxiety, and sleep over 8 weeks.

METHODS:

LUMIDEP was a double-blind, placebo-controlled trial. Adults with MDD were randomized to receive active portable BLT or a visually identical placebo. Final analyses included 18 patients (BLT n = 6; placebo n = 12). Patients underwent 30-min daily sessions for 8 weeks while continuing pharmacological treatments. Depressive symptoms were assessed using the Montgomery-Åsberg Depression Rating Scale (MADRS), anxiety with the Hamilton Anxiety Scale (HAM-A), and sleep with the Pittsburgh Sleep Quality Index (PSQI) and Epworth Sleepiness Scale (ESS).

RESULTS:

Baseline PSQI scores were higher in the BLT group. After 8 weeks, PSQI scores improved within the BLT group, while remaining stable in the placebo group, resulting in no significant between-group difference. No significant differences were observed between groups in HAM-A or MADRS.

CONCLUSIONS:

Portable BLT appeared to improve sleep quality but did not significantly reduce depressive or anxiety symptoms. Interpretation is limited by group differences, treatment heterogeneity, and small sample size. Larger, stratified trials are warranted to clarify the efficacy of portable BLT in MDD.

The placebo effect on depressive symptoms and brain age in patients with depression: a reanalysis study of a randomized controlled clinical trial on the antidepressive effect of bright light therapy

BACKGROUND:

Depression is associated with accelerated brain aging. Bright light therapy (BLT) shows promise in treating depression and alleviating cognitive deficits, but it remains unclear if BLT can improve brain age and if clinical benefits are related to changes in brain age. This study evaluates the impact of BLT on brain age in depression and explores the relationship between symptomatic improvement and changes in brain age.

METHODS:

This study reanalyzed data from a double-blind, randomized controlled trial comparing BLT to dim red light (dRL) therapy. Scores from the Hamilton Depression Rating Scale-24 (HAMD-24) and the nine-item Patient Health Questionnaire (PHQ-9) were recorded at baseline and 4 weeks after therapy. Brain MRI scans estimated individual brain age gap (BAG), which was the difference between predicted brain age and chronological age. BAGs were calculated for global and cognition-specific domains (executive function, memory, language, and vision). Changes in clinical scores and BAG were assessed within and between groups, including analyses based on symptom improvement.

RESULTS:

The study included 21 participants in the BLT group and 18 in the dRL group. Both groups showed improvements in clinical scores without significant between-group differences. Although there were no group differences in BAG, the combined group had a significantly younger BAG in global, vision, and language domains after intervention. Additionally, patients with improvement in PHQ-9 scores showed reduced BAG in global brain and executive function domains and greater improvement in PHQ-9 was correlated with a younger executive function brain age.

CONCLUSION:

No specific effect of BLT on brain age was observed compared with the control group. However, reductions in global and executive function brain age were associated with subjective symptom improvement, suggesting a potential role of placebo effects or neuroplasticity. These findings highlight the importance of considering subjective measures and brain aging biomarkers in evaluating antidepressant interventions.

Anaesthetics in aquaculture: comprehensive insights into agents, mechanisms, and applications

Anaesthesia is a cornerstone of modern aquaculture practice playing a crucial role in routine fish husbandry procedures and research activities. The present review discusses about anaesthetic use in fish, including commonly used synthetic agents such as MS-222, benzocaine, metomidate, quinaldine, 2-phenoxyethanol, and propofol, together with emerging plant-derived anaesthetics, especially clove oil and other essential oils that have reduced environmental impact. Attention is drawn to the physiological basis of anaesthesia in relation to fish welfare, inclusive of both sedation and general anaesthesia, emphasizing their role in mitigating stress. The review outlines the classical stages of anaesthesia in fish and summarizes important physiological and biochemical responses associated with the use of various anaesthetics. Pharmacokinetic processes, including absorption, primarily across the gills, tissue distribution, metabolism, and excretion are also discussed. Regulatory regimes governing approved anaesthetics are summarized to highlight compliance requirements in aquaculture settings. Particular attention is paid to the use of anaesthetics during transport, where light sedation is shown to effectively decrease metabolic rate thereby significantly improving fish welfare. Finally, the review points towards emerging future directions including the development of eco-friendly plant-based anaesthetics and nanotechnology-enhanced formulations aimed at improving solubility, stability, and efficacy of anaesthetic agents.

Alliance Pharma agrees to £362m acquisition by DBAY

UK-based Alliance Pharma has agreed to a final increased cash offer from asset management company DBAY Advisors, valuing the former at approximately £362m ($467.5m). The offer, made through a newly formed company called Aegres Bidco Limited, will see DBAY acquire all shares in Alliance, except those it already owns through its affiliates, for 64.75p per share. This represents a 3.6% increase from DBAY’s initial offer of 62.5p per share in January 2025 and a 46% premium on Alliance’s share price as of 9 January 2025, the day before the offer period began. Alliance Pharma specialises in the marketing and distribution of consumer healthcare products and prescription medicines. Its portfolio includes Kelo-Cote, a scar treatment; and Anbesol (benzocaine), an oral pain relief product. DBAY initially approached Alliance last year, following a series of challenges faced by the company, including the delayed publication of its annual results and the resignation of its CEO Peter Butterfield. Having gradually increased its stake in the company over two years, DBAY now holds a 27.9% share of Alliance. In its initial takeover offer, DBAY stated that it believed Alliance had “strong future prospects” but required significant operational changes and increased investment to maximise its growth potential. The asset management company argued that such changes would be more effectively implemented away from the pressures of the public market. In January 2025, Alliance’s board of directors recommended shareholders accept the initial offer of 62.5p per share. However, after receiving feedback from shareholders, the scheduled general meeting last month was adjourned to allow for further discussions. According to a 10 March filing on the London Stock Exchange, DBAY will fund the acquisition through a combination of equity investments from its affiliated funds and external financing, with a small portion of shares in Midco, the parent company of Bidco, being offered to certain lenders. Shareholders are expected to vote on the proposal at a rescheduled meeting on 13 March 2025. If approved, the deal is anticipated to be completed in the first half of 2025, resulting in Alliance Pharma becoming a privately owned company under DBAY’s control. In September 2024, Alliance reported underlying revenue of £84m for the six months to 30 June 2024, reflecting an increase from £81.4m in the same period the previous year. Profit before tax also rose year-on-year, reaching £12.7m compared to £10.3m in 2023.

Govt bans 156 medicines including antibiotics, painkillers, multivitamins: Here is the full list

New Delhi: In a significant move aimed at safeguarding public health, the Indian government has banned 156 fixed-dose combination (FDC) drugs, including a variety of antibiotics, painkillers, multivitamins, and medications for treating fever and hypertension. This decision marks the largest crackdown on FDCs since 2016, when 344 such drugs were prohibited. The Union Ministry of Health and Family Welfare issued a gazette notification on Thursday, officially prohibiting the manufacture, sale, and distribution of the banned FDCs. The ban was based on the recommendations of an expert panel that reviewed a total of 324 FDCs. Impact on Pharmaceutical Industry The banned FDCs include popular combinations such as mefenamic acid and paracetamol injections, commonly used for pain relief and fever, and omeprazole magnesium with dicyclomine HCl, used to treat abdominal pain. Insight Pharmaceuticals Issues Voluntary Nationwide Recall of Americaine® 20% Benzocaine Topical Anesthetic Spray Due to the Presence of Benzene

TARRYTOWN, N.Y., Dec. 22, 2023 (GLOBE NEWSWIRE) -- Insight Pharmaceuticals, a Prestige Consumer Healthcare Inc. company (“Insight”), is voluntarily recalling one lot of Americaine® 20% Benzocaine Topical Anesthetic Spray to the consumer level. While benzene is not an ingredient in any Americaine® 20% Benzocaine Topical Anesthetic Spray products, a recent review by our manufacturer and their third-party lab found that a sample from one lot of the product showed a low level of benzene came from the propellant that sprays the product out of the can. Risk Statement: Benzene is classified as a human carcinogen. Exposure to benzene can occur by inhalation, orally, and through the skin and it potentially can result in cancers including leukemia and blood cancer of the bone marrow and blood disorders which can be life threatening. Insight is recalling these products out of an abundance of caution. To date, the Company has not received any serious adverse events related to this recall. Benzene is ubiquitous in the environment. Humans around the world have daily exposures to it indoors and outdoors from multiple sources. Americaine® 20% Benzocaine Topical Anesthetic Spray is packaged in white aerosol cans with Teal caps with Lot codes located on the bottom of the can. Samples of the recalled lot below have been found to contain trace levels of Benzene related to the propellant that sprays the product out of the can. Benzene is not an ingredient in any Americaine products. Insight has notified retailers via overnight mail to remove any remaining recalled product from shelves and follow the instructions provided in the Drug Recall Notification. The company will also offer reimbursement for consumers who have purchased Americaine® 20% Benzocaine Topical Anesthetic Spray marked with the lot code in the table above. Consumers can contact Insight Pharmaceuticals via e-mail at medicalaffairs@prestigebrands.com, through its website at , or by phone at (800) 344-7239 on Monday – Friday 8:30-5:30 eastern time to receive a full refund by providing a picture of the bottom of the can of the Americaine® 20% Benzocaine Topical Anesthetic Spray with the affected lot number. Consumers that have product which is being recalled should stop using the product immediately and appropriately discard after taking the picture. Consumers should contact their physician or healthcare provider if they have experienced any problems that may be related to taking or using this anesthetic product. No serious adverse events have been reported to date. Adverse reactions or quality problems experienced with the use of this product may be reported to the FDA's MedWatch Adverse Event Reporting program either online, by regular mail or by fax.
